Licensing Program Analysts (LPA) Daiquiri Boyd arrived unannounced to conduct the Annual Inspection. LPA met with and explained the reason for the visit with Administrator (AD) Teresa Long. During this visit, LPA toured the facility inside & out. Resident rooms contained required furnishings and lighting. The bathrooms were found to be clean with faucets delivering hot water at 114.6 degrees. LPA observed required hygiene items, towels, extra bedding, and linens which were stored in the hall closet and available for use. The kitchen was clean, with necessary items and appliances, all sharp objects were in a locked cabinet in dining area. LPA observed required food supply and paper product storage. Medications are locked and centrally stored in a locked kitchen pantry closet. Common and activity areas were clean and occupied by residents throughout. There are visitation areas available inside and out. Doors and passageways are unobstructed throughout the facility. The fire extinguisher was serviced 08/05/2026. Temperature in the home was 76 degrees. Smoke/carbon monoxide detectors were seen throughout the home and smoke alarm was tested and working. LPA reviewed 3 resident files and 3 staff files. All files had required documents. LPA requested Licensee to submit the following documents: LIC500, LIC9020 No deficiencies were cited on this day. Licensing Program Analyst (LPA) Daiquiri Boyd conducted the complaint investigation visit to the facility. During this visit LPA delivered investigation findings regarding the above allegations. The Department has investigated allegations: Facilty staff did not answer the phone in a timely manner and did not respond to call bells in a timely manner. Based on the interviews conducted and/or records review the above allegation is UNSUBSTANTIATED. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated. Allegation investigated: Staff did not provide adequate supervision to residents in care

On 1/15/2026, Licensing Program Analyst (LPA) Daiquiri Boyd conducted a subsequent complaint investigation visit to the facility. LPA met with Teresa Long and explained the reason for the visit. During the course of this complaint investigation LPA conducted interviews, obtained and/or reviewed facility and resident records (LIC602A), and reviewed and obtained copy of video recordings from inside the facility from the night of the incident. Interviews conducted and videos reviewed show resident (R1) had left their bedroom, used the restroom and was wandering the facility halls and living area between the general hours of 1 a.m. and 2:30 a.m. Resident made a phone call from the house phone to the daughter in an attempt to seek assistance. Resident (R1) has diagnosed medical condition, as documented on LIC602A. It was determined, based on interviews, videos obtained, and records reviewed, that the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. 1569.312(a) - Basic Services Requirements-Every facility required to be licensed under this chapter shall provide at least the following basic services: (a) Care and supervision as defined in Section 1569.2. This requirement was not met as evidenced by the statements of witnesses and the video footage obtained from inside the facility showing R1 walking throughout the facility starting at 1:15AM until placing a call to daughter at 1:44AM, opening the alarmed front door for her daughter at 2:06AM. Night Staff was not seen by R1’s daughter that night or on submitted video, until seen at 2:23AM after the Night Shift Supervisor arrived. The floating Night Shift Supervisor arrived at the home between 2:06AM and 2:23AM, as they were seen on video at 2:23AM with the Night Staff as EMS arrived at the facility; which poses an immediate risk to the health, safety, or personal rights risk to the residents in care.the state’s words, verbatim · CDSS document, Jan 15, 2026

LPA reviewed six resident files and three staff files. All files had required documents. LPA requested Licensee to submit the following documents: LIC500, Proof of Liability Insurance.
