Allegation investigated: Facility did not ensure that medication was inaccessible to client
On June 16, 2026, at 10:30 AM, Licensing Program Analyst (LPA) Eldin Serrano visited the facility to investigate the above-mentioned allegation and deliver findings. LPA met with Administrator Meljorie Castelo to discuss the purpose of the visit. The investigation consisted of file review, interviewing relevant parties as well as observation. Allegation indicates that facility did not ensure that medication was inaccessible to client - Based on interviews with relevant parties, it was confirmed that Client #1 (C1) transported medication belonging to a caregiver on his wheelchair when he went to the day program. This confirms that the facility failed to ensure the medication remained inaccessible to the client, therefore the allegation is substantiated. Based on interviews, the preponderance of evidence standard has been met, therefore, the allegation is substantiated under the California Code of Regulations (Title 22, Division 6 Chapter 8). Type of visit: Required - 1 Year
On 3/26/2026 at 12:55 PM, Licensing Program Analyst (LPA) Eldin Serrano made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPA met with Administrator Meljorie Castelo and was granted entry to the facility. At the time of the visit there were two (2) staff present, and three (3) residents present.. The facility is a four (4) bedrooms, two (2) bathroom home with a kitchen/dining area, living room/activity room. The facility is Residential Care Facility for the Elderly (RCFE). The facility is licensed for a capacity of six (6) non-ambulatory, two (2) hospice care waiver and 1 maybe bedridden resident and the current census is four (4) residents. LPA was accompanied by Staff to conduct a general overall inspection, which included, but was not limited to the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing Division (CCLD). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature of 77 degrees Fahrenheit. LPA inspected resident bedrooms; they are equipped with required furniture such as: mattresses, nightstands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPA observed sufficient furniture and lighting throughout the facility. LPA measured and observed the water temperatures in the bathroom to be at 111 degrees Fahrenheit. The facility is equipped with operating smoke detectors and carbon monoxide alarms. Fire extinguishers were also observed at the facility. Posters such as personal rights, the CCLD complaint poster, labor laws, and the disaster plan were posted in a common area. ***Continuation in LIC809C *** Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to residents in care. There was a designated storage space for resident/staff files. There is a Medicine cabinet with the resident’s medications locked. LPA observed first aid kit and first aid book at the facility. Food Service: Seven (7) days’ supply of Non-perishable foods and two (2) days’ supply of perishable food supply were observed and sufficient for the number of residents in care. Care & Supervision: The facility has sufficient number of staff to provide care and supervision to the residents in care. Also, LPA observed that the facility does not have dementia residents. Record Review: LPA reviewed three (3) resident files for admission agreements, updated physician reports, Individual Program Plan (IPP), Medications/Medication Administration Record (MAR) were audited. LPA observed that resident #1 (R1) physicians report was not updated. Deficiency issued. LPA observed that resident #1 and resident #2 (R2) do not have an updated IPP. Technical Violation issued. LPA reviewed two (2) staff and administrator files for First Aid/CPR certification, criminal record clearance, trainings, and health screenings. LPA observed no issue. Based on the observations and record reviews made during today’s visit, one (1) deficiency was cited per Title 22, Division 6, of the California Code of Regulations. On 3/12/2025 at 1:30 PM, Licensing Program Analyst (LPA) Eldin Serrano made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPA Serrano met with Caregiver Myrna Sapigao and was granted entry to the facility. At the time of the visit there was two (2) staff present, and five (5) residents present. The administrator was out of town and the assistant administrator was not available at the time of the visit. The caregiver is the designated person. The facility is a four (4) bedrooms, two (2) bathroom home with a kitchen/dining area, living room/activity room. The facility is Residential Care Facility for the Elderly (RCFE). The facility is licensed for a capacity of six (6) non-ambulatory, two (2) hospice care waiver and 1 maybe bedridden resident and the current census is five (5) residents. LPA Serrano was accompanied by Staff to conduct a general overall inspection, which included, but was not limited to, the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing Division (CCLD). LPA observed sufficient furniture and lighting throughout the facility. LPA Serrano measured and observed the water temperatures in the bathroom to be at 105.2 degrees Fahrenheit. The facility is equipped with operating smoke detectors and carbon monoxide alarms. Fire extinguishers were also observed at the facility. Posters such as personal rights, the CCLD complaint poster, labor laws, and the disaster plan were posted in a common area. LPA observed that the facility does not have the Infection Control Plan available for review. Deficiency will be issued ***Continuation in LIC809C *** Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to residents in care. There was a designated storage space for resident/staff files. There is a Medicine cabinet with the resident’s medications locked. LPA Serrano observed first aid kit and first aid book at the facility. Food Service: Seven (7) days’ supply of Non-perishable foods and more than two (2) days’ supply of perishable food supply were observed and sufficient for the number of residents in care. Medications/Medication Administration Record (MAR) were audited. LPA observed that resident #3 (R3) PRN was not properly recorded in the medication administration record (MAR). Deficiency will be issued. LPA Serrano reviewed two (2) staff and administrator files for First Aid/CPR certification, criminal record clearance, trainings, and health screenings. LPA observed no issue. Based on the observations and record reviews made during today’s visit, two (2) deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. Feb 10, 2025Complaint investigation reportSubstantiated
Allegation investigated: Staff did not follow Emergency Disaster Plan Staff do not have required medication training
On 2/10/2025 at 9:30 AM, Licensing Program Analyst (LPA) Eldin Serrano arrived at the facility to investigate a complaint and deliver the findings for the above complaint allegation. Upon arrival, LPA Serrano met with the caregiver Myrna Sapigao and immediately called the Administrator/Licensee Meljorie Castelo who arrived ten (10) minutes later. LPA Serrano informed administrator of the purpose of the visit. The investigation consisted of file review, observation and interviews with relevant parties.The department received a complaint on 02/05/2025 regarding the following allegations: #1 Staff did not follow Emergency Disaster Plan. During the interview with the licensee, the licensee stated that they had an emergency shutdown of electricity in the area in which the Southern California Edison (SCE) did not give prior notification for the ouatge on 1/9/2025 which SCE usually do for scheduled outages. ****continue on LIC9099C***** Substantiated Licensee stated that they purchased a portable stove the day after the outage and was not prepared for that emergency outage. Since their water supply is from their own private water well, it needs electricity for it to work. Based on observation, the facility has a 50 gallon water container reserve for washing and hygiene use. Licensee, provided warm food and sandwiches the day of the outage from outside source. They used the portable stove the next day to provide warm food for the residents and to warm the water for daily hygiene for the residents. No residents medication needs refrigeration. Licensee stated that their personal residence is three (3) miles away from the facility and can bring enough supply/items as needed. Upon review of the Emergency Disaster Plan, the allegation corroborates that the licensee did not follow the emergency disaster plan because they have a non-working generator which the licensee stated that they tried to turn on at the time of the power outage but no longer working. That made the facility unprepared for the emergency by not ensuring all the equipment is in working condition. LPA will issue a citation. #2 Staff do not have required medication training. Based on file review of the staff, LPA observed that the medication training was dated on 01/17/2025 for two (2) direct support professional (DSP) Staff #1 (S1) and Staff #2 (S2). The certificates were not readily available upon the inspection of the reporting party (RP) 01/14/2025 in which the two DSP were already administering medication without the proper training and certification. LPA will issue a citation. Based on LPA's observations and interviews which were conducted and record review(s), the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 6 Chapter 8), are being cited on the attached LIC 9099D. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Anna Bueno conducted an unannounced visit to this facility for a required annual inspection. Entry into the facility is unobstructed and LPA met with two facility staff who were informed of the reason for today's visit. Licensee Meljorie Castelo arrived shortly during today's visit. The facility is approved to lock two backyard gates by the Riverside County Fire Department. LPA and staff toured the facility. Physical Plant: The facility is operating within capacity and not beyond the conditions of the license. There are no pools or other bodies of water located on the premises. The facility is being maintained at a comfortable temperature for residents. All passageways are kept free of obstruction. Hot water temperature was measured in five units at and random all measured within regulatory limits. There are grab bars for all toilets and shower used by residents. Fire safety installations such as extinguishers were last inspected on 12/14/23 and fire alarms and carbon monoxide were tested by staff. All units were found to be in working order. Overall the facility is in good condition; it is clean, sanitary and free of foul odors. Kitchen and Food Service: The total daily diet provided to residents appears to be of the quality and in the quantity necessary to meet resident needs. There is a one week supply of nonperishable foods and two days of perishable food items. Plates, cups, and utensils were observed stored in a safe matter. All readily perishable food or beverages capable of micro-organism growth are being stored in covered containers at appropriate temperatures. Refrigerator and freezer are maintained within regulatory temperatures. Medication, Care, and Supervision: The facility has ensured sufficient and competent staff to provide the services needed to meet resident needs. Chemicals and items which can constitute a danger are stored inaccessible to residents. LPA Bueno inspected medications and centralized list. Medications are in their original containers and appear to be dispensed according to physician's orders. Resident and Staff Files: LPA Anna Bueno reviewed a sample of staff and resident files. Staff files had the required documentation including a health screening report and current first aid and/or CPR certification. Resident files had the required documentation including admission's agreement, Individual program plans, and updated physician's reports. Current first Aid/CPR training are on file and training for proper care for restricted health conditions are present. Operations and Administration: Disaster Plan is present. Licensee is present in the facility a sufficient amount of hours and their administrator certification is up to date. The required licensing and ombudsman posters are posted in public view. Residents rights are posted and a copy is kept the resident's file. No deficiencies were cited during this visit.
