On 04/21/26, at 09:20am, Licensing Program Analyst (LPA) Gina Saucedo arrived at the facility to conduct an unannounced, annual visit. LPA met with, Mary Esparrago, Administrator and was advised of the visit. LPA asked for the census, resident, and staff files. There is one (1) entrance being utilized at the facility. The facility has a total of four (04) resident bedrooms and two (02) and 1/2 bathrooms. The facility has a hospice waiver for six (06) residents. Outside/Backyard: The facility has outdoor furniture with a covered shaded area for residents and visitors. The facility does not have a swimming pool/body of water. There is a detached garage. The garage has extra supplies and utilized as parking. There is an extra refrigerator outside. Kitchen area: Kitchen is sufficiently stocked with at least seven (07) days perishable and seven (07) days non-perishable food. There is one (1) refrigerator with frozen foods wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. Knives and sharps are observed to be locked and inaccessible to residents on one (1) of the left upper cabinets. There are toxins/chemicals under the sink locked and inaccessible to the residents. Fire extinguisher is located near the exit of the kitchen leading to the washer and dryer. It is fully charged and last purchased 02/2026. The medication is locked and inaccessible to the residents in a cabinet in the kitchen. LIC 809C continued No firearms observed or will be maintained on the premises. The smoke alarm and carbon monoxide detector were tested and operational. They are hardwired. The facility has fire sprinklers. Living Room and Dining Room: The living and dining room are neat and clean. The facility maintains a comfortable temperature at 75°F. There is a fire place that is covered. Bedrooms: Facility has four (4) bedrooms and were toured. The bedrooms are fully furnished with proper lighting and bedding. Four (4) of the bedrooms are currently single occupied. Bathrooms: There are two (2) 1/2 bathrooms. The bathrooms contained a trash can with tight-fitting lid. One (1) of the bathrooms is in the hallway. One (1) is in a resident room-private and the 1/2 bathroom is also in the hallway. Hot water was tested and measured 117.1 within regulations. Administrative: The administrative Certification is current and expires 08/29/2027. The liability insurance expires on 06/01/26. The Ombudsman Sign, YES, Rights of Resident Council and Personal Rights, Theft and Loss, House Rules and Designated Facility Responsibility is against the wall of the facility. The facility has a signal system and cameras in common areas. Staff/Client Files: Three (3) staff files were reviewed and four (4) resident files were reviewed. The last fire drill was on 01/2026. At 9:45a.m., Licensing Program Analyst (LPA) Antonia Alvizar-Ettima conducted an unannounced Required One (1) year inspection to the facility. LPA met with caregivers that granted entry to the facility and explained the reason for the visit. Later, Administrator joined today’s visit. At 10:30a.m., LPA and Administrator conducted a physical plant tour inside and out. During the tour, LPA observed that the facility is a home located in a residential community. Required postings were observed in the dining area. The facility has fire sprinklers throughout the home and fire extinguishers were observed. Fire extinguishers charge date is 02/05/2025. There were combo smoke/carbon monoxide detectors in each bedroom and hallway. Fire/Earthquake emergency drill was last conducted on 05/19/2025. During this visit the facility is at 73 degrees Fahrenheit. The facility is licensed for six (06) residents over the age of 60, six (06) bedridden, hospice waiver for six (06) residents. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following: Kitchen: LPA observed kitchen appliances and fixtures were functional. The kitchen has a working gas stove, faucet, freezer, refrigerator, and microwave. LPA found enough at least two (02) days perishable and seven (07) days non-perishable food at the facility that is properly stored. Frozen foods are wrap, dated, and stored properly as well. Knives were stored in a locked drawer in the kitchen. Food storage and preparation areas are clean and inaccessible to pests. Garbage cans have tight fitting covers. Cleaning supplies, pesticides or toxic cleaning supplies were stored and locked away in the kitchen under the sink. Cont. LIC 809-C Cont. LIC 809 Bedrooms: LPA observed four (04) bedrooms designated for residents use and one (01) bedroom designated for staff. All bedrooms were toured and observed to be clean and properly furnished with appropriate dresser, beddings, and linens with sufficient lighting. Bathrooms: LPA observed two (02) full bathrooms and a half (1/2) bathroom to be clean, sanitary and with necessary supplies. The appropriate grab bars and mats in the shower. Hot water temperature measured at a range of 116.9°F to 117.2°F and within the required range. Resident’s personal hygiene supplied are kept separate in caddy container stored in a locked cabinet under the sink. Towels and washcloths are not shared. Common Areas: These included the living room and dining area for residents. The common areas were properly furnished. Furniture in common area was observed to be in good repair. Residents dining table fits six (06) residents. No obstructions and or tripping hazards throughout the facility. There are no issues with Fire Clearance. Surrounding Grounds: Entry and exits were free of obstruction. There was furniture appropriate for outdoor use. The outdoor area was free of hazards. The outdoor area was enclosed, and no bodies of water were observed. Garage is detached from the house and observed to be locked and inaccessible to residents. The garage stores extra supplies and utilized as parking. Laundry Room: LPA observed a washer and dryer machine located next to the kitchen. Linens are stored in the hallway cabinet and observed to have ample supply of clean linen, comforters, and towels in facility. Cleaning and extra hygiene supplies are being stored in locked cabinets in the hallway. Medications: are in a centrally stored and locked medication cabinet the in kitchen, including over-the-counter medicines; medications are properly labeled and checked for expiration dates. Each centrally stored prescription and PRN medication has been logged in the medications log with proper documentation from the resident’s doctor. First-aid has all proper items and were observed to be stored in locked cabinet in the kitchen. Staff Files: were reviewed they all have criminal record clearances and associated to this facility. Staff have current first aid and training documentation showing training completed. Administrator's certificate was observed to be current. Resident Records: All three (03) resident records were reviewed. Residents’ records are complete and current at this time. Resident were also interviewed. Facility is within CA code of Regulations Title 22 or Health and Safety Code. Licensing Program Analyst (LPA) Rosaura Valenzuela conducted an unannounced annual required visit. LPA met with Administrator Mary Esparrago and explained the reason for the visit. LPA inspected the physical plant, observed food supply, and reviewed resident and staff files. The facility is licensed for six (6) residents over the age of 60, with a hospice waiver for six (06) residents. There is only one entrance being utilized at the facility. LPA inspected the interior and the exterior of the facility with Administrator Mary Esparrago including but not limited to: the living room, dinning room, staff / storage room, kitchen, four (4) resident bedrooms, two (2) bathrooms, laundry area and backyard. All indoor and outdoor passageways were observed to be free from obstruction. There is no body of water. Bathrooms and bedrooms were observed to be clean and in good repair. There is a locked storage area for medications and toxins in the kitchen which is inaccessible to residents. Laundry area is located in the back of the kitchen. The kitchen was toured. All appliances were operating properly. There was a sufficient amount of perishable and non-perishable food. Kitchen knives and sharps are stored in a locked drawer in the kitchen. The common areas including the living room and dining room are clean and have the required furniture. The temperature was comfortable. The backyard has a shaded sitting area. All resident bedrooms were toured. Each bedroom has a smoke detector, a bed, linens, a dresser, a lamp and sufficient closet space. Both bathrooms were toured. Bathrooms have the required grabs bars and non-skid mats. The hot water was 117.2 degrees which is within the required 105 - 120 degrees. LPA observed a fire extinguisher near the kitchen which was fully charged and last serviced on February 2024. Smoke detectors were observed throughout the facility. First-aid Kit is complete and located in the kitchen. LPA reviewed residents files to confirm emergency contact information has been updated. LPA also reviewed staff files to confirm health screenings and fingerprint clearances. All staff files reviewed were fingerprint cleared.
