On 06/03/2026, Licensing Program Analyst (LPA) M Vega arrived unannounced at the above facility to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was granted entry to the facility by Staff. LPA toured the facility with facility staff. AD was notified of Licensing visit over the phone and was able to attend the visit. LPA met with Assistant (A) - Cynthia Minor The facility was observed to be at a comfortable temperature, of 78 degrees F. Facility is free of debris, in good repair, and no passageway obstructions or fire hazards were observed. Common areas were properly furnished and well-lit throughout. LPA observed some residents in common area watching television, others in their rooms resting. Department phone number and infection prevention information signs were posted thought the facility. Inspecting kitchen LPA observed the required 7-day supply of non-perishable food and 2-day supply of fresh perishables to be properly stored. An emergency disaster supply was observed. A fire extinguisher was observed with a service date of 05/15/2026. All four (4) private residents’ bedrooms were observed to be at comfortable temperatures. Medications observed to be locked in a cabinet in the common area. Cleaning supplies were observed to be in a locked cabinet in the storage it the laundry room. An outdoor seating area was observed operational for residents in care. AD was notified of Licensing visit over the phone and was able to attend the visit. The facility was observed to be at a comfortable temperature, of 78 degrees F. Facility is free of debris, in good repair, and no passageway obstructions or fire hazards were observed. Common areas were properly furnished and well-lit throughout. LPA observed some residents in common area watching television, others in their rooms resting. Department phone number and infection prevention information signs were posted thought the facility. Administrator (AD) Juli Hergenroeder was notified of Licensing visit over the phone and was able to attend the visit. AD certification number 6004551740 and expiration date 05/14/2025. The facility was observed to be at a comfortable temperature, of 78 degrees F. Facility is free of debris, in good repair, and no passageway obstructions or fire hazards were observed. Common areas were properly furnished and well-lit throughout. LPA observed some residents in common area watching television, others in their rooms resting. Department phone number and infection prevention information signs were posted thought the facility. LPA reviewed medication records appears to be administered properly. Cleaning supplies were observed to be in a locked cabinet in the storage it the laundry room. Allegation investigated: Staff hit resident in care. Staff did not provide adequate supervision resulting in resident sustaining injuries while in care. Staff does not administer resident's medications as prescribed. Staff does not dispose resident's expired medications. Staff does not ensure resident's bathing needs are being met. Staff does not allow residents to finish meals.

This is amended of the original report. On 05/22/2024, Licensing Program Analyst (LPA) V. Gorban visited the facility to deliver findings. During this visit LPA met with facility staff Beatriz and stated the purpose of the visit. AD was notified of Licensing visit. During this visit LPA toured the facility inside and out and observed residents in care. Once the tour was complete, LPA discussed the findings. Allegation: Staff hit resident in care. Based of history file review and observations and staff interview no incident reports of staff hit residents reported or documented. Residents interviewed stated they did not witness staff abuse residents. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is Unsubstantiated. Based of observations on 2/22/24 and 4/19/24, interviews and records reviews, the facility providing adequate supervision to residents in care. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is Unsubstantiated. Allegation: Staff does not administer resident's medications as prescribed. Based of records review R2 refused to take her medications . Licensing department and R2's physician was notified. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is Unsubstantiated. Allegation: Staff does not dispose resident's expired medications. Based of observations on 2/22/24, interview and records review, no expired medications observed. Per staff interview, facility administrator disposes expired medications. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is Unsubstantiated. Allegation: Staff does not ensure resident's bathing needs are being met. Based of interview and records review residents receive bath as scheduled. Per interviews, alternative bath provided to residents unable to stand and residents always resume the rights to refuse baths. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is Unsubstantiated. Allegation: Staff does not allow residents to finish meals. Based of observations of residents during meals on 2/22 and 4/19/24, and interviews facility residents were not rushed or denied finishing meals. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is Unsubstantiated. On 04/19/24, Licensing Program Analyst (LPA) V. Gorban visited the facility to deliver findings. During this visit LPA met with facility staff Jermaine and stated the purpose of the visit. During this visit LPA toured the facility inside and out and observed residents in care. Once the tour was complete, LPA discussed the findings with the Administrator. Allegation: Correct medication is not being given at correct times. During complaint investigation the department interviewed facility staff, residents and reviewed facility files. Based of file review resident was provided medications as prescribed. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated. Report continues on LIC9099-C Unsubstantiated Allegation: Staff is isolating and neglecting resident. R1 stated staff makes them eat meal isolated in a bedroom. Based of observation, interviews and record reviews facility serves meals in main dining room where all residents share meal time. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated. On 04/19/2024, Licensing Program Analyst (LPA) Gorban conducted a case management in response to requested facility files for complaint investigation office requested on 2/23/2024 LPA Gorban met with facility Administrator Juli Hergenroeder and discussed with administrator the following.
