Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Alvaro Ramirez, Jr. conducted an unannounced visit for the Required 1 Year Inspection. LPA explained the purpose of today’s visit, and was greeted and granted entry by Administrator (AD) Bonnie Curkin. For today’s visit, LPA observed a total of five residents in care and two staff members on duty. LPA observed the facility AD certificate for AD Bonnie Curkin which expires on October 12, 2026. LPA observed that the PUB475 "See Something, Say Something" poster was posted in the hallway by the entryway. LPA Ramirez toured the interior and exterior portions of the facility with AD Curkin. The facility is a two-story home and is licensed for six non-ambulatory residents, of which four may be on hospice and zero bedridden. There are a total of five bedrooms. LPA Ramirez toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. Smoke and carbon monoxide detector and auditory exit alarms were tested and operational. There are a total of two restrooms. Restrooms were observed to be in good repair, toilets were operational, and grab bars and non-skid floor mats were provided. Water temperature tested at between 106.5-110.3 degrees Fahrenheit. Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked and inaccessible to residents in care. Fire extinguisher was charged and located by the hallway. The fire extinguisher was last serviced on May 18, 2026. CONTINUED ON LIC809-C... LPA Ramirez observed the emergency disaster and evacuation plan, which is located by the hallway. Licensee was advised to use the updated Emergency Distaste Plan (LIC610E) dated December 2025. Facility had back-up emergency food and water supply. LPA observed that First Aid Kit had all the required components. LPA observed that medications and toxins were locked and inaccessible to residents in care. The medications are locked in a cabinet by the dining room. For the exterior portion, LPA Ramirez observed a shaded area, patio furniture, and the grounds were free of any hazards. There is one gate in the backyard, which is both self-closing and self-latching. No bodies of water were observed. During today's visit LPA observed the resident enjoying live violin music. LPA reviewed six resident files and three staff files. LPA interviewed residents and staff present. For today's visit no deficiencies were issued per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted with Supervisor Gonzalez. Type of visit: Required - 1 Year

On June 18, 2025, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by care giving staff after explaining the purpose for the visit. Administrator (AD) Shimon Cagan was present and assisted LPA with the inspection. LPA observed that Shimon Cagan has a valid Administrator certificate which expires on November 21, 2025. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents and has a hospice waiver for four. The facility is a two story home with five private resident bedrooms, two shared resident bathrooms, a living room, a dining room, a kitchen, a sun room, and an attached two car garage. LPA, accompanied by the AD, conducted a tour of the interior portions of the facility. On today's visit, LPA observed five residents in care and three care giving staff present. LPA observed residents eating lunch in their respective bedrooms and in the sun room. LPA observed the See Something, Say Something poster (PUB 475) mounted on the wall by the entryway of the facility. LPA inspected the five resident bedrooms and observed them to be free of hazards. LPA observed resident bedrooms to have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPA observed resident beds to have clean linens and blankets. LPA observed additional linens to be stored in a hallway closet. LPA inspected the two shared resident bathrooms and they were observed to be free of hazards. Resident bathrooms were equipped with grab bars and nonskid floor mats. Faucets and toilets were operational. Hot water temperature measured between 116.7 to 118.2 degrees Fahrenheit. LPA observed the second story of the home to be off limits to residents in care. LPA observed a staff office on the second story which is kept locked and inaccessible to residents in care. CONTINUED ON 809-C LPA observed the kitchen has a two day perishable and a seven day non-perishable food supply on hand. LPA observed kitchen appliances to be clean and operational. The five burner gas stove lights unassisted. LPA observed kitchen knives and sharps to be stored in a locked kitchen cabinet. LPA observed chemicals and toxins to be stored in a locked kitchen cabinet under the sink. LPA observed fire extinguishers to be mounted on the wall in the dining room and in the resident hallway. Fire extinguishers were observed to be charged and serviced as of May 9, 2025. LPA tested the wired smoke detectors/carbon monoxide detectors which tested operational. LPA observed the facility conducted their last emergency disaster drill on March 27, 2025. LPA observed oxygen signs to be posted in the appropriate areas. The centrally stored medication is kept in a locked cabinet in the dining room. A First Aid Kit is stored in the locked closet and it was observed to have all the required components. LPA observed a fire place in the living room and it was adequately fenced and not in operation at time of visit. LPA observed the sun room to be clear of any obstructions or hazards. LPA observed the door leading to the attached two car garage to be kept locked and inaccessible to residents in care. LPA observed the garage to be used for storage. LPA observed the facility has a three day emergency food and water supply stored in the garage. LPA, accompanied by the AD, conducted a tour of the exterior portion of the facility which was observed to be free of hazards. LPA observed a shaded outdoor seating area with furniture for resident use. LPA observed the perimeter gates of the facility to be self latching and can be opened in an evacuation. There are no bodies of water on the premises. LPA reviewed all five resident files. All the required documentation was present and current in the resident files reviewed. LPA reviewed residents' medication and medication administration records. LPA reviewed three staff files. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, no deficiencies are being cited per the Title 22 of the California Code of Regulations. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Jessica Cho arrived at the facility unannounced for the purpose of conducting the Required 1-Year annual inspection using the CARE Inspection Tool. LPA met with Licensee/Administrator Shimon Cagan and explained the reason for the visit. The facility is a two story structure located in a residential neighborhood. Facility is licensed to operate for six (6) non-ambulatory and maintains a hospice waiver for four (4). There are five residents in care during today's visit with two caregivers on duty. LPA observed the facility to be clean and sanitary. There are five resident bedrooms and two resident bathrooms. All common areas were inspected including the attached two car garage and the sun room which doubled as a laundry room. The residents' bedrooms were appropriately furnished. Beds and bedding supplies were in good condition, adequate lighting was provided, sufficient storage space for each residents' personal belongings were observed. Bathrooms were found to be in compliance, clean, and operational. The water temperature measured at 113.3 and 109.2 degrees Fahrenheit. Toxins, disinfectants, sharps, and medications were secured and inaccessible. LPA observed sufficient two-day supply of perishables and seven-day supply of non-perishable food available. LPA toured the exterior portion of the facility. LPA observed the outdoor passageway free of obstructions. The exit gate was self-closing and self-latching. LPA observed sufficient seating and shading. Facility maintains two fire extinguishers. Both were mounted, charged, and serviced on March 12, 2024. The auditory devices and dual-functioning smoke/carbon monoxide detectors were tested and operational. LPA observed the emergency disaster supplies including food/water in the garage. Emergency evacuation drills are being conducted quarterly. The first aid kit contains all necessary elements. A working facility telephone number, 714-542-0382, remains available. LPA observed the required 'See Something, Say Something' (PUB475) poster in the correct size posted in the entry way. The Administrator's Certificate for Shimon Cagan expires on November 21, 2025, August 21, 2024 for Administrator Seth Curkin, and October 11, 2024 for Bonnie Curkin. LPA conducted an audit of five residents' files and two personnel files. No discrepancies were noted. Staff and resident interviews were conducted. Medications were audited for five residents. No discrepancies noted. Based on the observations made during today's visit, no deficiency is being cited today.
