Licensing Program Analyst (LPA), Robert Campbell arrived at the facility unannounced to conduct the Required Annual Inspection and met with Ma Satchel Lecita - Administrator. The facility file review was conducted in the Regional Office and additional forms were requested and reviewed on site. The facility is licensed for six (6) Elderly Adults. LPA toured the facility along with Administrator, Ma Satchel, and made observations pertaining to the one year required annual. The facility was inspected inside and outside, there were no obstructions or debris to the indoor or outdoor passageways currently. The facility is a one-story home, five (5) bedrooms, three (3) bathrooms. Physical Plant: The facility phone number is (951) 878-7027 and is currently operable. LPA observed all six (6) residents’ bedrooms, and each was equipped with required furniture as per Title 22. LPA inspected the two facility bathrooms, and the hot water temperature tested within regulations (114 degrees Fahrenheit). The bathrooms were clean, and appliances were operating appropriately currently at the time of this visit. The facility is equipped with operating smoke detectors, carbon monoxide alarms, and fire extinguishers (8/10/2026). LPA observed required postings such as "If you See Something, Say Something" the "Personal Rights." The cleaning supplies and sharp items were kept locked and inaccessible to the residents in care. There was a designated storage space for the residents and staff files, and it was locked and inaccessible to residents in care currently at the time of this visit. Medications: Were locked and inaccessible to residents in care, and there was a sufficient supply of medication for each resident. The medications were not observed to be pre-filled during this inspection, and the Fire and/or Disaster Drills were logged and conducted quarterly. (Continued to 809-C) Continued... The overall facility is clean, the furniture is in good condition. The facility cooling system and other appliances were operable currently at the time of this visit, and there were safety lights for night throughout the facility, along with a first aid kit and manual. Food Service: Non-perishable and perishable food supply is sufficient per regulations, and there are a variety of food types available for residents. Dishes and utensils were in sufficient supply and stored properly, and sharp items are locked. Food shopping for groceries once a week or as needed. Care & Supervision: Facility has sufficient staff, four (4)staff on site at the time of this visit for the current residents in care. Ma Satchel Lecita, Administrator's license is posted in the facility with an expiration date of 10/31/2027. Records Review: The LPA reviewed five (5) residents and five (5) staff files, five (5) Resident files Previous Community Care Licensing forms were reviewed. There were no Title 22, Division 6 Regulation violations observed or cited during today’s visit. Type of visit: Prelicensing

On 07/16/25 Licensing Program Analyst (LPA) Javina George made an announced prelicensing inspection. LPA met with Applicant Dante Bautista whom accompanied LPA throughout the inspection. On 05/06/25 the Riverside County Fire department granted a fire clearance for six (6) non ambulatory residents only. The applicant has applied for a hospice waiver for (4). The facility is a single story structure that consists of (1) shared resident bedroom (4) private resident bedrooms with (2) resident bathrooms and(1) staff bedroom and bathroom for the live in caregivers, kitchen, dining area, laundry room, backyard and detached garage. The bathrooms were observed to have grab bars, and non skid mats. The hot water measured within regulatory limits at 114.6 degrees Fahrenheit. All resident bedrooms have an exit ramp leading outside, and were observed to have adequate lighting, chest of drawers, chair and a bed. The facility was observed to have the required postings emergency disaster plan, Long Term Care Ombudsman poster, CCL complaint poster, Theft and Loss policy and personal rights. There are books and games available to encourage socialization. The facility was observed to have a fully stocked first aid kit and manual, as well as emergency supplies including a generator. The fire extinguishers were serviced in May 2025, and were fully charged with the tag in tact. The sharps are stored in a locked drawer inside the kitchen next to the sink. The chemicals are stored in a double locked cabinet underneath the kitchen sink. The facility has combined smoke and carbon monoxide detectors were then tested and found to be operable. There are no known guns or ammunition, or pools/bodies of water on the premises. The emergency shut off for water and gas are stored in the underground cellar, in the back of the building. The medications, resident and staff files will be stored in a locked file cabinet inside the office area. The facility was observed to have ample amount of linen, dishes and utensils that are in good repair. The applicant successfully completed COMP III orientation on 07/08/25. Based on today's inspection the facility is ready for licensure.
