Type of visit: Case Management - Deficiencies

Licensing Program Analyst (LPA) Erica Mosley conducted a case management - Deficiencies visit due to deficiencies observed during an initial visit on 07/21/2026. Upon arrival at 10:06 a.m. LPA was greeted by staff, explained the purpose of the visit and staff contacted the Administrator. The Administrator Dr. Rashita Aggarwal was unavailable and and Co-Administrators Eleanor Jimenez and Manju Natarajan arrived shortly after and the purpose of the visit was explained, and an entrance interview was conducted. LPA was greeted by Staff #1 (S1) who is currently "pending" and not cleared. The staff has been working at the facility since 03/11/2026 which poses an immediate safety and personal rights risk to persons in care. At the time of the visit the LPA informed Administrative staff that S1 was not cleared and S1 was removed from the facility. During file review LPA discovered Staff #2 (S2) to be on the staff schedule and has been working at the facility since 06/09/2026 who is currently "pending" and not cleared which poses an immediate safety and personal rights risk to persons in care. During the physical plant tour LPA discovered a furnished bed and night stand to be set up in the garage. During both the pre-licensing and post-licensing visit the facility was informed that the fire clearance states that "Garage rooms shall not be used for sleeping rooms". At the time of the visit the Administrative staff took apart the bed frame, flipped the mattress and stated that staff do not sleep in the garage and noted that it "gets too hot for staff to sleep in the garage." LPA explained that fire clearance violation and criminal record clearance are a zero-tolerance violation and an immediate civil penalty in the amount of $1,500 will be assessed on today’s date 07/21/2026. LPA informed Administrative staff that failure to adhere to the requirements of their fire clearance may result in the assessment of additional civil penalties. The Administrative staff were made aware that failure to correct the deficiencies may result in civil penalties. Exit interview conducted. Type of visit: Post Licensing

Licensing Program Analyst (LPA) Erica Mosley arrived at the facility unannounced to conduct a post licensing visit and entered the facility at 10 a.m. Upon arrival, LPA Mosley was greeted by staff, and Administrative staff Eleanor Jimenez who called the Administrator to inform them of the visit. The Administrator / Licensee Representative Dr. Rashita Aggarwal arrived shortly after and the reason for the visit was explained. Entrance interview. The facility is single story house located in a residential neighborhood with an attached garage. At 10:06 a.m. a physical plant tour was conducted inside and out. An approved fire clearance was received, clearing them for six (6) non-ambulatory residents in bedrooms 1-6. The facility's bedridden capacity is approved for one (1) resident. Bedrooms 1,2,3,4 and 6 are approved for a single bedridden resident. COMMON AREAS: This includes the living room, and dining room. At the time of the visit, furniture in the common areas was observed to be in good condition. The facility maintained a comfortable temperature. At 12:48 p.m., hardwire combination of smoke, carbon monoxide detectors and a fire door were tested and operational at the time of the visit. The fire extinguisher was observed and fully charged on 01/15/2026. The emergency exiting plans/sketch are posted in every room. The emergency telephone numbers are posted in the common hallway. The LPA observed required postings throughout the common space. The last emergency disaster drill took place on 2/17/2026 and will be conducted quarterly. Activities were observed in the common areas.The fireplace in the living room is non-functional and screened and there are no tools. There is a functioning telephone on the premises. Auditory alarms at the entrances and exits were observed and functional at the time of the visit. Report Continued on LIC 809-C PAGE 2... (PAGE 2) Report Continued from LIC 809-C... INTERVIEWS: Starting at 10:05 a.m. and throughout the visit two (2) staff and two (2) resident interviews were conducted. Staff interview revealed that staff are knowledgeable in Resident rights, different forms of abuse, and reporting procedures. Resident interview revealed that no concerns were noted or voiced at the time of the visit. BEDROOMS: The facility has six (6) private resident bedrooms, and one (1) office / staff break area. Bedrooms 1,2,3,4, and 6 have direct exits to the facility. There is no staff room and staff stated that staff remain awake at night. All passageways were observed to be clear of obstructions. All resident rooms were observed to be furnished appropriately with clean linens, appropriate furnishings, and sufficient lighting. In addition, no bedroom was used as a passageway to another room, bath or toilet. All rooms were free of odors. All window screens were clean and maintained in good repair. RESTROOMS: There are three (3) total restrooms of which two (2) are private resident restrooms, and one (1) shared /common resident restroom. Resident restrooms were observed to be equipped with a slip resistant surface / mat. Grab bars were observed in the restrooms. The restrooms were sufficiently stocked with supplies and paper towels. The hot water temperature was measured in all resident restrooms and ranged between 107.4 - 108.3 degrees Fahrenheit, all within the required range. KITCHEN: The LPA inspected the kitchen/food service area. Knives and sharps were observed in a locked cabinet. Kitchen appliances were in operable condition. The facility has a sufficient supply of two (2) day perishable and seven (7) day non-perishable food. Refrigerator and food pantry were checked for proper labels and expiration dates. The kitchen faucet was measured for hot water temperature, and it measured 108.3 degrees Fahrenheit. Cleaning supplies and other chemicals are kept in a locked cabinet under the sink and inaccessible to residents in care. GARAGE: LPA observed the garage which is accessible from the house and the entry way is adjacent to the kitchen, and bedroom 1. The laundry area including a washer and dryer are located in the garage. The garage has two (2) storage rooms, used for storage only. Garage rooms shall not be used for sleeping rooms. Personal hygiene items (shampoos, soaps) were adequate and are stored in the locked garage. Extra incontinence supplies, and personal protection equipment (PPE) are stored in garage. RECORDS: Record review began at approx. 11:13 a.m. Resident Records: Three (3) Resident files were reviewed for, but not limited to, the following: signed admission agreements, current medical assessments with TB results, LIC627(c) Consent for Treatment form, and current needs and services plan. All records were in order. Personnel Records: Five (5) Personnel files including the Administrator’s file were reviewed for, but not limited to: personnel records, health assessments, criminal record clearances, first aid/CPR training, and the appropriate training. All records were in order. INFECTION CONTROL/ EMERGENCY DISASTER PLANNING: During today’s visit the LPA reviewed the facility’s infection control practices and the facilities emergency disaster plan. Both documents were observed to be complete and updated annually as required. The facilities policies and procedures, as they pertain to infection control and emergency planning meet the regulatory standard. MEDICATIONS: Medication review began at approximately 1:20 p.m. Medications are centrally stored and locked in a closet at adjacent to the entrance. Medications for two (2) residents were reviewed. Medications are labeled and checked for expiration dates. All medications including PRNs were labeled, stored, and locked inaccessible to residents in care. Medications reviewed were found to be self-administered as prescribed and documented on the centrally stored medication and destruction records. No errors observed during review. LPA observed the first aid supplies to be complete, including a thermometer and a current version of a first aid manual located in the laundry room. DOCUMENTS: Documents obtained during the visit include: LIC 500 facility roster and LIC 9020A Resident roster and copy of the current Limited Liability insurance. At the time if the visit the LPA reviewed the facilities contact information on file including phone numbers, email and annual fees. Administrator confirmed that all information is accurate. Licensing Program Analyst (LPA) Mosley conducted a pre-licensing visit to the above listed facility. The LPA met with applicant, Dr. Rashita Aggarwal, and Administrative staff Manju Natarajan and Marisol Flamenco. This is a new facility A Hospice Waiver has been approved for six (6) residents. The facility is single story house located in a residential neighborhood with an attached garage. At 9:03 a.m. a physical plant tour was conducted inside and out. An approved fire clearance was received, clearing them for six (6) non-ambulatory residents in bedrooms 1-6. The facility's bedridden capacity is approved for one (1) resident. Bedrooms 1,2,3,4 and 6 are approved for a single bedridden resident. The facility has six (6) private resident bedrooms, and one (1) office. Bedrooms 1,2,3,4, and 6 have direct exits to the facility. All resident bedrooms are set up with beds, nightstands, lamps, chests of drawers, chairs, and closet space. The beds are furnished with box springs, comfortable mattress, and clean linen, which includes, a mattress pad, top and bottom linens, pillowcases, blanket (if needed) and a bedspread. Lighting in the rooms appeared adequate. The bedrooms were large enough to allow for easy passage between the beds and furniture with a wheelchair or walker. In addition, no bedroom was used as a passageway to another room, bath or toilet. All rooms were free of odors. All window screens were clean and maintained in good repair. There are three (3) total bathrooms of which two (2) are private resident bathrooms, and one (1) shared /common resident bathroom. The resident bathroom(s) have a shower with slip resistant surface / mat / materials. The toilets and showers have grab bars. Bedrooms 1 and 5 have a private bathroom. The hot water temperature was tested in all bathrooms and the kitchen and was found to be within the range of 105*F and 120*F degrees Fahrenheit. The supply of extra bed and bath linens is adequate. Resident and staff records are stored in the locked main entry closet, located at the entrance adjacent to the living room. Medications will also be centrally stored in this locked closet. Medications requiring refrigeration will be kept in a locked box inside the main refrigerator. The first aid supplies were complete, including a thermometer and a current version of the first aid manual. These supplies are stored in the main entry closet along with the records and medications. Kitchen knives are stored in a locked cabinet in the kitchen. Stove burners are rendered inaccessible to the residents by removing them when not in use. The supply of dishes, utensils, pots, pans and drink ware is adequate. The freezer was maintained at zero degrees Fahrenheit (0*F) and the refrigerator was maintained at 40*F. The supply of nonperishable food is adequate. There are no pesticides (poisons) or toxins stored in any food storage area or preparation area with utensils. Appliances in the kitchen were clean and all appeared functional. Trash cans had tight fitting lids. Kitchen, laundry and house cleaning supplies are stored in a locked cabinet located in the garage /laundry area. No flies or other vermin were observed. The common areas were appropriately furnished, and the lighting was adequate. There are televisions and other entertainment equipment, games and/or activity supplies in the living room and dining area. There was sufficient space to accommodate both indoor and outdoor activities. Night lights were maintained in hallways and passageways to non-private bathrooms. All ramps were secure and non-slippery and were positioned at the level where wheelchairs and walkers may enter and exit the facility safely. There is a fireplace in the living room. It is screened and there are no tools. Alarms on all exterior doors were engaged at the time of visit and functional. In addition, the physical plant is consistent with the submitted facility sketch/floor plan. The facility had emergency lighting, which included flashlights. The facility has a furnace, which is able to heat rooms that residents occupy to a minimum of 68 degrees Fahrenheit; and, they have central air conditioning and are able to cool rooms to a comfortable range, not to exceed 85 degrees Fahrenheit. The facility smoke alarm system is hard wired. The smoke detectors, fire door and carbon monoxide detectors were tested at 9:58 a.m. and functioned properly during the time of visit. There is one (1) fire extinguisher located in the dining room adjacent to the kitchen that was fully charged and purchased on 01/14/2026 and does not exceed the expiration date. In the back of the house there is a retractable awning for shade, patio area including tables and chairs where residents can sit. The entire property is fenced. The back and sides of the house are separated from the front yard by gates at the north and south side passageways. There are no other structures on the property. During today’s visit COMP III orientation was completed with applicant, Dr. Rashita Aggarwal, and Administrative staff Manju Natarajan and Marisol Flamenco. This report will be sent to the Centralized Application Bureau (CAB). You will be notified by the CAB Analyst when your license has been approved. You are not allowed to begin operating until you have been notified that your license has been approved by the CAB Analyst. Failure to comply could affect approval of your license.
