On July 08, 2026, at 2:15 PM, Licensing Program Analysts (LPAs) Avelina Martinez and Sulma Lopez made an unannounced visit to this facility to conduct an annual required inspection. LPAs met with Janet Juntilla and explained the purpose of today's visit. LPAs inspected the physical plant including but not limited to the kitchen, dining room, resident bedrooms; resident bathrooms, laundry room, activity room, and outside courtyards of the facility to ensure compliance with Title 22 regulations. Administrator holds current certificate. The facility is licensed for six non-ambulatory residents, one of which can be bedridden. There are currently six residents who reside at this facility. The facility has an approved hospice waiver for six residents. One out of six hospice waivers are being used. The LPAs toured the facility with the staff on 07/08/2026 at 3:00 PM. LPAs reviewed two resident files and two staff files and all files were current and maintained. LPAs reviewed two Medication Administration Records (MAR) which were current and maintained. The facility has an infection control plan and a disaster preparedness plan. The last emergency drill was on June 12, 2026. The facility kitchen is sanitary and has an adequate food supply. The facility common areas, shared bathrooms, and activities area were furnished and sanitary. The facility has a public phone available for residents. LPAs inspected all 6 resident bedrooms and they were furnished and sanitary. LPAs inspected the exterior of the facility. The emergency exit gate is in good repair and clear of debris. The facility has a furnished and shaded patio for resident use. The facility water temperature measured at 105 degrees and the facility temperature measured at 72 degrees. Based on this year's annual inspection, there were no deficiencies cited. Allegation investigated: ) Staff was sleeping while providing care and supervision 2) Staff did not respond timely to a resident's alerts

Licensing Program Analyst (LPA) Kevin Gould conducted an unannounced complaint inspection at Greenstar RCFE on 9/3/25 at to inform the licensee of complaint allegations mentioned above. During this investigation LPA Gould interviewed The RP, S1 and S2 (See confidential names list, LIC 811 dated 9/3/25). Based on the interviews and statements obtained during the investigation process, the allegations are substantiated. LPA obtained corroborating statements and documentation confirming the staff member present during an overnight shift fell asleep shortly after arriving for an overnight shift and not being aware a resident was in need for staff assistance. Immediately after being made aware of staffing needs the Licensee ensured a new staff member arrived to relieve staff member within 20 minutes. Staff member was reportedly exhausted from a combination of school and work schedules. Per staff records, staff member was drug tested with all results negative. Facility has made changes to staff schedule to ensure all staff members are awake and well rested to ensure the needs of residents are being met. Report Continued on LIC 9099-C. Substantiated The Department has determined, based on the preponderance of the evidence obtained during this investigation, that the allegations of personal rights are substantiated. On 5/16/25 at 9:00am Licensing Program Analyst (LPA) Kevin Gould arrived at Greenstar RCFE for the purpose of conducting a required 1 year annual inspection. LPA met with staff, Aldi Raranta and together conducted a tour of the home. LPA and staff evaluated the physical plant to ensure the health and safety of the residents in care. Areas inspected are including but not limited to the kitchen, resident bedrooms; resident bathrooms, living and dining room and outdoor areas. LPA observed the facility to be free of odor, clean and in good repair. LPA observed that all rooms are equipped with the required furniture and sufficient lighting throughout the facility. LPA measured the water temperature, temperature measured at 111 degrees F which meets the 105-120 degree Fahrenheit regulation. LPA observed sufficient seven day non-perishable and two day perishable food supplies. Fire extinguishers and smoke detectors are current and in compliance with fire safety. LPA notes the facility had the required carbon monoxide detectors. First aid kit was checked and is complete. LPA observed centrally stored medications secure from residents. LPA observed some boards on the back deck are beginning to lift up and pose a potential tripping hazard. LPA observed previous repair efforts that facility is addressing back deck repairs timely. Per California Code of Regulations, Title 22 there were no deficiencies cited during today's inspection. LPA observed medications pre-poured and advised facility staff of the medication guidance changes that removed the ability to pre-pour medications. LPA Requested the following documents for facility file: LIC 308 Designation of Facility Responsibility, LIC 500 personnel report, LIC 9020 client roster and current administrator certificate. Per California Code of Regulations, Title 22 there were no deficiencies cited during today's inspection. Allegation investigated: Other: Staff's conduct poses a risk to residents in care.

Licensing Program Analyst (LPA) Kevin Gould made an unannounced inspection to Grenstar RCFE on 12/28/23 at 1:15pm to conclude the investigation of the above allegations and to deliver the findings. LPA met with staff member Jeanne Bayan and together discussed the investigation details. Based on the interviews conducted and statements obtained during the investigation process, the allegations cannot be corroborated because the staff member who is alleged to have an out of state criminal activity record has never worked at this facility. Per staff interview and interviews with the licensee, both parties denied the allegation and provided statements and documentation that the identified staff member, although associated to all facilities owned by the licensee, has never worked at this location. The Administrator agreed to create a guardian account with CCLD and disassociate the staff member who never worked at the facility to ensure this type of allegation does not reoccur. Unfounded The Department has investigated the complaint alleging other. Based on the investigative interviews, record reviews and other supportive evidence, the complaint is determined to be unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. The Complaint has been dismissed. There are no deficiencies noted or cited per California Code Regulation, TITLE 22. Exit interview was conducted with the facility administrator.
