On 8/5/2026 at 9:45 am, Licensing Program Analyst (LPA) Y. Brown conducted an unannounced annual 1-year required inspection. LPA met with Administrator Maria Hosain and explained the purpose of the visit. The administrator currently holds a certificate (#7002922740) that expires on 2/10/2028. The facility’s fire clearance was approved for six (6) residents, six (6) may be non-ambulatory. LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. All indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature for residents is maintained at 73 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. Hot water temperature in the residents shared bathroom was measured at 108.1 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for residents. Smoke detectors and carbon monoxide combination were in operating condition during visit. Fire extinguisher was last purchased on 7/7/2026. First aid kit was observed to be complete. LPA reviewed three (3) staff and four (4) resident records. Emergency disaster plan was last reviewed on 6/6/2026. LPA reviewed a sample of medication. Continued on LIC809C. Continued from LIC809. On 7/2/2025 at 10:30 am, Licensing Program Analyst (LPA) Y. Brown conducted an unannounced annual 1-year required inspection. LPA met with caregiver Romulo Torres and explained the purpose of the visit. Administrator Maria Luisa Hosain arrived to the facility at 11:00 am. The administrator currently holds a certificate (#7002922740) that expires on 02/10/2026. The facility’s fire clearance was approved for six (6) non-ambulatory residents. LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of four (4) resident bedrooms, two (2) staff bedrooms and three (3) bathrooms. All indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature for residents is maintained at 71.1 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared resident bathroom was measured at 114.6 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for residents. Smoke detectors/carbon monoxide were in operating condition during visit. Fire extinguisher was serviced on 04/22/2025 and the last fire drill was conducted on 4/7/2024.The emergency Disaster plan was observed and updated on 7/2/2025. First aid kit was observed to be complete. Continued on LIC809C... (Continued from LIC809...) LPA reviewed three (3) staff and three (3) resident records and all were current and complete. LPA reviewed a sample of medication. Type of visit: Required - 1 Year
On 8/12/2024 at 10:20am, Licensing Program Analysts (LPAs) K. Nguyen and D. Doidge conducted an unannounced 1-Year Required inspection. LPAs met with administrator (AD) Maria Luisa Hosain, and explained the purpose of the visit. The Administrator currently holds a certificate (#6010517740) that expires on 02/10/2024. AD already renew her certificate and waiting for her new certificate to arrive. The facility’s fire clearance was approved for six (6) non-ambulatory residents. Fire drill was conducted on 4/8/24. Facility has a current liability insurance 11/4/23 to 11/4/24. LPAs toured the facility with Caregiver including but not limited to bedrooms, bathrooms, kitchen, common area, garage and backyard. The facility consists of five (5) total bedrooms which one (1) bedroom is occupied by staff. No bodies of water observed. A comfortable temperature is maintained at 69 degrees Fahrenheit. LPAs observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 120.0 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced on 11/6/2023. First aid kit was observed to be complete. LPAs reviewed four (4) of four residents files and four (4) of four (4) staff files, which were all complete. LPAs reviewed three staff files. 3 out of 3 are fingerprint cleared and associated to the facility. No deficiency cited during visit. On 12/16/2023 at 9:11am, Licensing Program Analyst (LPA) Carol Fowler conducted an unannounced 1-Year Required inspection. LPA met with Genoveva Torres, and Eloisa Guevara Caregivers, and explained the purpose of the visit. The Administrator currently holds a certificate (#6010517740) that expires on 02/10/2024. The facility’s fire clearance was approved for six (6) non-ambulatory residents. LPA toured the facility with Caregiver including but not limited to bedrooms, bathrooms, kitchen, common area, garage and backyard. The facility consists of five (5) total bedrooms which one (1) bedroom is occupied by staff. No bodies of water observed. A comfortable temperature is maintained at 73 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 120.0 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced on 11/6/2023. First aid kit was observed to be complete. LPA reviewed four (4) of four residents files and four (4) of four (4) staff files, which were all complete. Continued on LIC809C. Continued from LIC809.
