Type of visit: Required - 1 Year

On today's date Licensing Program Analyst (LPA) William Vanegas made an unannounced visit for the purposes of completing an annual inspection. Upon arrival LPA was greeted and granted entry to the facility by facility staff. Administrator (AD) Glenn Baul Navarro was notified via telephone and arrived shortly after LPA's arrival and was able to assist LPA with the annual inspection. LPA conducted a tour of the facility and observed the following. AD has a valid Administrator Certificate valid from July 23, 2024 to July 22nd, 2026. The facility is a two storied home equipped with 3 bathrooms and eight bedrooms two of which are utilized for live in staff and six of which are utilized for residents in care. LPA observed kitchen area to be clean and clear of any mildews and debris. LPA observed for there to be a refrigerator, gas stove, dishwasher, and a microwave in the kitchen area. All appeared to be in good repair and tested operational. LPA observed for there to be a two day supply of perishable food and a seven day supply of non-perishable food on hand, LPA observed a sufficient amount of emergency water on hand. LPA observed all resident bedrooms to be clean and free of any hazards or debris. Resident bedrooms are large enough to walk about freely and accommodate all required furnishings. LPA observed all bedrooms to have all required furnishings including a chair, a reading lamp, a chest of drawers, a bed, clean linens in good repair; meaning no strains or tears, and enough storage space to store personal belongings. LPA Observed all bathrooms to be clean and clear of any mildew and debris. LPA observed bathrooms to have all required furnishings including slip resistant floor matts, grab bars, and a shower chair if needed. LPA observed all water faucets and toilets to be in good repair and tested operational. Hot water temperature to be between 109.3 and 111.2 degrees Fahrenheit. CONTINUED ON LIC809-C LPA observed all fire extinguishers to be fully charged and have an updated service tag attached to it. LPA observed all smoke detectors and carbon monoxide detectors to be in good repair and tested operational. LPA observed first aid kit to have all required items including adhesive tape, scissors, tweezers, bandages, a thermometer, and a first aid manual. LPA toured the outside of the facility and observed the following; the backyard was observed to be clean and free of any hazards, or debris. LPA observed an out shaded sitting area. LPA observed side doors to be unlocked and self latching. LPA observed the backyard to be large enough to participate in outdoor activities upon resident request. LPA reviewed six resident files, and two staff files. All files (Staff and Resident) had all required documents. All staff training was up to date, and documented correctly. LPA reviewed all medications with AD and observed all medications to be documented correctly. Per LPA review all medications are being administered per physicians orders. Based on today's observations no deficiencies will be issued per title 22 chapter 6 division 8 of the California Code of Regulations. Type of visit: Case Management - Annual Continuation

Licensing Program Analyst conducted a continuation of the annual inspection, and observed the following, Resident bedrooms are all clean and contain all required furnishings such as a bed, chair, lamps, clean linens meaning no strains or tears, enough storage space to store personal belongings, and the rooms are large enough to roam about freely. LPA Vanegas observed all resident restrooms to be clean and sanitary as well as free of any mildew or debris. LPA Vanegas observed water faucets, and toilets to be operational and in good repair. LPA Vanegas observed the water temperature to be between 120.8 and 121.4 degrees. LPA Vanegas observed all toxins, sharps and medications to be inaccessible to clients in care and locked away. LPA Vanegas observed the backyard of the facility to be clean and free of any obstructions blocking the exit routes. LPA Vanegas observed an outdoor shaded sitting area available to residents in care upon request. LPA Vanegas reviewed medications with Care Giver (CG) Glen Navarro, and per LPA Vanegas review all medications are being administered per physicians orders. LPA Vanegas observed all fire extinguishers to be fully charged and up to date. LPA Vanegas observed all smoke and carbon monoxide detectors to be in good repair and operational. LPA Vanegas reviewed two staff files and five resident files. All files (Staff and Resident) contained all required documents, and all staff annual training was complete. Based on observations made during today's inspection no deficiencies will be cited. Licensing Program Analyst (LPA) William Vanegas conducted an unannounced annual inspection for the purposes of completing an annual inspection. Upon arrival LPA Vanegas was greeted an granted entry to the facility by Care Giver on duty. LPA Vanegas explained the nature of the visit and began a tour of the facility and observed the following. This is a two storied home with 7 bedrooms one of which is a staff bedroom, and six of which are resident bedrooms. LPA Vanegas observed kitchen area to be clean and free of debris and mildew. LPA Vanegas observed a dishwasher, microwave, refrigerator, washer, and dryer at the facility. All appeared to be in good condition and tested operational. LPA Vanegas observed a seven day supply of non- perishable food, and a two supply of perishable food. LPA Vanegas conducted interviews with clients in care, however LPA Vanegas was advised that they had to return to the office for in office duties. LPA Vanegas advised that they would have to return to complete the annual inspection. Licensing Program Analyst (LPA) Alvaro Ramirez, Jr. conducted an unannounced visit for the Required 1 Year Inspection. LPA explained the purpose of today’s visit, and was greeted and granted entry by Caregiver Herminiapaz Navarro. For today’s visit, LPA observed a total of six residents in care and two staff members on duty. LPA Ramirez toured the interior and exterior portions of the facility with Navarro. The facility is a two-story structure and is licensed for six non-ambulatory residents, of which six may be on hospice and zero bedridden. There are a total of eight bedrooms of which six are for residents and two for staff. LPA Ramirez toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. Smoke and carbon monoxide detector and auditory exit alarms were tested and operational. There are a total of five restrooms of which four are for residents and one for staff. Restrooms were observed to be in good repair, toilets were operational, and grab bars and non-skid floor mats were provided. Water temperature tested between 107.7-116.8 degrees Fahrenheit. Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked and inaccessible to residents in care. LPA observed that the fire extinguisher was charged and mounted by the resident's bedroom hallway. During the visit LPA observed that the residents were having lasagna, fruit, juice, milk and/or water for lunch. CONTINUED ON LIC809-C.. LPA Ramirez observed the emergency disaster and evacuation plan, which is located by the staff office area. Facility had back-up emergency food and water supply. LPA observed that First Aid Kit had all the required components. LPA observed that medications were locked and inaccessible to residents in care. For the exterior portion, LPA Ramirez observed a shaded area, patio furniture, and the grounds were free of any hazards. There are two gates in the backyard, which both are self-closing and self-latching. No bodies of water were observed. LPA reviewed three resident files and two staff files. LPA interviewed residents and staff present. For today's visit one deficiency was issued per Title 22 Division 6 of the California Code of Regulations.
