Allegation investigated: Staff left resident in soiled diapers for an extended period of time

Licensing Program Analyst (LPA) Seo Jeon conducted an unannounced visit to the facility to deliver findings of the above allegation. LPA met with Arcita Cayabyab, Administrator. The Department investigation involved interviews with staff and residents and reviews of records. On 08-14-2025, Community Care Licensing (The Department) received a complaint report with the following allegation. It was alleged that staff left resident in soiled diapers for an extended period of time. Information received indicated that Resident #1 (R1) did not receive timely incontinence care. LPA conducted an interview with R1, who stated that R1 was receiving all necessary cares. R1 did not express any concerns with the facility services. LPA conducted interviews with two (2) other residents, all of whom denied ever being neglected by staff. LPA attempted to conduct interviews with two (2) additional residents but was unsuccessful due to their cognitive condition. Continued on LIC9099-C.... Unsubstantiated LPA conducted an interview with one (1) staff member who stated room check has been done every 1 to 2 hours for all residents. On one occasion, two (2) staff members stayed with R1 all night due to R1's behavior. Based on records review and interviews conducted, this allegation is Unsubstantiated. A finding that the complaint is UNSUBSTANTIATED means the allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation occurred. Licensing Program Analyst (LPA) Seo Jeon conducted an unannounced annual required visit. Upon entry, LPA was greeted by Reynan Lising, caregiver, and informed them of the purpose of the visit. At the time of the visit, there were two (2) staff members and five (5) residents present. Arcita Cayabyab, Administrator arrived later during the inspection. Facility Overview: The facility is a one-story home with five (5) bedrooms and four (4) bathrooms, including an attached garage. There are no pools or known firearms on the premises. Infection Control: LPA observed that hygiene and cleaning supplies were available for regular facility maintenance. The facility has infection control plan in file. Physical Plant: The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. The outdoor area was free of hazards. Laundry equipment was in good working condition. Sharp and dangerous objects were securely locked and inaccessible to residents. Both the smoke detector and carbon monoxide detector were operational, and the hot water temperature was 110°F. Fire extinguisher located at family room area has current inspection tag. Food Service: The facility’s kitchen was clean and equipped to prepare food. The facility maintained the required two-day supply of perishable foods and a seven-day supply of non-perishable foods. Continued on LIC809-C.... Care & Supervision/Administration: Adequate staff were present to supervise clients during the visit. The administrator holds a current administrator’s certificate. Record Review and Resident/Staff Files: LPA reviewed files for four (4) staff members, confirming criminal clearances, updated training, and CPR/First Aid certification. Five (5) resident files were reviewed and contained all required documentation. Health-Related Services/Incidental Medical Services: All resident medications were securely locked. LPA reviewed medications for two (2) residents, confirming that all medications were listed on the Medication Administration Record (MAR) and accounted for. Disaster Preparedness: LPA reviewed the facility’s emergency and disaster plan, including documentation of the last fire drill conducted on 12/17/2025, which met department requirements. All facility exits were clear of obstructions. No deficiencies were cited during the visit. Licensing Program Analysts (LPA) Seo Jeon and Ahliah Sharp conducted an unannounced visit to the facility to deliver findings of the above allegation. LPA met with Arcita Cayabyab, Administrator. The Department investigation involved interviews with staff and residents and review of records. On 01-15-2026, Community Care Licensing (The Department) received a complaint report with the following allegation. It was alleged that staff are not ensuring that the needs of the resident in care are being met. Information received indicated that staff became upset and withheld care, such as shower service and incontinence care to residents. Continued on LIC9099-C.... Unsubstantiated LPA conducted interviews with Resident #1 (R1), but only limited information was obtained due to R1’s inability to express themselves. LPA conducted interviews with another two (2) residents, all of whom stated they have received all the care from the facility staff. LPA’s attempted interviews with another two (2) residents were unsuccessful due to their cognitive condition. LPA conducted interviews with two (2) staff members, all of whom denied ever withholding care to residents. Based on interviews conducted, the Department’s investigation did not find enough information to corroborate the allegation that staff are not ensuring that the needs of the resident in care are being met. This allegation is unsubstantiated. A finding that the complaint is UNSUBSTANTIATED means the allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation occurred. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Seo Jeon made an announced visit to the facility for the purpose of conducting a pre-licensing inspection. LPA met with Applicant, Susan Nuguid, who accompanied LPA for the inspection. The Applicant has submitted an application for 6 residents (6 non-ambulatory). On 6-18-2024, the Riverside County Fire Department approved a fire clearance for which the applicant has applied for. The facility is a single story home consisting of 5 bedrooms, 4 bathrooms, kitchen, dining room, family room, garage, backyard with a covered patio, and a locked shed that is being used for storage. There is no swimming pool on the premises. The bedrooms were observed to have bed, lighting, night stand, chest of drawers and area for sitting. The bathrooms had nonskid mats, and grab bars. There is plenty of extra linen (sheets, blankets, towels) that were observed to be in good repair. The smoke and carbon monoxide detectors were tested and found to be operable. The hot water temperature was tested and was found to be within regulatory limits measuring at 109 degrees Fahrenheit. The facility is equipped with flashlights, night lights and solar panels. The facility has an emergency disaster plan, dementia plan and infection control training plan on file. The facility has a sufficient supply of dishes, cooking and eating utensils, that were observed to be in good repair. The facility food supply was observed to be sufficient as there was 2-day supply of perishable and a 7-day supply of nonperishable food items. The facility has an emergency food and water supply. Continued on LIC809-C.... The passageways, and ramps/inclines are clear and free from obstruction. The home has 1 fully charged fire extinguisher. The facility does not have any known guns or ammunition stored on grounds. The sharps/knives are stored in a locked cabinet in kitchen. The medications will be kept in individual boxes that will be stored in a locked cabinet located in living room. There is a fully stocked first aid kit with manual. LPA observed the required postings (facility sketch, resident council, theft and loss policy, personal rights, PUB475 CCL/dept complaint poster and the Long term Care Ombudsman poster) in the kitchen wall. The facility was observed to have activities to encourage socialization like various board games as well as a covered patio with plenty of outdoor space for walking and physical activities. LPA observed that the physical plant is clean, in good repair, and to be hazard-free during today’s visit. LPA determined the facility meets the operational requirements for licensure. The Pre-licensing inspection is complete, and this facility has no deficiencies.
