Type of visit: Required - 1 Year
On June 1, 2026, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by care giving staff after explaining the purpose for the visit. Administrator (AD) Shimon Cagan was notified via telephone but was unable to assist with today's inspection. LPA observed that Shimon Cagan has a valid Administrator certificate which expires on November 20, 2027. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents and has a hospice waiver for three. The facility is a single story home with five resident bedrooms, one of which is shared, two shared resident bathrooms, a living room, a dining room, a kitchen, a laundry room, and an attached two car garage. LPA, accompanied by a care giving staff, conducted a tour of the interior portions of the facility. On today's visit, LPA observed five residents in care and three care giving staff present. LPA observed the See Something, Say Something poster (PUB 475) mounted on the wall by the entryway of the facility. LPA inspected the five resident bedrooms and observed them to be free of hazards. LPA observed resident bedrooms to have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPA observed resident beds to have clean linens and blankets. LPA observed additional linens to be stored in a hallway closet. LPA inspected the two shared resident bathrooms and they were observed to be free of hazards. Resident bathrooms were equipped with grab bars and nonskid floor mats. Faucets and toilets were operational. Hot water temperature measured between 110.4 to 111.2 degrees Fahrenheit. LPA observed the kitchen has a two day perishable and a seven day non-perishable food supply on hand. LPA observed kitchen appliances to be clean and operational. CONTINUED ON LIC809-C The five burner gas stove lights unassisted. LPA observed kitchen knives and sharps to be stored in a locked kitchen cabinet. LPA observed fire extinguishers to be mounted on the wall in the dining room and in the laundry room. Fire extinguishers were observed to be charged and serviced as of May 18, 2026. LPA tested the wired smoke detectors/carbon monoxide detectors which tested operational. LPA observed the facility conducted their last emergency disaster drill on April 15, 2026. LPA observed a fire place in the living room and it was adequately fenced and not in operation at time of visit. The centrally stored medication is kept in a locked closet by the dining room. A first aid kit is also stored in the locked closet and it was observed to have all the required components. LPA observed chemicals and toxins to be stored in a locked cabinet located in the laundry room. LPA observed the door leading to the attached two car garage to be kept locked and inaccessible to residents in care. LPA observed the two car garage to be used for storage. LPA observed the facility has a three day emergency food and water supply stored in the garage. LPA, accompanied by a care giving staff, conducted a tour of the exterior portion of the facility. The exterior was observed to be free of hazards or obstructions. LPA observed a shaded outdoor seating area with furniture for resident use. LPA observed the perimeter gate of the facility to be self latching and can be opened in an evacuation. There are no bodies of water on the premises. LPA reviewed all five resident files. All the required documentation was present and current in the resident files reviewed. LPA reviewed residents' medication and medication administration records. LPA reviewed three staff files. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, no deficiencies are being cited per the Title 22 of the California Code of Regulations. On May 13, 2025, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by care giving staff after explaining the purpose for the visit. Administrator (AD) Shimon Cagan was present and assisted LPA with the inspection. LPA observed that Shimon Cagan has a valid Administrator certificate which expires on November 21, 2025. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents and has a hospice waiver for three. The facility is a single story home with five resident bedrooms, one of which is shared, two shared resident bathrooms, a living room, a dining room, a kitchen, a laundry room and an attached two car garage. LPA, accompanied by the AD, conducted a tour of the interior portions of the facility. On today's visit, LPA observed five residents in care and three care giving staff present. LPA observed residents participating in a dancing and music activity. LPA observed the See Something, Say Something poster (PUB 475) mounted on the wall by the entryway of the facility. LPA inspected the five resident bedrooms and observed them to be free of hazards. LPA observed resident bedrooms to have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPA observed resident beds to have clean linens and blankets. LPA observed additional linens to be stored in a hallway closet. LPA inspected the two shared resident bathrooms and they were observed to be free of hazards. Resident bathrooms were equipped with grab bars and nonskid floor mats. Faucets and toilets were operational. Hot water temperature measured between 107.6 to 108.5 degrees Fahrenheit. LPA observed the kitchen has a two day perishable and a seven day non perishable food supply on hand. LPA observed kitchen appliances to be clean and operational. CONTINUED ON 809-C The five burner gas stove lights unassisted. LPA observed kitchen knives and sharps to be stored in a locked kitchen cabinet. LPA observed fire extinguishers to be mounted on the wall in the dining room and in the laundry room. Fire extinguishers were observed to be charged and serviced as of May 9, 2025. LPA tested the wired smoke detectors/carbon monoxide detectors which tested operational. LPA observed the facility conducted their last emergency disaster drill on March 27, 2025. LPA observed a fire place in the living room and it was adequately fenced and not in operation at time of visit. LPA observed oxygen signs to be posted in the appropriate areas. The centrally stored medication is kept in a locked closet by the dining room. A First Aid Kit is stored in the locked closet and it was observed to have all the required components. LPA observed chemicals and toxins to be stored in a locked cabinet in the laundry room. LPA observed the door leading to the attached two car garage to be kept locked and inaccessible to residents in care. LPA observed the garage to be used for storage. LPA observed the facility has a three day emergency food and water supply stored in the garage. LPA, accompanied by the AD, conducted a tour of the exterior portion of the facility which was observed to be free of hazards. LPA observed a shaded outdoor seating area with furniture for resident use. LPA observed the perimeter gate on the southside of the facility to be self latching and can be opened in an evacuation. There are no bodies of water on the premises. LPA reviewed all five resident files. All the required documentation was present and current in the resident files reviewed. LPA reviewed residents' medication and medication administration records. LPA reviewed three staff files. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, no deficiencies are being cited per the Title 22 of the California Code of Regulations. Jun 11, 2024Facility evaluation reportReport on file
Type of visit: Required - 1 Year
On June 11, 2024 at 1:20pm, Licensing Program Analyst (LPA) Edward Kim conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Kim met with Administrator (AD) Shimon Cagen and explained the purpose of the visit. The facility is licensed to operate for six (6) non-ambulatory and have a hospice waiver for three (3) residents. The facility is a single-story structure located in a residential neighborhood. It consists of the following: five (5) resident bedrooms, two (2) bathrooms, living room, dining room, kitchen, attached garage, and an outside covered patio area. LPA Kim toured indoor and outdoor of the physical plant with AD Cagen. There are no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident’s personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. All bedrooms were inspected: Resident Room 1, Resident Room 2, Resident Room 3, Resident Room 4, and Resident Room 5. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured at 113.1 and 114.1 degrees F. A comfortable temperature of 74 degrees F was maintained in the facility. LPA Kim observed the facility to be sanitary and appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to residents. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food available and maintained properly. Emergency food and water are stored in the garage. Emergency supplies are stored in medication closet. The facility has two (2) fire extinguishers that are charged and they were all serviced on March 12, 2024 and smoke/carbon monoxide detectors were operable. Evaluation Report Continues on LIC 809-C During the visit, LPA Kim observed the facility's infection control practices, plan of operation, and screening protocols for visitors, staff, and residents. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). All mandated inspection control posters were posted. The facility conducts Fire/Safety Drill quarterly. A working telephone (714-543-3937) remains available. First Aid Kit contained all the necessary elements. LPA Kim conducted an audit of resident files (R1-R6), staff files (S1-S7), and medication and medication administration review that were all in order and complete. LPA Kim conducted five (5) staff interviews. No deficiencies were cited during this inspection visit.
