Licensing Program Analyst (LPA) Imaculada Vasquez arrived unannounced to conduct an annual inspection. Upon arrival LPA was greeted by Caregiver, Rose Tabula and granted entry. Administrator, John Allen Bulaoro arrived 15 minutes later. LPA began inspection with introduction and visit purpose. There are currently three (3) residents living at the facility. The single story facility consists of four (4) bedrooms and two (2) baths. Resident record review began- Three (3) records were reviewed. LPA reviewed for admission agreement, medical assessment and TB test results, consent forms, identification and emergency information, appraisal needs and service plans, centrally stored medication/destruction records, and personal rights notification. This facility meets documentation requirements. Physical Plant and Safety of Environment/Operational Requirements- LPA toured the facility inside and outside. The home is maintained at a comfortable temperature for the clients. Lighting is sufficient for safety and comfort. Water temperature measured 106.8 degrees F. Laundry chemicals, cleaning chemicals, and sharped objects are observed in a locked cabinet. Medications are locked in a cabinet. LPA verified there is a telephone working at this location (951)223-3018. Food Service- Food supply meets the requirement of one (1) week supply of nonperishable and two (2) day supply of perishables food on hand. Food prep areas are clean and organized. (Continued on next page) (Continued from Page 1) LPA began review of employee records- Three (3) records were reviewed. LPA reviewed employee records for first aid, fingerprint clearance, personnel/job application, health screening, criminal record statement, employee rights, training verification, and current administrator certifications. CPR and First Aid requirements have been met. The facility employs enough staff to maintain cleanliness and meet the needs of the clients in care. Administrator certifications present and current, expires on 12/02/2027. LPA made observation throughout the inspection process to assess if the facility remains in conformity with the State Fire Marshall regulations. The facility has not exceeded its capacity limitation and the structure remains unchanged according to the approved floor plan. Smoke detectors and carbon monoxide detectors were tested and found to be operational. One (1) Fire extinguisher is tested or replaced annually and was last done so on 04/29/2026. The facility is conducting quarterly emergency disaster drills. The last disaster drill was conducted on 04/21/2026. Type of visit: Case Management - Other

Licensing Program Analyst (LPA) Imaculada Vasquez conducted an unannounced case management visit in order to increase the capacity per licensee request. LPA met with Administrator, John Bulaoro. Licensee request for capacity increase approval of building from four(4) to six(6). Fire Clearance is approved for six (6) bedridden clients approved by the Riverside Fire Department on 05/13/2026. Facility sketch on file with designation of capacity for each room; licensee is advised that the noted designated capacity for each room is to remain in compliance as described on the facility sketch and each room capacity attachments. All rooms are designated for bedridden residents. Resident bedrooms observed to be in sufficient compliance to capacity increase. LPA observed the Administrator has installed sprinkler system and appropriate fire alarms. Bedroom #4 has a required door leading to the exterior of the facility. The physical plant is ready for increase in capacity. There are currently three(3) residents in care as of today. . Type of visit: Prelicensing

On May 13, 2025, Licensing Program Analyst (LPA), Venus Mixson arrived for a scheduled visit for the purpose of conducting a Pre-Licensing visit. LPA Mixson met with Christine Bulaoro, Administrator, introduced herself, and stated the purpose of the visit. LPA toured the location along with the Administrator and made observations following is a summary. Physical Plant: The location is a single-story facility located at 32454 Bullard Street Menifee CA 92584, and has four bedrooms, two full bathrooms, a living room, dining room, and a kitchen. A backyard and front yard with two car drive-way. The Riverside County Fire Department City of Menifee approved this facility for zero (0) non-Ambulatory, zero-Ambulatory, and four (4) bedridden residents, on April 28, 2025. The home has a first aid kit and manual, the Administrator has received First Aid and CPR training, and the Administrator’s certificate is current, and posted. With an expiration date of 11/18/2025. Medications: LPA Mixson observed where medications are to be stored, locked, and inaccessible to the residents. The home is equipped with lights in the passages and stocked with emergency night lights throughout the home. The smoke and carbon monoxide detectors were observed and are operable. LPA Mixson observed one brand new fire extinguisher, charged and in the green, and obtained on 04/10/2025. The cleaning supplies were locked and inaccessible, along with the sharp objects and the Administrator informed LPA there are no firearms on the premises. Bedrooms: four (4) bedrooms were observed and are large enough to allow for easy passage. The required items of furniture were observed currently at the time of this inspection. Chair, nightstand, and sufficient lighting. Kitchen/Food: Knives were locked in a kitchen drawer, plenty of pots, pans, and other kitchen accessories. Facility has the required seven-day supply of non-perishable food items. LPA Mixson observed hygiene supplies for residents. There were no pesticides, poisons, or other toxic substances stored in any food storage or preparation area currently at the time of this inspection. (87555). Records: There is confidential storage space for personnel records at the time of this visit. (87412). Bathrooms: Two bathrooms floors were clean and sanitary and free of odors. Other appliances were operable, water temperature tested within regulations and was logged. There was at least one toilet and sink for each six persons to include residents, family, and personnel. (87307), and at least one bathtub/shower for each ten persons which includes residents, family and/or live-in personnel. (87307). Administration: LPA Mixson observed emergency exiting plans and telephone numbers posted, Personal Rights, Complaint Poster, and other required documents were posted currently at the time of this visit, along with the current Administrators certification, which has an expiration date of 11/18/2025. The facility theft and loss program policy are posted (87218). Activities: There are activity supplies and equipment, including access to a variety of reading materials in the language of choice. There is an outdoor activity area equipped for outdoor use. (87219). Miscellaneous: There is a first aid kit, including sterile dressings, bandages, thermometer, and other items as required by regulations. (87465). There are laundry supplies and equipment and there is space for clean linen storage and a separate space for soiled linen. (87307). LPA Mixson observed emergency lighting supplies to include flashlights, and extra batteries. (87303). Dementia Care: There were no bodies of water on the premises currently at the time of this visit, and there are auditory devices in place to monitor exits and completely enclosed outdoor activity space with self-closing latches and gates. (87705). Inside/Outside: All doors, and passageways are clear of obstruction. There were no bodies of water on the premises, and the fireplace was covered with a screen. There was enough clean linen and hygiene items, and there was appropriate lighting in each room. LPA Mixson observed central heating and air conditioning systems, and they are operable currently. The Administrator dialed the land line phone number (951) 223-3018, and it was operable. Outside/Yards: Had shade and covering for shaded visits, and activities. There were no obstructions observed. The Licensee stated there were no firearms, and/or ammunition on the premises. CAB 8.0 Pre-licensing / COMP III requested and anticipation to scheduled.
