Type of visit: Required - 1 Year

On 9/30/25 at 9:15am Licensing Program Analyst (LPA) Kevin Gould arrived at EZ Care RCFE for the purpose of conducting a required 1 year annual inspection. LPA met with Licensee, Khaula Nixon and together conducted a tour of the home. There are currently no residents in care. the facility is in process of making alterations to facility and will submit a change for fire clearance to accommodate non-ambulatory and bedridden residents. LPA and Administrator evaluated the physical plant to ensure the health and safety of the residents in care. Areas inspected are including but not limited to the kitchen, resident bedrooms; resident bathrooms, living and dining room and outdoor areas. LPA observed the facility to be free of odor, clean and in good repair. LPA observed that all rooms are equipped with the required furniture and sufficient lighting throughout the facility. LPA measured the water temperature, temperature measured at 111 degrees F which meets the 105-120 degree Fahrenheit regulation. LPA notes the facility had the required carbon monoxide detectors. First aid kit was checked and is complete. LPA observed centrally stored medications secure from residents. Per California Code of Regulations, Title 22 there were no deficiencies cited during today's inspection. Type of visit: Prelicensing

On 9/12/24 at 1:30pm Licensing Program Analyst (LPA) Kevin Gould arrived at EZ Care for the purpose of conducting a pre-licensing inspection. LPA met with Applicant Khaula Nixon and current licensee and administrator Modori Katuka and together conducted a tour of the home. There are currently no residents in placement. LPA confirmed the following corrections have been made: All light fixtures are working and operational. Back bedroom has a bed and all bedrooms and common areas have had personal items removed. LPA observed a new lock on medications storage cabinet. At the time of inspection, the applicant has met all requirements to be licensed. Type of visit: Prelicensing

On 8/30/24 at 9:00am Licensing Program Analyst (LPA) Kevin Gould arrived at EZ Care for the purpose of conducting a required 1 year annual inspection. LPA met with Applicant Khaula Nixon and current licensee and administrator Modori Katuka and together conducted a tour of the home. There are currently no residents in placement. LPA and Administrator evaluated the physical plant to ensure the health and safety of the residents in care. Areas inspected are including but not limited to the kitchen, resident bedrooms; resident bathrooms, living and dining room and outdoor areas. LPA observed the facility to be free of odor and in good repair. LPA observed that one of the bedrooms requires a bed. All other required furniture was present in the facility. LPA observed a few light bulbs out and in need of replacement throughout the facility. all bedrooms and common areas bust be cleared of personal items not utilized for resident use or care. LPA measured the water temperature, temperature measured at 111 degrees F which meets the 105-120 degree Fahrenheit regulation. LPA observed sufficient seven day non-perishable and two day perishable food supplies. Fire extinguishers and smoke detectors are current and in compliance with fire safety. LPA notes the facility had the required carbon monoxide detectors. First aid kit was checked and is complete. LPA observed centrally stored medications area but requested a new lock as it appeared to be easily bypassed. The following corrections are required prior to being licensed by the department: All light fixtures to be working and operational. Back bedroom requires a bed and all bedrooms and common areas need to have personal items removed. New lock on medications storage cabinet. Per California Code of Regulations, Title 22 the facility does not meet all regulations to be licensed at the time of inspection. COMP II by CAB successfully completed Facility Type: RCFE Application Type: CHOW Capacity: 6 Census: 2 Method: Telephone call with CAB COMP II Participants: Midori Kakuta (Administrator), Khaula Nixon (Licensee) & Tammy Edwards, (Analyst). Licensee & administrator participated in COMP II at CAB via telephone call with analyst at CAB. Identification of the Licensee and administrator was verified by confirming driver’s license number. During COMP II, licensee and administrator confirmed the understanding of Title 22. Component II was successfully completed. Licensee and administrator were advised to email signed LIC 809 with copy of photo ID to CAB. During COMP II, CAB analyst confirmed Licensee & Administrator’s understanding of following areas: 1. Facility operation: License type, client/resident populations, and program 2. Admission Policies 3. Staffing requirements & Training 4. Restrictive/Prohibited Health Conditions 5. General provisions 6. Emergency Preparedness 7. Complaints & Reporting 8.
