Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Talwinder Bains arrived on 6/23/26 to conduct the annual inspection. LPA met with Assistant Administrator, Jacob Bostanchyan and explained the purpose of today's visit. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed two (2) residents and two (2) staff files and found all required documents. LPA and administrator toured the facility together to ensure the health and safety of residents in care. The areas toured included resident rooms, bathrooms, kitchen, and common areas and outside area. The food supply is within compliance, 2 days of perishable and 7 days worth of non-perishable food items. Grab bars were present at the toilet and in the shower. All exits were unobstructed. There is a side gate for emergency access. LPA checked the kitchen area for the ability to prepare and store food. Knives and Sharp objects found to be locked. LPA observed the area used for medication to be locked and inaccessible to residents. LPA observed smoke detectors and carbon monoxide detector at the care home are operational. Fire extinguisher was last serviced on 6/16/26 and was ready for emergency use. Hot water temperature was observed to be 105 degrees F, which is within the regulation range of 105-120 degree. Inside temperature was 73 degree F. Facility was clean and well organized. All required postings were observed. Facility is conducting quarterly fire and disaster drill as required. LPA requested a copy of the LIC 500, LIC610E and current liability insurance to be sent to the Department by 07/10/26. No deficiencies were observed or cited per Title 22, CCR Regulations during this visit. LPA met with Assistant Administrator, Jacob Bostanchyan and explained the purpose of today's visit. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed two (2) residents and two (2) staff files and found all required documents. LPA and administrator toured the facility together to ensure the health and safety of residents in care. The areas toured included resident rooms, bathrooms, kitchen, and common areas and outside area. Allegation investigated: Facility is trying to evict resident illegally.
On 01/29/25, Licensing Program Analyst (LPA) Talwinder Bains and Licensing Program Manager (LPM) Laura Munoz arrived at the facility unannounced to deliver complaint findings and met with Administrator, Jacob Bostanchyan. The department conducted records review ,facility observations, staff and residents interviews to investigate the complaint. **Report continued on LIC9099-C** Unfounded ***Report continued from 9099.... Allegation- Facility is trying to evict resident illegally. The Department conducted facility’s observations, reviewed records, interviews with one staff and one resident to investigate this allegation. Staff interview disclosed that facility was trying to vacate resident, R1 room as repair work (new floor, electric work and other repair) were needed in R1s room. The facility indicated the work would be completed in February 2025. Facility notified R1 and R1s family and placement agency in January 2025 regarding this matter and issued ‘Temporary Relocation’ notice for R1. Facility reached out to Department regarding to this matter prior to this complaint and rescinded this ‘Temporary Relocation’ notice per Department’s directions since it was not allowable under Title 22 Regulations. Record review and staff interview reflected that facility never issued an eviction notice to R1 regarding this issue. Interview with R1 indicated that they were happy with the care and services from staff and were aware that repair work was needed in their room and facility was not trying to evict them. During facility visit on 01/14/25, Department observed that repair work was needed in R1s room. Based on gathered information, this allegation was found to be UNFOUNDED. Based on information obtained, LPA finds the allegation to be UNFOUNDED-means that the allegation is false, could not have happened, and/or is without a reasonable basis. Licensing Program Analyst (LPA) Talwinder Bains arrived on 07/23/24 to conduct the annual inspection. LPA met with Assistant Administrator, Jacob Bostanchyan (Jacob) and explained the purpose of today's visit. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed two (2) residents and two (2) staff files and found all required documents in residents files but missing CPR/FIRST AID certification in staff, S1s file and citation has been issued as indicated on LIC809-D. LPA and Jacob toured the facility together to ensure the health and safety of residents in care. The areas toured included resident rooms, bathrooms, kitchen, and common areas and outside area. The food supply is within compliance, 2 days of perishable and 7 days worth of non-perishable food items. Grab bars were present at the toilet and in the shower. All exits were unobstructed. There is a side gate for emergency access. LPA checked the kitchen area for the ability to prepare and store food. Knives and Sharp objects found to be locked. LPA observed the area used for medication to be locked and inaccessible to residents. LPA observed smoke detectors and carbon monoxide detector at the care home are operational. Hot water temperature was observed to be 109 degrees F, which is within the regulation range of 105-120 degree. Inside temperature was 76 degree F. Facility was clean and well organized. All required postings were observed. Facility is conducting quarterly fire and disaster drill as required. LPA requested a copy of the LIC 500, LIC610E and current liability insurance to be sent to the Department by 08/05/24. LPA reviewed medications of two (2) residents (R1,R2) comparing with physician orders and issues were observed with R1s medications management. LPA observed that facility's only Fire Extinguisher was last serviced on 10/19/2022. Deficiencies were observed and cited per Title 22 Regulations during this visit as listed on LIC809-D. Type of visit: Case Management - Incident
Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced on 05/13/24 to do case management visit. LPA met with administrator David Bostanchyan and explained the purpose of the visit. Department followed up on Incident Report (IR) sent by facility on 05/08/24 stating that resident, R1 had a wrist fracture which was diagnosed on 05/08/24 after staff noticed swelling in R1s right hand on 05/06/24 around 11am and notified administrator. IR indicated that administrator notified R1s family and doctor regarding this hand swelling on 05/06/24. R1s doctor ordered lab work and X-RAY and results dated- 05/08/24 indicated wrist fracture on R1s right wrist .Department conducted interviews with resident, R1 ,administrator and 1 staff member regarding this incident. At this time, this case is under review and department will do follow up as needed. No citations were issued per Title 22 Regulations. Type of visit: Case Management - Incident
On 11/08/23, LPA Bains conducted an unannounced case management visit to the facility to follow up on an incident which happened at facility on 05/31/23 related to 2 facility staff members. LPA met with administrator, David Bostanchyan and explained the purpose of today’s visit. The department conducted a record review to investigate this incident. Based on the records reviewed, it has been determined that the facility did not report an incident to the Department where staff, S1 made inappropriate sexual conduct to staff S2 on 05/31/23. Law enforcement was called and S1 got arrested due to this incident. Per facility records, S1 and S2 were ‘live-in’ staff at the time of incident. The facility should have reported this incident to CCL as required by Title 22. Although this incident did not involve residents but S1 and S2 were on duty at the facility and were responsible for the care and supervision of the residents ultimately putting the residents’ health and safety at risk. Additionally, it has been concluded that administrator was aware about this incident which happened at the facility between staff, S1 and S2 but administrator did not comply with reporting requirement. During today's visit, administrator stated that facility send SIR (LIC624) for this incident to CCL via Fax but facility could not provide any verification to prove it when LPA requested. Per California Code of Regulations, Title 22 Division 6, Chapter 8, deficiencies are being cited on the attached 809-D page. Exit interview conducted.
