Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to conduct an Annual Required Evaluation. LPA was greeted and granted entry by a resident at 8am. LPA spoke with staff and met with Administrator (AD) Camille Santos and explained the purpose of the visit. The facility is a single story, five bedroom, two bathroom residential home with an approved fire clearance of six ambulatory residents; of which three may be non-ambulatory and one may be bedridden. The facility has an approved hospice waiver for three residents. Currently there are six residents in care with one resident on hospice. At time of entry, residents were waiting for breakfast and one was being showered. During today's visit LPA toured the facility and tested the smoke and carbon monoxide detectors. Both smoke and carbon monoxide detectors are operational and are connected with other alarms throughout the facility. The facility has an entrance gate and an enclosed courtyard. The detached garage had a working washer and dryer and emergency supplies. Fire extinguishers were charged and replaced on January 18, 2026. The facility's last fire drill was conducted on July 21, 2026; in which they reviewed the Emergency Disaster Plan. LPA observed both the See Something, Say Something and Long Term Care Ombudsman posters. Resident Council, Personal Rights, Client and Staff rosters, Infection Control Plan and Theft and Loss were posted in a prominent location with facility Emergency Disaster Plan and facility hours and contact information. The facility has current liability insurance and licensing fees are also paid. (Continued on LIC 809-C) (Continued from LIC 809) The facility was 75 degrees Fahrenheit. LPA toured the kitchen and observed the facility had more than two days of perishable food and seven days of non perishables on hand. LPA and staff reviewed expiration dates on condiments. Kitchen stove was in working order; as well as other kitchen appliances. Sharps and knives were secured in a locked drawer and chemicals/cleaning supplies were locked beneath the kitchen sink. Hot water temperatures were measured in two of two resident bathrooms and measured between 114.4 to 116.4 degrees Fahrenheit. Both bathrooms have walk-in showers with non skid flooring and grab bars. LPA observed resident rooms were clean and had the required furnishings and linens. Side rail orders were documented in resident files. LPA toured the exterior of the property and observed a shaded seating area and both exit gates were self-latching. LPA observed a staff member and resident walking around the perimeter and there is ample room for activities. LPA and AD reviewed the centrally stored medications. Per medication review, medications are being given as prescribed. A First Aid Book is available and the First Aid Kit had all of the required elements. LPA reviewed three of three staff training and fingerprint records and conducted a complete review of resident records. LPA interviewed alert residents regarding their quality of care and spoke to staff present regarding care provided. LPA confirmed that administrator has a current administrator certificate which expires on May 6, 2028. Based on the observations made during today’s visit, the facility appears to be in compliance with Title 22 Division 6 of the California Code of Regulations, no deficiencies cited on this date. On September 25, 2025, Licensing Program Analyst (LPA) Garlli Tat conducted an unannounced visit to the facility for the purpose of a required annual inspection. LPA explained the purpose for the visit and was greeted and granted entry by staff on duty. During the visit, staff on duty contacted the facility administrator (AD) Camille Santos about the visit. For this visit, there are three staff members on duty, all of which are background cleared and associated. AD later arrived to assist with the inspection. The PUB475 ‘See Something, Say Something’ poster was observed to be located by the front entrance. LPA observed the Administrator's Certificate for Camille Santos, which expires on May 6, 2026. The facility is a Residential Care facility for the Elderly (RCFE) licensed for six residents, three of which may be non-ambulatory, one may be bedridden, and three may be on hospice. LPA toured the interior and exterior portions of the facility with AD. For this visit, there are a total of two non-ambulatory residents in care, one of which is on hospice. The facility is a single-story home. There are a total of five bedrooms, two of which are private resident rooms, two are shared resident rooms, and one serves as staff quarters. LPA toured each bedroom with the AD and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and free of any hazards. LPA observed the staff quarters to be kept locked and inaccessible to residents in care. Smoke and carbon monoxide detectors as well as auditory exit alarms were tested and operational. There are a total of two bathrooms, both of which are for residents and staff. Continued on LIC 809-C. Bathrooms were observed to be in good repair, toilets and faucets were operational and showers were equipped with grab bars and non-skid floor mats. Water temperature in the bathrooms were measured to be between 113.7 and 113.9 degrees Fahrenheit. Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Two food items were observed to exceed expiration dates. AD disposed of items immediately during the visit. Sharp items and knives were locked in the kitchen and inaccessible to residents in care. Fire extinguisher were charged, mounted and located in the kitchen and the living room. Fire extinguishers were dated and tagged on February 4, 2025. LPA observed the emergency disaster and evacuation plan, which is posted in the kitchen. Facility had back-up emergency food and water supply, located in the garage. LPA observed that the First Aid kit had all the required components. Medications were observed to be locked in a cabinet in the kitchen. Toxins were also observed to be locked in a cabinet below the sink in the kitchen, inaccessible to residents in care. For the exterior portion, LPA observed patio furniture under shading, and the grounds were free of any hazards or obstructions. There are two self-latching gates in the backyard that can be opened in case of an emergency. No bodies of water were observed. Two out of two resident files and three out of five employee files were reviewed during this visit. Based on today's observations, there is one deficiency being cited per Title 22 of the California Code of Regulations. An exit interview was conducted with Camille Santos. Licensing Program Analyst (LPA) Lydia Martinez made an unannounced visit for the purpose of conducting a Required 1 Year inspection. LPA met with Caregivers Clark John Culala and Gerald Alarcon and purpose of visit was shared. Assistant Administrator (AD) Veahlou Daelto arrived shortly after. AD Daelto informed LPA Martinez that Kennellie Ferguson is the Licensee/Administrator but facility is going through a change of ownership. LPA Martinez, along with staff toured the inside and outside of the facility. Facility currently has five residents in care of which two are receiving Hospice services. LPA observed residents in their perspective rooms either asleep or watching TV. All residents were observed clean and well taken care of. LPA observed the facility to be clean and in good repair. The home is maintained at a comfortable temperature. Lighting is sufficient for safety and comfort. Backyard has a covered patio for outdoor activities and sufficient seating for residents and visitors. Bedrooms were observed to be spacious and easily accommodate furnishings such as lamps, chair, dresser and a bed. Bathrooms were observed to be clean and have a supply of soap and paper towels. Hot water temperature was within regulatory requirements. Linen and hygiene supplies were stocked. Emergency Phone Numbers and Exit Plan were reviewed. Food prep area is clean and organized. Food supply meets the requirement of one (1) week supply of non-perishable and two (2) day supply of perishables. Smoke detectors and carbon monoxide detectors were found to be operational. Fire Extinguishers were charged and mounted. Stove burners, dishwasher, microwave, washer, and dryer are operational. Chemicals and sharps are made inaccessible to the residents. Medication was observed locked in a kitchen cabinet. Medications reviewed appear to have been dispensed accurately. LPA observed the Emergency Disaster Plan posted. LPA reviewed 5 resident files and 2 staff files; and interviewed residents and staff. LPA confirmed Administrator's certificate expires on 10/06/2024. Based on observations made during today's visit, no deficiencies are being cited. Allegation investigated: Facility is improperly administering medication resulting in a resident having to be admitted to the hospital.

Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced visit to deliver findings on an investigation completed by the Department. LPA was greeted and granted entry into the facility by Caregiver Arcy Vinuya and explained the reason for the visit. House Manager Veahlou Daelto was present during the visit. During course of the investigation, the Department interviewed staff and witnesses as well as reviewed and obtained pertinent documentation including Physician Report, Resident Service Plan, and St. Joseph Hospital Medical Records. The purpose of today’s visit is to follow up on an investigation conducted by the Department regarding the above allegation. The investigation conducted revealed the following: Resident 1 (R1) was admitted to the facility on 09/29/2021. Physician report dated September 20, 2021, notes that R1 is unable to manage their own medications. On December 16, 2023; December 23, 2023; and December 29, 2023, R1 sustained medical emergencies requiring hospitalization related to their sodium levels which was being managed by a physician prescribed medication. CONTINUED ON LIC 9099 DATED 06/13/2024 Unsubstantiated Interviews with three of three staff reported that caregivers would hand resident’s their medications at the appropriate times. Staff interviewed reported they would fill out the medication forms whenever they would deliver medications to a resident. A review of R1’s medication records showed no abnormalities. An interview conducted with a medical professional who attended to R1 during one of their hospitalization's denied that low sodium levels or high doses of R1’s medication could cause seizures. When asked if there were concerns about the facility providing R1 incorrect medication dosages, they went on to say R1’s condition was very complicated, and that there were many factors contributing to it. Although R1 sustained multiple hospitalization's while in the care of the facility, a review of records obtained and interviews conducted determined that R1’s hospitalization's were not sustained as a result of neglect by the facility staff. Therefore, based on interviews conducted and documents reviewed, the allegation that facility is improperly administering medication resulting in a resident having to be admitted to the hospital is deemed to be Unsubstantiated, meaning that although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. Licensing Program Analyst (LPAs) Kimberly Lyman conducted an unannounced visit for the purpose of a Plan of Correction (POC) visit based on deficiencies cited on 01/08/2024. LPA was greeted and granted entry into the facility and explained the reason for the visit. *Deficiency cited under Title 22 Regulation 87608(a)(5)(B) pertaining to Postural Supports has been cleared. Full bed rails have been removed. Licensee has complied with the terms of the POC. Licensee received an advisory note on 01/08/2024. LPA observed the following: Sharps are secured during today's visit. All half rails have been moved to the top of the beds. There is ample emergency water and canned goods. Licensee has been advised to maintain all items in compliance with Title 22 regulations. Type of visit: Case Management - Deficiencies

Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced health and safety visit in conjunction with complaint visit 22-AS-20240104163024. LPA was greeted and granted entry into the facility and explained the reason for the visit. At 2:00 PM, LPA toured the facility and observed the following: Facility appears clean and sanitary with ample food supply and operational appliances. There are three residents in care during today's visit. All residents appear clean and taken care of. Residents are relaxing in rooms or common areas during the visit. At 2:05 PM, LPA observed full rails on Resident 1's (R1) bed. R1 is not on hospice care. Facility has all required postings. LPA observed resident medications as well as toxins and cleaning supplies are secured. Facility has ample outdoor area with appropriate shade. Based on the observations made during today's visit, the following violation is being cited per California Code of Regulations, Title 22, Division 6, Chapter 8.
