Licensing Program Analyst (LPA) Albert Johnson conducted an unannounced annual visit on this date. LPA met with Virginia and explained the purpose of the visit. The facility is a single story structure with wheelchair accessibility. LPA observed all required signage, including license to be prominently posted. LPA toured the facility indoors and outdoors including but not limited to dining room, living room, kitchen, covered garage/ smoking area, 5 bathrooms, 3 bedrooms and backyard. The facility is licensed for 6 clients. Hot water temperature was measured at 118.5 degrees Fahrenheit in resident's bathroom sink, which is within the required range of 105 to 120 degrees. Fire extinguishers are current and in compliance with fire safety. Carbon dioxide monitor present. Smoke detector are operational . LPA reviewed 3 resident and 2 staff files, including criminal record clearances. All staff today are associated to the facility. First aid kit was checked and is complete. Per the California Code of Regulations, Title 22, Division 6, Chapter 8, no violations cited during this visit. On 7-17-2025 at 12:58pm, Licensing Program Analyst (LPA) Michael Bilger arrived at this facility unannounced to conduct an annual inspection visit. LPA met with the administrator Corazon Poier and explained the purpose of the visit. LPA inspected the physical plant including but not limited to the kitchen, dining room, resident bedrooms; resident bathrooms, laundry room, living area, common TV area, and outside of the facility to ensure compliance with Title 22 regulations. Facility is an residential care facility for the elderly (RCFE) with a current census of 3. Facility has 5 bedrooms and 3 bathrooms for resident use. 1 extra bedroom is for staff use. Facility has a formal dining area and a formal living room. LPA also conducted the inspection using the CARE tool. Facility currently provides care for 1 ambulatory residents, 2 non ambulatory residents, 0 hospice, and 0 bedridden. The facility has an approved infection control plan in place. Water temperature reads 105*F to 120*F in the bathroom and room temperature reads 77*F. LPA observed the facility to have adequate food supply. Resident rooms were sanitary and had the required furniture and furnishings. The facility common areas were clean and furnished. Smoke and carbon detectors were in good repair. Fire extinguisher was checked 2-18-25. Facility has an emergency food and water kit. All toxins and other dangerous items including sharp objects were locked and inaccessible to residents in care. Medication storage area was observed to be locked and inaccessible to residents in care. Medications were reviewed and contained accompanying regulatory required Physician’s orders. First aid kit was observed to have adequate supplies and accessible to staff. {Cont. on 809C} During this inspection 3 resident files and 3 staffing files were reviewed for regulatory compliance. All files contained required contents including staff training requirements. All staff noted on LIC 500 contained criminal background clearances. LPA completed 2 resident interviews and 2 staff interviews. Resident files reviewed contained all required contents including updated admission agreements, medical assessments, and updated appraisal forms as required. Facility’s surety bond is current and update to date per regulatory requirements. Facility does not contain any bodies of water. LPA observed personal rights and complaint information posted. Facility has appropriate internet access available for resident use. LPA observed facility’s activity calendar and sufficient equipment and supplies to meet activity program needs of residents in care. LPA reviewed facility’s disaster plan to ensure regulatory compliance. Facility conducts monthly fire drills. LPA requested an updated copy of LIC 308 and LIC 500. Per California Code of Regulations, Title 22, no deficiencies were observed during this visit. On 06/18/2024 Licensing Program Analyst (LPA) Avelina Martinez made an unannounced visit to this facility to conduct an annual required inspection. LPA Martinez met with Virginia Cristobal and explained the purpose of the visit. LPA Martinez inspected the physical plant including but not limited to the kitchen, dining room, resident bedrooms; resident bathrooms, laundry room, activity room, and outside courtyards of the facility to ensure compliance with Title 22 regulations. Administrator holds current and expires on 01/18/2025. The facility is licensed 6 ambulatory clients, which two can be non-ambulatory and two may be bedridden. The facility has a hospice waiver for one. There are currently three residents who reside at this facility. The LPA Martinez toured the facility with Virginia Cristobal on 06/18/2024 at 11:00 AM. LPA Martinez reviewed three client files and two staff files. The facility files were maintained. The facility has the required postings, which are posted throughout the facility. In addition, the facility has a locked cabinet for medications. LPA Martinez reviewed two client medication administration records (MAR), and the MARs were maintained. The facility has an infection control plan and a natural disaster plan. The last fire drill was on June 09, 2024. The facility had and adequate food supply, and the kitchen was sanitary. The facility common areas, bedrooms, bathrooms, laundry room were sanitary and furnished. The exterior of the facility was free of debris, and the emergency exit gate door is in good repair. The facility temperature measured at 75 degrees, and the facility water temperature measured at 108 degrees. Based on this annual inspection there were no deficiencies cited.
