Allegation investigated: Staff do not properly maintain the facility grounds

On 04/01/2026, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility for the purpose of delivering complaint findings. LPA Pascua met with Facility Designated Representative (FDR), Igor Zakhardeuich and explained the purpose of the visit. There was one other staff member present, Adiya Reimbek. LPA Pascua spoke with Licensee, Haykush Harutyunyan and went over the report. Current census was 6. A brief interview with Haykush Harutyunyan was conducted. It was alleged that the staff do not properly maintain the facility grounds. During the course of this investigation, LPA Cynthia Tamayo conducted a facility tour of this facility and observed that the backyard did have an overgrowth of grass. A photo was taken during this visit. Based on observation of this photo, it was observed that the backyard had signficant overgrowth of grass and it appears thick, dense, and uneven, with the tops of the grass reaching several inches high. In some areas the grass was clumping and laying over itself. Based on the information gathered, the staff did not properly maintain the facility grounds Substantiated As a result of this investigation, this LPA found the allegations to be SUBSTANTIATED - A finding that the complaint was Substantiated meant that the allegation was valid because the preponderance of the evidence standard had been met. The following deficiencies were cited on the following LIC 9099-D pursuant to Title 22 Rules and Regulations, Division 6 and Health and Safety Codes. Exit Interview As a result of this investigation, this Department found the allegations to be UNSUBSTANTIATED. A complaint allegation finding of Unsubstantiated meant that although the allegations may have happened or was valid, there was not a preponderance of the evidence to prove that the alleged violation occurred. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors. This is not met as evidenced by: The licensee did not ensure that maintanence of the yard and overgrowth of grass was maintained overtime. Plan of correction: Haykush Harutyunyan contact a new vendor ensure that facility grounds at maintained at all times. Cleaning of the facility grounds will be conducted within the next two weeks. A picture shall be sent to LPAs email.

Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Christina Valerio arrived to the facility to conduct an annual required inspection. LPA Valerio met with facility staff, and explained the purpose of the visit. LPA Valerio was later met by Licensee Haykush Harutunyan and Qiao 'Vicky' Jiao. LPA Valerio observed two Caregiver staff on shift. LPA Valerio and facility staff toured the facility to ensure compliance with Title 22 regulations. LPA Valerio observed resident bedrooms to be fully furnished, free from odors, and clean. LPA Valerio observed residents sleeping in their room, being visited by family, and conducting a doctor appointment with assistance with staff. Resident bathrooms were observed to be clean, fully stocked with hygiene supply, and sanitary. The faucet was measured to deliver hot water within the regulatory range of 105.0 - 120.0 degrees. The facility was observed to have an adequate food supply. The facility thermostat was set to a comfortable temperature. All emergency exits were free from obstructions. LPA Valerio observed two (2) resident files and two (2) staff files. Files were observed to be up to date with required annual documentation. LPA Valerio also reviewed the death report that was submitted to the Regional Office on 02/18/2026. No health or safety concerns observed. Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to conduct an annual inspection. LPA Moleski met with facility administrator Vicky Jiao and explained the purpose of the visit. LPA Moleski reviewed six resident files (R1-R6) and three staff files (S1-S3). LPA Moleski observed that R6 is bedridden, according to their most recent LIC 602, dated 3/9/24. R4's most recent LIC 602, dated 12/13/24, indicates that they are non-ambulatory. A prior LIC 602 dated 11/25/24 indicated that R4 was bedridden. Jiao said that R4 currently needs to be turned and repositioned in bed to prevent bed sores. Health and Safety Code Section 1569.72(b)(1) defines a bedridden resident as one "requiring assistance in turning and repositioning in bed." This facility is cleared for six non-ambulatory residents, and is not cleared to accept or care for any bedridden residents. LPA Moleski toured the facility with Jiao and inspected common areas, the kitchen, bedrooms, bathrooms, and backyard areas. Furniture and furnishings were sufficient to meet the needs of residents. The facility temperature was 73 degrees Fahrenheit, which is within the required range of 68 and 85 degrees. The facility's water temperature measured 115 degrees Fahrenheit, which is within the required range of 105 and 120 degrees. LPA Moleski observed first aid supplies, a fully-charged and up-to-date fire extinguisher, and carbon monoxide/smoke detectors. LPA Moleski observed a minimum 2-day supply of perishable food and a minimum 7-day supply of nonperishable food. LPA Moleski observed a locked closet for the storage of medication. LPA Moleski observed locked cabinets for the storage of cleaning solutions and knives. LPA Moleski interviewed one staff member (S2) and one resident (R5). This facility is hereby cited per 22 CCR Section 87202(a)(2). An immediate civil penalty in the amount of $500 is hereby assessed due to a fire clearance violation. The facility temperature was 74 degrees Fahrenheit, which is within the required range of 68 and 85 degrees. The facility's water temperature measured 111 degrees Fahrenheit, which is within the required range of 105 and 120 degrees. This facility is cleared for six ambulatory residents and five non-ambulatory residents. All six residents are non-ambulatory, according to their LIC 602s. Additionally, LPA Moleski observed both gates leading out of the back yard locked during this visit. Staff removed the locks. LPA Moleski observed first aid supplies, fully-charged and up-to-date fire extinguishers, and working smoke detectors. LPA Moleski observed a minimum 2-day supply of perishable food and a minimum 7-day supply of nonperishable food. LPA Moleski observed a locked cabinet for the storage of medication.
