On 9/14/2025 at approximately 09:4(5AM, Licensing Program Analyst (LPA), Angelica Segovia conducted an unannounced annual visit to the facility. LPA was greeted by staff and stated the reason for their visit. The Co-Administrator, Jeffrey Ezros arrived shortly after to assist with today’s visit. LPA asked for the census, Staff/Resident Roster, and Liability Insurance. LPA conducted a physical plant tour at approximately 1:00 PM and the following was noted: The facility is a single-story building with six (6) bedrooms and six (6) bathrooms. The facility is currently occupying six (6) residents. There is a designated staff office. The facility has an approved fire clearance for six (6) non-ambulatory residents. Hospice waiver approved for four (4). Common areas: The living room and dining room were observed to be neat, clean, and organized. The rooms were observed to be properly furnished and in good repair. The facility maintains a comfortable temperature at 74°F. LPA observed a fire extinguisher to be located underneath the kitchen sink and dated 09/14/2025. LPA observed required postings such as Long-Term Care Ombudsman, Emergency Disaster Plan, and Personal Rights to be located throughout the common areas. A working telephone was observed. Kitchen: The kitchen was observed to be clean and free from pests. Sufficient supplies of seven (7) day nonperishable foods and two (2) day perishable foods were observed. Knives/sharps were observed to be kept in a locked kitchen drawer. The cleaning solutions/toxins were observed to be kept locked underneath the kitchen sink. Kitchen appliances were observed to be working and in proper condition. (continued on LIC 809-C) Bedrooms: The residents’ rooms are adequately furnished with appropriate furniture and lighting system. Hallways/passageways are lighted appropriately. Extra linens/covers were observed to be stored in each resident’s room and a storage dresser located within the hallway’s passageway. Bathrooms: The bathrooms were checked for cleanliness and proper operation. The hot water temperature was measured within regulations. LPA observed appropriate grab rails and slip-resistant mats to be in proper condition. Backyard: The backyard of the facility is equipped with a designated shaded area with outdoor furniture for residents. LPA observed there to be a pool. LPA observed the pool to properly fenced and locked. Garage: The garage is located outside in the backyard separate from the facility. The garage was observed to be kept locked and used for storage purposes. Laundry Room: The laundry appliances were observed to located within the garage. LPA observed cleaning solutions and toxins stored appropriately within the garage and inaccessible to residents. The laundry appliances were observed to be working and in proper condition. Medications: The medications were observed to be kept locked in a storage closet. First-aid kit observed to be equipped with but not limited to bandages, scissors, digital thermometer, tweezer and First-Aid Manuel. Smoke detectors and carbon monoxide observed to be working properly and were tested. Staff Office: The staff and resident files are kept in the staff office. LPA observed the staff office to be kept locked. Residents/Staff Records: LPA conducted a complete file review of resident records. Resident records appeared to be complete and updated. Staff records: LPA conducted a file review of staff records. LPA observed one (1) of five (5) staff members (S2) not to have fingerprint clearance. Further record review revealed S2 did not have a personnel record. LPA observed the staff member (S2) to be working and helping residents. Per the Co-Administrator, S2 had just begun to work at the facility. The Co-Administrator stated they would be taking S2 this week to get their fingerprint clearance. LPA’s record review of The Department of Social Services Guardian and Licensing Information System (LIS), confirmed S2 was not associated nor cleared to work within the facility. Additionally, LPA observed one (1) of five (5) staff members not to have their Health Screening Report with their tuberculosis screening. (Continue to LIC 809-C) Citation issued, please refer to LIC 809-D A civil penalty for $100 per day for a maximum of 5 days in the amount of $500 for criminal record clearance violation is being assessed on the attached LIC 421BG. There were no other immediate health and safety hazards observed during the day of inspection. Licensing Program Analyst (LPA) Abeye Duguma met with Ezros Stella for a Required One (01) Year visit. LPA explained the reason for the visit. A tour of the physical plant was conducted at around 8:00 AM and the following was noted: There is one entrance being utilized at the facility. The facility has a total of six (06) bedrooms and six (06) bathrooms. The facility is fire cleared for six (06) non-ambulatory and a hospice waiver for four (04). The facility is currently occupying four (04) residents. The facility has outdoor furniture with a covered shaded area for residents and visitors. The facility does not have a swimming pool/body of water. Laundry detergents, cleaning agents and other toxins are locked away. Kitchen is sufficiently stocked with at least two (02) days perishable and seven (07) days non-perishable food. Frozen foods are wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. Knives and sharps are observed to be locked and inaccessible to residents. The living and dining room are neat and clean. The facility maintains a comfortable temperature at 75°F. The smoke and carbon monoxide detectors are hardwired, interconnected and observed to be operational. Fire extinguisher is located in the kitchen, observed to be fully charged. (continued on LIC 809-C) The residents' rooms are adequately furnished with appropriate lighting system. Hallways are well lit. Residents have enough personal hygiene product provided by the licensee. The bathroom was checked for cleanliness and proper operations. The hot water temperature was measured at 115.2°F. Towels and washcloths are not shared. There was enough clean linen available in the cabinets. LPA observed medication to be locked and inaccessible to residents. Facility maintains a complete first aid kit. No health and safety hazards noted during the visit. Exit interview conducted. Licensing Program Analyst (LPA's) Gina Saucedo, Michael Cava and Angela Panushkina arrived at the facility at 10:30 am to conduct an annual. LPA's met with Natalya Sadovskaya and explained the purpose of this visit. She then called the administrator Stella Ezros whom arrived about ten minutes after. Stella Ezros walked us through the facility for our physical plant inspection. A tour of the physical plant was conducted at 10:55am and the following was observed: The facility has six (06) bedrooms and (06) bathrooms located in the bedrooms currently occupying (1) one residents. The facility has one main entrance being used, there are required Covid-19 prevention signage (handwashing, coughing etiquette, and physical distancing) posting in the entrance. The PPE screening station is located on a table at the entrance equipped with sufficient PPE readily accessible, a thermometer, hand sanitizer, gloves, mask, and sign in sheet at the time of visit. The facility maintains an overall temperature at 75 degrees Fahrenheit. Kitchen: The LPA's observed the kitchen to be clean and an adequate supply of perishables and non-perishable food. There is a pantry storing dry food, condiments, and can goods above the refrigerator. Food was properly labeled and stored. All appliances observed to be operational and in good repair. Fire extinguisher observed to be under the kitchen sink fully charged. Sharps and chemicals were observed to be stored in the kitchen pantry inaccessible to residents in care. LPA's observed (6) bedrooms to be appropriately furnished with sufficient lighting. LPA observed appropriately bed linen and comforters on all beds. All bedrooms are single occupancy. All bedrooms observed to be clean and clear from obstruction. LPA's observed (6) bathrooms which are located inside of the bedrooms to be clean and in proper operation. Water temperature measured at 115.2 degrees Fahrenheit. LPA's observed appropriate grab bars in shower and toilet area with appropriate non-skid mats in six (6) out of (6) bathrooms. Bathrooms are stocked and equipped with soap and private towels use. Medications: LPA's observed the locked medication cabinets in the medication room located in the hallway. LPA's observed the first aid kit and manual located and stored in the medication room. Living, dining room and common areas: LPA's observed these areas to be appropriately furnished with tables and chairs and adequate lighting. Observed to be neat and clean. The smoke detector was tested and observed to be working, it is not hard wired and interconnected throughout facility. The facility has does not have a carbon monoxide detector a Type A citation was issued. Backyard: LPA's observed the outside area and surrounding the facility it was clean and clear from obstruction and debris. The facility has a covered area with a table and chairs for seating, and additional table and chairs for lounging. LPA's observed a swimming pool with a self-latching gate measured a pad lock that requires a key located on the premises to be locked during the visit. LPA's observed a detached garage that is separated into two sections to be locked and storing extra personal care items, incontinent supplies, PPE in one section, and the laundry machines and toxins, extra wheelchairs, walkers, and mattresses on the second section of the garage.
