On 08/04/2026 Licensing Program Analysts (LPAs) Raquel Hernandez and Matthew Aguilar conducted an unannounced case management visit to follow up on complaint 56-AS-20260605120020. LPAs were granted entry to facility by Staff Joann Salem. LPAs met with Administrator Bene Molinitas and explained the purpose of today's visit. During today's visit, LPAs received pertinent information in regard to complaint. LPAs conducted facility tour and did not observe any health or safety concerns. Licensing Program Analyst (LPA) Raquel Hernandez made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPA met with Administrator Bene Molinitas and was granted entry to the facility. Licensed capacity is (6) current census (6). LPA was accompanied by Administrator Bene Molinitas to conduct a general overall inspection, which included, but was not limited to, the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature. LPA inspected resident bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPA observed sufficient furniture and lighting throughout the facility. The facility is equipped with operating smoke detectors and carbon monoxide alarms. Posters such as personal rights, the CCL complaint poster, and the disaster plan were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to residents in care. There was a designated office for resident/staff files. Overall, the facility is clean, in good repair, and operating in safe conditions for residents in care. Food Service: Non-perishable and perishable food supply is sufficient for number of residents in care. Facility has a variety of food available for residents. Dishes, cups, and utensils were also stored properly. Care & Supervision: Facility has sufficient care staff for coverage 24 hours a day, 7 days a week. Record Review: LPA reviewed (3) resident files for admission agreements, updated physician reports, ,pre-placement appraisals and needs and services plans. LPA reviewed (3) resident medications. LPA observed no proper documentation and or administration of daily medications for all (3) residents during medication audit. Deficiency will be issued. Additionally, LPA observed for R2 and R3 no proper documentation for PRN medication. No date and resident's response noted. Deficiency will be issued. LPA observed Resident #2 (R2) has full bed rails with no current hospice or doctor order. Deficiency will be issued. LPA also reviewed (3) staff files for First Aid/CPR certification, criminal record clearance, training's, and health screenings. No issues were observed. Based on the observations made during today’s visit, deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. Licensing Program Analyst (LPA) Sarina Ramirez made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Adminstrator Restituto Calilung, and discussed the purpose of the visit. The facility is a Residential Care Facility for the Elderly (RCFE) with a license capacity of (6), a current census of (5) residents in care and a hospice waiver for (6). LPA Ramirez conducted an overall inspection of the facility, which included, but was not limited to, the following: Physical Plant/Environment: Indoor and outdoor passageways are free of obstruction. The facility has a swimming pool gated and locked inaccessible to residents in care. Outdoor shaded area is sufficient for resident activities and is enclosed with self-latching gate. The facility has sufficient lighting and is maintained at a comfortable temperature. The facility has operating carbon monoxide alarms and telephone service. Resident’s showers, toilets, and hand washing areas were operating in a safe and sanitary conditions. The hot water temperature in thee (3) resident bathrooms measured 105,106, and 108 degrees F. Five (5) resident’s bedrooms had beds, bed linen, chairs, storage space and sufficient lighting. The facility has sufficient linens, towels, and personal hygiene items for residents. The facility has posted in a common area, complaint poster, Ombudsman poster, facility license, disaster evacuation plan, emergency telephone numbers, food menu, and activities. Food Service: Facility kitchen and dining areas are maintained clean. The facility has sufficient non-perishable and perishable food supply for residents in care. Sharps were kept unlocked and accessible to residents in care, technical violation cited. Continuation on LIC – 809C: Care & Supervision: Facility has 24-hour/7days a week care staff. Facility staff have current CPR/first aid training. Medical Related Services: Resident’s medications are labeled and centrally stored in a locked cabinet, Technical Assistance issued. The facility has sufficient first aid supplies. Record Review: The Administrator's certification is current. The facility’s last fire drill was conducted on 4/2/24, and last earthquake drill was conducted on 7/2/24. Three (3) Staff files reviewed were observed to be complete. Three (3) Resident files reviewed were observed to be complete. Based on observations and record review, technical violation and technical assistance will be issued per Title 22, of The California Code of Regulations.
