Type of visit: Required - 1 Year

05/04/2026, Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr. inspection visit for this facility. There are currently no residents in care. Facility has an approved fire clearance for 6 non-ambulatory. Upon arrival no staff were present. Administrator, Reggie Capili arrived at approximately 10:30am. LPA and Administrator toured the building and grounds. Facility has 4 bedrooms, 2 bathrooms, a living room, and an activity room. The facility was found to be at a comfortable temperature. All rooms were furnished and in good repair. Extra hygiene products and linens were available. Water temperature in sinks were measured and found to be within the range of 105 to 120 degrees F. Fire extinguishers were found to be recently purchased. Smoke/Carbon Monoxide detectors located throughout the facility are hardwired and were tested and operational. Sharps were located in a locked box in a kitchen drawer. Chemicals were found to be inaccessible in the garage. Facility has a medication storage closet in the hallway where medication will be kept. LPA conducted review of two staff records. Upon a review of staff records, LPA found staff to have required training as well as current 1st Aid & CPR certification on file. Administrator certificate for Reggie Capili is current from 01/18/2025 - 10/22/2027. No deficiencies cited during today's inspection. Updated copies of the following documents were requested for facility file and are to be submitted to CCL by 06/04/2026: LIC500- Personnel Report LIC308- Designation of Responsibility Updated Liability Insurance LIC610- Emergency Disaster Plan (review, update if needed) Exit interview conducted with Administrator and a copy of this report was provided.the state’s words, verbatim · CDSS document, May 4, 2026

Type of visit: Post Licensing

09/11/2025; Licensing Program Analyst (LPA) Loera made an unannounced Post Licensing Visit. There are currently no residents in care. Facility approved/cleared for 6 ambulatory. LPA was greeted by Administrator, Reggie Capili. LPA was notified facility is currently working with a fire marshal for regulations regarding non ambulatory as facility plans to receive approval for non ambulatory residents. Administrator will send an updated facility sketch to CCL when completed. LPA toured the building and grounds. The facility was found to be at a comfortable temperature. Facility had required postings. All exits were free from obstruction. All rooms were furnished per regulation. All rooms were in good repair. Extra hygiene products and linens were available. Water temperature was measured and found to be within the range of 105 to 120 degrees F. Fire extinguishers were found to be fully charged. Smoke/Carbon Monoxide detectors located throughout the facility were tested and operational. Toxins, sharps and other items that could pose threat if available to residents were located in the garage and in the kitchen and were found to be secured. Medication storage closet was observed to be locked. Facility has an activity room with various games available for resident use such as chess, karaoke, and painting, along with an activities calendar that will be rotated monthly. May 22, 2025Facility evaluation reportReport on file

Type of visit: Prelicensing

At approximately 11:15 AM, Licensing Program Analyst (LPA) Magdaleno arrived announced to conduct a Pre-Licensing Inspection and met with Applicants, Reggie Capili and Marife Capili. Facility received an approved fire clearance dated 11/27/2024 that allows for a total capacity of 6 clients, where 6 may be ambulatory only. Facility plans to be a Residential Care Facility for the Elderly (RCFE). LPA conducted a walk-though of facility with Applicant and observed the following: Per facility sketch, facility is a one story residence with 4 residents bedrooms, two bathrooms, 1 staff room, living/dining room, and an activity room. Facility was found to be clean and at a comfortable temperature with all exits free from obstruction. Facility has an Infection Control plan on file. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations. Facility's water temperatures for all sinks were within Title 22 Regulations of 105 degree F to 120 degrees F. There was an appropriate supply of cleaning products, linens, hygiene products, and paper products available for clients. Toxins were observed to be stored in facility's laundry room and inaccessible to clients. Bathrooms were equipped with necessary grab bars, and non-slip floors mats were present. All client rooms were furnished with a bed, lamp, dresser, and chair with shared rooms having two of each. Facility has sufficient items for cooking and serving. Per Applicant, Staff and Client file will be located in the locked office. Medication closest was found to be locked. Facility has indoor/outdoor areas for visiting and activities. Signal system on all doors leading out of facility were tested and found operational. Facility's fire extinguisher is newly purchased and was observed to be fully charged. Facility's combination smoke and carbon monoxide detectors were tested and operational. LPA confirmed that contents of the facility's First Aid Kit was sufficient. Facility has internet accessible devices and internet available for residents in care. LPA and Applicants discussed many topics including reporting requirements, documentation requirements, grievance procedures, staffing, disaster drills, and yearly training requirements. Continued LIC809C... Continued from LIC809 No Deficiencies or Advisories given during visit. Pre-Licensing completed. Facility is ready to be Licensed as a Residential Care Facility for the Elderly (RCFE). LPA will submit Pre-Licensing Application Report to the Application Unit Analyst in Sacramento. Application Unit Analyst will notify Applicant of Status. Exit interview conducted. Copy of report discussed and provided to Applicant.
