Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Martha Arroyo arrived at the facility unannounced to conduct the required annual visit today at 09:30 a.m. Upon arrival, the LPA was greeted by the Administrator, Maria Rosario Tanglao and the reason for the visit was explained. Entrance interview conducted. Beginning at 09:40 a.m., the LPA along with the Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards, and facility is in compliance with Title 22 Regulations. The following was observed: Common Areas: The furniture in the living room, dining room, and sunroom area was observed to be in good condition at the time of the visit. The facility maintained a comfortable temperature. The LPA observed a fireplace in the living room; adequately covered. The LPA observed activity supplies in the common areas accessible to residents in care. The LPA observed a fire extinguisher to be fully charged with a charge date of 07/14/2026. Required postings were observed throughout the common space. There is a working telephone on premises. Working cameras were observed in the common areas. No hazards/obstructions observed inside or out. Restrooms: There are two (2) full restrooms and a 1/2 bathroom for resident use. Bathrooms were clean and fixtures were in operating condition. Showers had non-skid surfaces and grab bars. The bathrooms were sufficiently stocked with soap and paper towels. Report Continued on LIC 809C... Report Continued from LIC 809... Starting at 09:43 a.m., the hot water temperature was measured in resident bathrooms, and they measured between 112.5 and 116.2 degrees Fahrenheit which is within the required range of 105 – 120 degrees Fahrenheit. Bedrooms: There are five (5) bedrooms for resident use. Four (4) bedrooms are designated as private / single occupancy and one (1) bedroom is designated as shared / double occupancy. Bedrooms were observed to be furnished appropriately and had sufficient lighting. Kitchen: The LPA inspected the kitchen/food service area at approximately 09:35 a.m. Knives and sharps were observed in a locked closet inaccessible to residents in care. Kitchen appliances were in operable condition. The facility has a sufficient supply of perishable and non-perishable food. Refrigerator and food pantry were checked for proper labels and expiration dates. Garage: The garage is attached to the house and was locked at the time of the visit. Washer and dryer were observed inside the garage. Detergents and cleaning supplies were observed locked and inaccessible to residents in care at the time of the visit. The LPA observed an adequate amount of emergency food and water. Personal hygiene items were observed locked and inaccessible at the time of the visit. Additional clean linens and towels were observed inside the garage. Outdoors: The backyard has a patio umbrella with adequate furniture for resident use. Emergency passageway was observed to be clear of any obstructions. There is a side gate with latching mechanisms for emergency purposes. The LPA observed a locked shed for storing purposes. No bodies of water noted at the time of the visit. Report Continued on LIC 809C... Report Continued from LIC 809C... Records: Record review began at approximately 10:10 a.m. Four (4) resident files were reviewed for, but not limited to, the following: signed admission agreements, current medical assessments with TB results, consent for treatment form, preplacement appraisals, appraisals, and current needs and services plan. All files were in order. Four (4) personnel files were reviewed for, but not limited to: personnel record, health assessment with negative TB test results, criminal record clearance, first aid/CPR training, and the appropriate yearly training. All personnel files were complete. The Administrator’s certificate is valid until 11/28/2026. Medications: The LPA conducted a medication review at approximately 12:20 p.m. Medications are centrally stored and kept in a locked closet by the hallway. All medications including PRNs were labeled, stored, and locked inaccessible to residents in care. Medications appear to be administered as prescribed at the time of the visit. Emergency Disaster Planning: During today’s visit, the LPA reviewed the facility's emergency disaster plan, which was observed to be complete and recently reviewed. Emergency disaster drills conducted quarterly as per regulation; last disaster drill conducted on 07/04/2026. No citations issued at this time. Exit interview conducted. Jul 8, 2025Facility evaluation reportReport on file
Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Erica Mosley arrived at the facility unannounced to conduct a required annual visit and entered the facility at 10 a.m. Upon arrival, LPA Mosley was greeted by staff and the Administrator and informed them of the visit. LPA met with Administrator, Maria R.E. Tanglao and the reason for the visit was explained. Entrance interview. The LPA toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. COMMON AREAS: The facility is a single-story structure with a large sun room, dining room and living room. At the time of the visit, furniture in the common areas was observed to be in good condition. The facility maintained a comfortable temperature. At 12:50 p.m., hardwire combination of smoke / carbon monoxide detector were tested and operational at the time of the visit. The fire extinguisher was observed and fully charged on 07/18/2024 and scheduled for service on 07/10/2025. The emergency telephone numbers are posted in common hallway. The LPA observed required postings throughout the common space. The last emergency disaster drill took place on 06/01/2025 and are conducted monthly. Activities were observed in the common areas. The fireplace in the living room was adequately screened. There is a functioning telephone on the premises. LPA did not observe any obstructions or hazards. KITCHEN: The LPA inspected the kitchen/food service area at 10:14 a.m. Knives and sharps were observed in a locked lock box in a bottom drawer. Kitchen appliances were in operable condition. The facility has a sufficient supply of two (2) day perishable and seven (7) day non-perishable food. Refrigerator and food pantry were checked for proper labels and expiration dates. The kitchen faucet was measured for hot water temperature, and it measured 119.4 degrees Fahrenheit at 10:15 a.m. REPORT CONTINUED ON LIC 809-C PAGE 2... (PAGE 2) REPORT CONTINUED FROM LIC 809... INTERVIEWS: Starting at 10:37 a.m. one (1) staff and five (5) resident interviews were conducted. Staff interview revealed that staff are knowledgeable in Resident rights, different forms of abuse, and reporting procedures. Resident interview revealed that no concerns were noted or voiced at the time of the visit. BEDROOMS: There are five (5) total bedrooms in the facility; all bedrooms are designated as private, single occupancy, resident rooms. All resident rooms were observed to be furnished appropriately with clean linens, appropriate furnishings, and sufficient lighting. No staff room. Licensee provided proof of 24 hour staffing. RESTROOMS: There are two in a half (2.5) restrooms. One in a half (1.5) are designated as shared / common restrooms and one (1) is designated as a private resident restroom. Resident restrooms were observed to be equipped with a nonslip surface. Grab bars were observed in the bathrooms. The restrooms were sufficiently stocked with supplies and paper towels. The water temperature was measured all resident restrooms and ranged between 113.9-114.1 degrees Fahrenheit, all within the required range. OUTDOOR SPACE: The LPA observed the back yard which had a shaded patio area with patio furniture including a table and chairs for resident use. The LPA observed one gate that self-latches with a clear passageway in case of an emergency. There are no bodies of water on the premises at the time of the visit. LPA observed a locked storage unit with supplies and outdoor equipment that remains locked at at all time. GARAGE: The garage is not accessible to residents. Laundry units, cleaning solutions, toxins, chemicals, and hazardous items are kept in the locked laundry room/ garage. There was a linen cabinet in the garage with extra towels and linens. Administrator's office is also set up in the garage. The garage is kept locked and inaccessible. RECORDS: Record review began at approx. 10:50 am. Resident Records were reviewed beginning at 10:50 a.m. and Personnel Records at 11:50 a.m. Five (5) Resident files were reviewed for, but not limited to, the following: signed admission agreements, current medical assessments with TB results, LIC627(c) Consent for Treatment form, and current needs and services plan. All records were in order. Four (4) Personnel files including the Administrator’s file were reviewed for, but not limited to: personnel records, health assessments, criminal record clearances, first aid/CPR training, and the appropriate training. All records were in order. REPORT CONTINUED ON LIC 809-C PAGE 3... (PAGE 3) REPORT CONTINUED FROM LIC 809 PAGE 2... Infection Control / Emergency disaster planning: During today’s visit the LPA reviewed the facility’s infection control practices and the facilities emergency disaster plan. The facilities policies and procedures as it pertains to infection control and emergency planning are satisfactory. MEDICATIONS: Medications review began at approximately 2:15 p.m. The medications are in a locked medication closet adjacent to the kitchen. Medications for four (4) residents were reviewed. Medications are labeled and checked for expiration dates. All medications including PRNs were labeled, stored, and locked inaccessible to residents in care. Medications reviewed were found to be self-administered as prescribed and documented on the centrally stored medication and destruction records. No errors observed during review. LPA obtained the following documents - Resident roster - LIC 9020, Staff roster - LIC 500, and a copy of the Limited Liability insurance. During today's inspection, the facility is in compliance with Title 22 regulations. No citations issued. Exit interview conducted. Type of visit: Required - 1 Year
Licensing Program Analysts (LPA) Zabel Chochian arrived at the facility unannounced to conduct a required annual visit. Upon arrival LPA was greeted by staff. LPA introduced self. Staff contacted Administrator Maria Tanglao who arrived shortly and reason for the visit was explained. At approximately 12pm, the LPA and Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. COMMON AREAS: The facility is a single-story structure with a large sun room, dining room and living room. The LPA did not observe any obstructions or hazards. At the time of the visit, living room, sun room and dining room furniture was observed to be in good condition. There is a fireplace in the living room, which is screened and inaccessible. The facility maintained a comfortable temperature of 74 degrees. Smoke detector(s) and carbon monoxide detector were tested and operational at the time of the visit. The LPA observed (2) fire extinguishers throughout the facility which were last serviced 07/18/2024. KITCHEN: Kitchen/food service area observed to be clean and sanitary. Knives and cleaning supplies are stored inaccessible. Kitchen appliances observed to be in operable condition. The facility had a sufficient supply of two day perishable and seven day nonperishable food items. BEDROOMS: Bedrooms were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. There are five designated resident rooms. No staff room therefore licensee shall provide 24 hour staffing. Licensee shall submit an updated LIC500 to show 24 hour staffing schedule. RESTROOMS: Resident restrooms (2.5) observed clean and sanitary and in operating condition with grab bars and non-skid surfaces. The bathrooms were sufficiently stocked with soap and paper towels. OUTDOOR AREA: The backyard has outdoor area equipped with furniture for client use, a patio table with an umbrella. Storage unit in the back; side gate with exit and single latch. No bodies of water noted. GARAGE: The attached garage observed setup with two separate sleeping/resting area for staff use. Ms. Tanglao stated that staff who live in use the garage area to rest/sleep overnight. Licensee/Administrator was informed that no individual may sleep in the garage area overnight. The washer and dryer are also in the garage, including emergency supply food items. Cleaning supplies and disinfectants are kept in the garage. There was a linen cabinet in the garage with extra towels and linens. Administrator's office is also set up in the garage. The garage is kept locked and inaccessible. RECORD REVIEW: Residents’ records review began at approximately 12:45 p.m., records were reviewed for, but not limited to care plans, medical records, admissions agreement, consent forms. All required records were observed in the resident files. At approximately 1:30 p.m., Personnel records were reviewed for, but not limited to health assessments, criminal record clearances, first aid/CPR training, and the appropriate training. Staff training records observed incomplete; missing training topics; unable to verify. Extensive time was spent on staff training records; consultation with Licensee/Administrator. Licensee/Administrator will review each staff training records and update accordingly to show specific training topics; training hours complete - time. Training records must be signed by the trainer and staff receiving the training. Licensee/Administrator and staff confirmed that emergency drills are conducted quarterly however not recorded as part of the training. Moving forwarded Licensee/Administrator stated that staff training provided and completed will be recorded accordingly as required by law.
