Type of visit: Annual/Random

On 08/17/2026, Licensing Program Analyst (LPA) Lavinia Muscan, arrived at the facility unannounced to conduct a 1-Year Required Annual Inspection. LPA met with Facility Administrator Heather Craig and explained the purpose of the visit. LPA and Administrator toured facility together to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, resident bedrooms, hallways, common restrooms, and outside area. LPA observed the facility to be clean, in good repair and odor-free. Facility has a 2-day perishable and a 7-day non-perishable amount of food and sharps to be locked. The hot water temperature measured within the required range of 105-120 degrees. LPA observed facility's fire extinguishers, fire detectors, and carbon monoxide detectors. In the areas toured no immediate health, safety, or personal rights violations were observed. LPA reviewed a total of five (5) residents' files and five (5) staff files. There were no deficiencies cited at this time. LPA requested a copy of the LIC 500, LIC610E and current liability insurance to be sent to the Department. Exit interview conducted. Allegation investigated: Licensee did not ensure staff met criminal record clearance requirements prior to working at the facility.

On December 8, 2025, Licensing Program Analyst (LPA) Lavinia Muscan arrived at the facility unannounced to deliver complaint findings into the allegation listed above and met with Administrator Melisa Tiburcio. During the investigation, the Department conducted interviews and reviewed documentation pertinent to the investigation. The results of the investigation are as follows: Based on documents obtained facility did not ensure that a criminal record clearance was obtained for S1 in 2022, when S1 was working at the facility. Record review indicated that S1 started working on April 13, 2022, and did not get the fingerprint clearance until February 2, 2023, which poses an immediate health, safety, or personal rights risk to persons in care. The preponderance of evidence standards has been met. Therefore, the above allegation is found to be SUBSTANTIATED. Per California Code of Regulations, Title 22, Division 6, Chapter 8, a deficiency is being cited on the attached 9099-D page. Immediate civil penalty of $500.00 was issued today. Failure to correct shall result in civil penalties. Appeal Rights given to administrator. A copy of this report has been provided to facility. Exit interview conducted. Plan of correction: Facility will complete a statement of understanding regarding criminal background clearances and ensure that staff obtain a criminal background clearance before providing services to residents. Allegation investigated: Staff violated resident’s personal rights by not allowing her to leave facility

On 8/5/24, Licensing Program Analyst (LPA) Lavinia Muscan arrived at the facility unannounced to deliver complaint findings into the allegations listed above and met with Administrator Melisa Tiburcio. During the investigation, the Department conducted interviews and reviewed documentation pertinent to the investigation.The results of the investigation are as follows: Through the course of the investigation process, CCL conducted interviews, toured the facility, and reviewed records regarding the allegation above. During the investigation, it was determined that the care provider was trying to provide care and supervision to R1 based on the resident's needs and restraining order documents on file. The facility responded based on the most current documents they were provided. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, and the findings are Unsubstantiated.
