Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Angela Hood arrived at the facility unannounced and met with the Executive Director, Cheryl Martin, to conduct a Required-1 Year Inspection. LPA conducted an inspection of the care home to ensure compliance with Title 22 regulations. LPA observed six (6) bedrooms in assisted living, two (2) bedrooms in memory care, and nine (9) common area bathrooms. LPA observed apartments to be properly furnished, with appropriate bedding and lighting. The bathrooms were in sanitary condition and properly maintained. The hot water temperature was observed to be 118.4 degrees F in assisted living and 116 degrees F in memory care. LPA checked the kitchen area for the ability to prepare and store food. Care home has required (2) two-day perishable and (7) seven-day non-perishable food supply on hand. LPA observed knives, cleaning products and other toxins to be locked away and inaccessible to residents. LPA observed the outdoor area and perimeter of the care home to be free of clutter and debris and there appeared to be no potential safety hazards to the residents in care. Smoke and carbon monoxide detectors are operational. Fire extinguishers and first aid kits are maintained and ready for emergency use. The facility has emergency evacuation chairs in each stairwell. LPA checked medication storage and found medications to be locked away and inaccessible to the residents. LPA reviewed six (6) resident files and also reviewed six (6) staff files. As a result of this visit, no deficiencies were cited per California Code of Regulations, Title 22. Type of visit: Case Management - Incident

On 12/16/25, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit and met with the Executive Director(ED) . On 12/15/25, the department received an incident report regarding a theft of R1's belongings. The theft was investigated by facility staff and the local police department. The investigation found that S1 had stolen a ring belonging to R1. The ring was recovered and S1's employment was terminated. During today's visit, LPA received employment records for S1, interviewed R1 and the ED. LPA finds that the licensee followed required procedures and that this was an individual's illegal action. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Kevin Mknelly arrived at the facility unannounced on 1/2/24 to conduct a Annual Inspection utilizing the CARE inspection tool. LPA met with the Executive Director (ED)/ Administrator and explained the purpose of the visit. . LPA and ED toured the interior and exterior of the facility together with staff to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, resident bedroom, medication rooms, kitchen and dining, and The Harbor (Memory Care section). In the areas toured no immediate health, safety, or personal rights violations were observed. The facility is clean and well maintained. Residents observed appeared to be assisted as needed with an appropriate number of staff present. The licensee utilizes an electronic records system. LPA was assisted by management team members to review required documentation. LPA reviewed 5 resident files. Files were complete. LPA interviewed 4 residents. R1 resident is in process of a exception request, by licensee, for a health condition. 5 Staff files were reviewed. Files are complete. LPA received a copy of liability insurance certificate.
