Allegation investigated: Staff did not ensure the facility met the fire safety requirements. Staff unlawfully evicted a resident.

On 07/28/2026, Licensing Program Analyst (LPA) Beena Singh conducted an unannounced visit to the facility to deliver findings on above allegation. LPA Singh was greeted and granted entrance to the facility by a staff and facility representative, was contacted and informed of the visit. LPA Singh explained the purpose of today's visit to The investigation conducted by LPA Singh consisted of interviews and records review. First Allegation:-Staff did not ensure the facility met the fire safety requirements. LPA Singh reviewed records and fire safety inspection was conducted on August 27, 2024 and LPA Singh confirmed facility met the fire safety requirements for Five (5) non-ambulatory and one(1) bedridden residents. Based on the information gathered, LPA Singh was not able to find sufficient evidence to corroborate the allegation listed above. Statements, records, and interviews obtained did not provide sufficient information to corroborate the above allegation. Unsubstantiated Second Allegation:-Staff unlawfully evicted a resident. During a review of records, LPA Singh confirmed that no admission agreement had been signed by Resident 1 or their family with facility representatives. Facility Administrator Elen Markosyan and House Manager Joseph Walker stated that the family was not signing the paperwork since resident#1 moved in on 1/22, during the resident's stay, Resident#1 experienced a blood sugar spike that required hospitalization. Subsequently, the family informed the facility that Resident 1 would not be returning, noting that the facility had issued a notice of non-payment and required residency agreement to be signed on 1/27 but never issued a formal eviction letter to the resident. Based on the evidence found during the investigation, the allegations listed above Staff did not ensure the facility met the fire safety requirements and Staff unlawfully evicted a resident are deemed UNSUBSTANTIATED. A finding that the complaints are UNSUBSTANTIATED means although the allegation may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. During today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. On 07/28/2026, Licensing Program Analyst (LPA) Beena Singh conducted an unannounced visit to the facility to deliver findings on above allegation. LPA Singh was greeted and granted entrance to the facility by a staff and facility representative, was contacted and informed of the visit. LPA Singh explained the purpose of today's visit to Administrator, Elen Markosyan, and House Manager, Joseph Walker. The investigation conducted by LPA Singh consisted of interviews and records review. Allegation:-Staff did not get consent from authorized representative prior to relocating resident. LPA Singh reviewed records and interviews conducted with Administrator Elen Markosyan and House Manager Joseph Walker, LPA Singh was informed that Resident #1 was not relocated because the resident had been transported to the hospital due to a spike in their blood sugar levels, family were contacted/informed and family contact/information was provided to the paramedics by the facility. Additionally, the resident's family subsequently informed the facility that Resident #1 would not be returning to the facility from the hospital. Unsubstantiated During the investigation, LPA Singh was not able to find sufficient evidence to corroborate the allegation. Four(4) out of Five(5) residents and Four(4) out of Four(4) staff confirmed that Staff do communicates with the responsible person and residents regarding any care, change of condition or scheduled appointments. Based on the evidence found during the investigation, the allegation listed above Staff did not get consent from authorized representative prior to relocating resident is deemed UNSUBSTANTIATED. Licensing Program Analysts (LPAs) LaVette Farlow and Beena Singh conducted an unannounced visit to this facility to initiate a Case Management visit for complaint numbers: 56-AS-20260126103801 and 56-AS-20260129135530. LPAs met with Administrator, Elen Markosyan and House Manager Joseph Walker. During today's visit, LPAs conducted interviews with residents, staff, obtained and reviewed facility records, and did a walk-through of the facility. During today's visit no health and safety concerns were discovered. No deficiencies were cited during this visit. Licensing Program Analysts (LPAs) LaVette Farlow and Andrew Martinez arrived at Aurora Comfort Care Home Inc to conduct an unannounced annual inspection. LPAs were greeted by Ryan Quiboloy, Caregiver. LPAs introduced themselves and stated purpose of the visit. LPAs met with House Managers Floretta Castro and Joseph Walker, and Floretta Castro escorted LPAs on a tour of the facility. During the tour the following was observed inside and outside: Structure: Facility is a one story house with (4) resident bedrooms, (2) resident bathrooms, office/staff area, living room, dining area, kitchen, pantry, backyard, and attached garage. The facility is equipped with a central heating and air conditioning system installed with the central panel located in the hallway to control entire house. Two bedrooms accommodate two non-ambulatory residents in each room, and two bedrooms across the hallway accommodating one non-ambulatory resident and 1 bedridden resident privately in each room. The resident bathrooms have working toilets, wash basins, and showers with an adequate supply of toilet paper and soap. Kitchen/Bathroom/Laundry: An adequate supply of dishes, glasses, utensils, pots, and pans were observed. Knives, sharps, detergent, and chemicals are stored in locked cabinets. The facility pantry has non-perishable and perishable food for residents in care. LPAs observed an insufficient amount of non-perishable food for the number of residents in care for 7 days. A Deficiency cited. LPAs observed the emergency food supply. Refrigerators and freezers are in working condition. Water tested in the bathrooms and kitchen faucet measured at 137.9, 135.9 and 126.1 degrees Fahrenheit. A deficiency cited. The facility has a washer and dryer located inside the garage. Living/Family Room/Hygiene Supplies: The facility has sufficient amount of furniture for number residents in care in living room with reading material and a TV observed. The facility maintained an adequate supply of linens and hygiene supplies stored in a closet for residents in care. Yards/Outside: A self-latching handle gate on the left and right side of the house that leads into the backyard. There are no firearms, ammunition, swimming pool or bodies of water observed. LPAs observed the Licensee is completing construction on a second home in the backyard. The construction work is scheduled to be completed in the next 30 days. LPAs observed the left side of the house walkway has rocks that can be a potential hazard. LPAs recommended the area be cleared of any debris and hazards. A Technical Advisory issued. Facility Files: The facility has an active landline telephone that LPAs verified. The facility maintained a facility sketch, visiting hours, See Something Say Something, and personal rights postings observed in the common areas. LPAs reviewed the facility files and observed the Licensee has not reviewed nor updated the Infection Control Plan and the Emergency Disaster Plan. A Technical Violation issued. Record Review: LPAs reviewed three (3) residents file for Admission Agreement, Physician Report, and Needs and Service Plan. LPAs observed that 3 out of 3 residents file were maintained and complete. Medications/Medication Administration Records (MARs) were audited for three (3) residents. LPAs observed medication errors of the 3 residents (R1, R2, R3) with their medication(s) still in bubble pack(s) and not dispensed, as well as medication dispensed without being properly logged on the residents' MARs. Deficiency issued. LPAs reviewed three (3) staff files for Criminal Record Clearance, Health Screening, TB test results, CPR/First Aid, and Training. LPAs observed personnel file were maintained and complete. General Items: The smoke and carbon monoxide detectors were tested and are fully operable. There is one fully charged fire extinguisher observed. First Aid kit with required components, and a locked area for medication storage was observed. During today's visit, three (3) deficiencies were cited and three (3) technical violation or advisories are being issued in accordance with the California Code of Regulations (CCR), Title 22. Licensing Program Analyst (LPA) LaVette Farlow, arrived at Aurora Comfort Care Home INC, to conduct an announced Pre-Licensing visit for licensure. LPA was greeted by Administrator Georgi Amirjanyan. LPA introduced self and stated purpose of the visit. LIC200 application was submitted on 12/09/24 for (5) non-ambulatory residents, and (1) bedridden resident. Fire Safety Inspection clearance was granted for (5) non-ambulatory residents, and (1) bedridden resident on 08/29/2024. LPA toured the facility inside and outside and observed the following: Structure: Facility is a one story house with (4) resident bedrooms, (2) resident bathrooms, office/staff area, living room, dining area, kitchen, pantry, backyard, and attached garage. Heating/Cooling System: Central heating and air conditioning system installed with a central panel located in the hallway to control entire house. Bedrooms: Two bedrooms accommodates two non-ambulatory residents in each room, and two bedrooms across the hallway accommodates one non-ambulatory private and 1 bedridden private resident in each room. Bathrooms: The resident bathrooms have a working toilet, wash basin, and shower with an adequate supply of toilet paper and soap. Kitchen/Laundry: An adequate supply of dishes, glasses, utensils, pots, and pans were observed. Knives, sharps, detergent, and chemicals are stored in locked cabinets. There was a pantry stocked with non-perishable food and perishable food found in the refrigerator. LPA observed the stove to be operational. Refrigerator/freezer were in working condition. Water tested in the bathroom and kitchen faucet measured at 109.7, 121.9 and 119.6 degrees Fahrenheit. A functional washer and dryer are located inside the garage. Living/Family room: There was a furnished living room with reading material and a tv observed. Linens and Hygiene Supplies: An adequate supply of linens and hygiene supplies stored in a closet. Yards/Outside: A self-latching handle gate on the left and right side of the house that leads into the backyard. There are no firearms, ammunition, swimming pool or bodies of water observed. LPA observed construction work in the backyard. Licensee is building a second home on the grounds, and expected completion date is March/April 2025. All outdoor pathways were free of obstructions. Emergency Phone Numbers, and Exit Plan: Facility sketch, visiting rules, and personal rights were observed posted by the entrance. General items: The smoke and carbon monoxide detectors were tested and are operable. There was two fully charged fire extinguishers observed. Resident/Staff records stored inside the locked office. First Aid kit with required components, and a locked area for medication storage was observed. The facility was evaluated in accordance with the California Code of Regulations (CCR), Title 22. Based on the observations and evaluation of the facility this date, the facility is ready for licensure. LPA completed COMP III with the Licensee at the conclusion of the inspection. Licensee will be notified once facility is licensed.
