On 10/16/2025, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct an annual inspection. Upon arrival LPA introduced herself to Alefian Fnu- Support Staff and he was informed of the purpose of my visit and allowed LPA entry into the facility. At 12:15 PM, LPA reviewed three (3) staff files for First Aid/CPR certification, criminal record clearance, training's, and health screenings, which all appeared to be current. LPA reviewed three (3) residents’ files for admission agreements, updated physician reports, and needs and services plans which appeared to be current. At 1:25 PM, LPA toured residents’ bedrooms, and they all have the required furniture including beds, dressers, and nightstand, bedding supplies, which were in good condition, adequate lighting, and storage for resident’s personal belongings. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms appeared to be clean and operational. All showers have secured safety handrails and nonskid material and there are storage areas for personal hygiene items. The water temperature measured between 105-120 degrees Fahrenheit. LPA conducted an inspection of the kitchen and there LPA observed a 5-day supply of perishable foods and a 7-day supply of nonperishable foods. All sharps and knives are locked and secured in cabinet. All cleaning supplies were inaccessible to residents. Continued..... LPA observed that the facility appeared to be sanitary and appropriately furnished at the time of visit. LPA observed the fire extinguisher to be fully charged, smoke detectors and carbon monoxide were tested and they were in operable condition. LPA inspected the First Aid Kit and found it contained the required items and a manual. All required postings were observed. LPA observed the facility's infection control practices. LPA toured the physical plant inside, and outside walkways were clear and free of obstructions. Overall, the facility appeared to be clean, sanitary, and in good repair All window screens are clean and in good repair. Facility temperature 74° degrees. Based on the observations made during today’s visit, no deficiencies were cited. Type of visit: Case Management - Annual Continuation

Licensing Program Analyst (LPA) Sparkle Day conducted a continued Annual visit from 7/24/2025. During todays visit LPA met with Eilat Nahum who assisted with the visit. LPA conducted a records review of (5) client records and (5) staff records. LPA reviewed (4) medication Administration records. All Resident & Staff records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit. LPA did not observe any discrepancies at the time of visit. Licensing Program Analyst (LPA) Sparkle Day conducted an unannounced visit to the above facility. The purpose of today’s visit was to conduct the one-year inspection. LPA met with Eliat Nahum, Administrator and the purpose of the visit was discussed. Facility is licensed to serve 6 non- ambulatory residents of which 1 may be bedridden,. None of the residents are diagnosed with dementia or receiving home health or hospice care services. The facility does not handle any of the residents’ money. This home is a single story home consisting of: (5) resident bedrooms, (2) Full bathroom, den, living room, kitchen with dining area, laundry room and an outdoor shaded patio area. LPA toured the Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure, shower was free of mold/mildew and a non-skid mat was in place, water temperature measured between 115.1F. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards; doorways were free of obstructions. Kitchen was checked and observed to be within Title 22 regulations. Perishable and non-perishable food supply was checked. All cleaning solutions, hazardous items, and medications were securely locked and inaccessible to residents. Smoke detectors were working properly and fire extinguisher was fully charged. Carbon monoxide detector was operational. First Aid kit was available. Outside grounds were toured and no bodies of water were observed. Walkways around the home were clear of hazards. There are no security bars or weapons on the premises.
