Licensing Program Analyst (LPA) Blanca Gonzalez conducted an unannounced required annual visit. LPA was greeted by staff and the reason for the visit was explained. Licensee Rowena Santos arrived shortly after to assist. The facility is licensed to serve age range 60 and over, six (6) non-ambulatory of which one (1) may be bedridden and an approved hospice waiver for six (6). Bedroom #3 is for the bedridden resident. There are currently two (2) residents receiving hospice services. The facility is a single-story home located in a residential area of Glendora. The home consists of a kitchen, living room, dining area, two (2) bathrooms, five (5) resident bedrooms, an attached garage with laundry, front and backyards. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following: The front and backyard are well maintained and there are no pools or large bodies of water. There is a covered patio providing a shaded seating area located in the backyard with sufficient seating for residents in care. Passageways and exits are free of obstructions. Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to residents, were observed to be inaccessible to residents. continued on LIC 809C Combination smoke and carbon monoxide detectors were observed in the hall and in each resident bedroom. There are two (2) fire extinguishers located in the kitchen and living room. Emergency drills conducted quarterly, last drill was 05/03/26. The hot water temperature was tested in bathrooms and measured 114.9°F, which is within the required 105° F – 120° F. LPA observed grab bars and non-skid flooring in both the bathrooms. Resident bedrooms have the required furniture and have sufficient closet and storage space. Resident beds have the required linen and the linen is in good repair. Kitchen was observed to be clean and free of pests. Kitchen appliances are clean and were operating at the time of the visit. Sharps are locked in a kitchen drawer and are inaccessible to residents. Cleaning supplies and disinfectants are locked in a kitchen cabinet and are inaccessible to the residents. Sufficient supply of 2 days perishable and 7 days non-perishable foods were observed. Dining area observed clean with sufficient seating for residents in care. Attached garage has laundry area. Washing machine and dryer wear clean and operable at the time of visit. There is a cabinet with laundry supplies, inaccessible to clients. LPA reviewed five (5) resident files. All files were observed to contain the required admission agreement, medical assessment with TB clearance, medical consent, pre-appraisal, functional capabilities needs and service plan, and personal rights. Centrally stored medication is located in a locked cabinet located in the living room. LPA reviewed three (3) staff files. All files were observed to contain the required personnel record, health screening with TB clearance, current CPR certificate, annual training, and fingerprint clearance. Per California Code of Regulations, Title 22, and California Health and Safety Code, no deficiencies were cited during today’s visit. Licensing Program Analyst (LPA) Sanjay Vaid and Elena Mallett conducted required annual inspection. LPA met with Caregiver Nicholas Loterena and was allowed entry into the facility discussed purpose of today’s visit. Rowena Santos Licensee arrived shortly after. This facility is licensed to serve six (6) non-ambulatory of which one (1) may be bedridden. This facility may retain no more than six (6) hospice residents. There are three (3) residents under hospice care currently. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following: Physical Plant and Environment safety: LPA Vaid and Mallett observed the following: disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to residents, were observed to be inaccessible to residents. Carbon monoxide detectors and smoke alarms in hallways. Inspected five (5) resident rooms. All resident bedrooms contained required furniture, linens and lighting. Water temperatures in all grooming and bathing areas were measured to be with 105 – 120 degrees F. Observed postings encouraging proper handwashing etiquette in restrooms. Observed grab bars near toilets and inside showers. LPA’s observed no-slip mat in showers. Showers were observed to be wheelchair accessible. Food Service: LPAs Vaid and Mallett observed sufficient supply of non-perishable for one week and perishable foods for a minimum of two days in the facility kitchen area. Soaps, detergents, and cleaning compounds were observed to be stored away from food supplies. Freezers and refrigerators were observed to be clean. Continued on 809C............ Planned Activities: Per facility staff, residents receive seated exercises during the week. Staff plays a video and leads residents in different exercises. Residents Rights-Information: LPA Vaid and Mallett observed the following postings in common areas throughout the facility: Complaint Poster (PUB 475), personal rights, and nondiscrimination notice. LPAs observed facility land line. Disaster Preparedness: The facility has the Emergency Disaster Plan (LIC610D) in place. LPA Vaid and Mallett observed facility sketches with exits and emergency exits routes throughout various locations of the facility. LPA Mallett observed emergency food supply and water supply. Residents with Special Needs No large bodies of water were observed. LPA Vaid and Mallett observed signs posted indicating “No smoking - Oxygen in Use” in various locations of the facility. LPA Vaid observed several oxygen tanks in resident rooms secured in stands. Knives, sharps or other items that could pose a danger to residents with dementia, were observed to be inaccessible. Auditory devices and delay egress perimeters were observed to be in working order. Health Related Services/Incidental Medical Services: The medications are centrally stored in the medication rooms and in bubble packs and/or original containers. The facility uses the Medication Administration Record (MAR) log to document medications given. The facility provides incidental medical services. Staffing: Administrator Certificate for Luis Santos expires 12/08/2025. Staff employed are over the age of 18 and are fingerprint cleared and associated to the facility. Personnel Records Training: Staff files are maintained at the facility. LPA observed required annual training, CPR and First Aid. Three (3) personnel records reviewed. LPA observed TB testing results, Health screening, and fingerprint clearance for three (3) personnel records reviewed. Infection Control: There are using appropriate hand hygiene and wearing gloves while assisting clients. Staff are cleaning and disinfecting often for high touched surfaces. Facility has an Infection Control Plan in place. Operational Requirements: The fire clearance is approved for six (6) non-ambulatory of which one (1) may be bedridden. This facility may retain no more than six (6) hospice residents. Resident Records/Incident Reports: LPA reviewed Resident files for six residents. Resident files are maintained at the facility. Admission Agreement, Physician's Report (including T.B and Ambulatory Status), and Preplacement Appraisal. No deficiencies were observed. Exit interview was conducted.
