Licensing Program Analyst (LPA) Nadia Shahbazian conducted an unannounced Annual Required visit and inspection of the facility. LPA met with Administrator/Christine Lekhlyan and explained the reason for the visit. The facility is licensed to serve six (6) non-ambulatory clients and is approved for six (6) hospice waivers. Current census is three (3). Clients receive services from the Frank D. Lanterman Regional Center. LPA asked for copies of staff and resident rosters and files. At 9:15 am, with the assistance of Administrator LPA toured the physical plant and observed the following: Mandated postings were observed by the entry door and in the office area. Smoke/carbon monoxide detectors are interconnected. At 9:34 am, smoke/carbon monoxide detectors were tested and observed to be functioning properly. There are two (2) fully charged fire extinguishers, one in the kitchen and one by the rear exit door; both fire extinguishers were serviced on 03/24/26. LPA observed fire sprinklers throughout the facility. There are two (2) exits, one in the dining room and one in the hallway, leading to the backyard. Facility uses the main entry door as emergency exit. All exit doors were equipped with functional alarms. Facility conducts monthly fire/earthquake drills; the last fire drill was conducted on 03/13/26 and last earthquake drill was conducted on 02/26/26. Kitchen: The kitchen appliances consist of a refrigerator, stove/oven, dishwasher, microwave. All appliances and fixtures were observed to be clean and in functional condition. Knives and sharp objects were stored in a locked drawer in the kitchen. LPA found sufficient supply of perishable foods for two (2) days and non-perishable foods for seven (7) days. The kitchen cabinets were observed to have ample dishes and cook ware. Chemicals and detergents were locked in a cabinet underneath the kitchen sink. Properly labeled medications were locked in a kitchen cabinet. First Aid Kit with First Aid Manual were observed in the medication cabinet as well. Continued on 809-C Common Areas: The living room and dining area are located near the kitchen. There is a storage/pantry in the living room. The common areas were properly furnished with ample chairs/sofas for six (6) residents and staff. LPA observed a gaming area in the living room, along with a television set. Facility offers cable, internet and telephone for resident use. Bathrooms/Bathrooms: There are three (3) bathrooms designated for residents' use and one (1) bathroom for staff use. Bedroom# 3 has a wheelchair access shower but no toilet. Bedroom# 4 has a full bathroom. All bathrooms were sanitary and had the required grab bars and non-skid mats and functional fixtures. Hot water temperature was measured at 114.8 - 116.2 degrees Fahrenheit. There were six (6) private bedrooms designated for residents' use and one (1) for line-in staff; there is always awake staff at nights. All bedrooms in use by residents were properly furnished with appropriate beddings, chairs, linens and sufficient lighting. Bedroom #2 has a non-functional fireplace. Currently bedrooms #1, #5 and #6 are vacant. Surrounding Grounds: The backyard was large enough for activities. Outdoor furniture included a table, 6 chairs and an umbrella. The outdoor area was free of hazards and obstructions; no body of water was observed. The garage is detached and is currently used as office space. LPA observed all resident and staff files locked in a cabinet in the office area. Laundry room is adjacent to the garage/office. All laundry detergents were kept locked in the laundry room, inaccessible to clients in care. Staff Files: LPA conducted a file review of staff records to ensure forms and training certificates are up to date and compliant with licensing forms. Medications/Resident Files: LPA conducted a file review of resident records to ensure compliance of licensing forms. Medication were observed to be locked in a kitchen cabinet. Medication records were reviewed for proper documentation. LPA counted the medications for all three (3) clients and ensured proper dosage were given to the residents, based on physician orders. LPA reviewed LIC610E Emergency Disaster Plan. Liability insurance has expiration date of 09/28/2026 and Administrator Certificate has expiration date of 07/24/2027. Upon arrival at 11:15 am, LPA met with Lena Badalyan/Caregiver and explained the purpose of the visit. LPA telephoned Administrator/Christine Lekhlyan, who designated Lena Badalyan to sign the report. LPA asked for copies of staff and resident rosters and files.The facility is licensed to serve six (6) non-ambulatory residents and approved for six (6) hospice waivers. Current census is one (1); the resident receives services from the Frank D. Lanterman Regional Center. At 11:45 am, with the assistance of staff, LPA toured the physical plant area inside and outside to ensure there are no health and safety hazards. Mandated postings were observed by the entry door and in the office area. Smoke alarms and carbon monoxide detectors are interconnected and were tested to be functioning properly. There were two (2) fire extinguishers, both purchased on 12/16/2024, one in the kitchen and one by the rear exit door. There are two (2) exits leading to the backyard, the main entry door is used as emergency exit. All exits were equipped with functional alarms and were free on obstruction. The common areas were properly furnished with ample chairs/sofas for six (6) residents and staff. LPA observed a gaming area in the living room, along with a television set. All bedrooms in use by residents were properly furnished with appropriate beddings, chairs, linens and sufficient lighting. Bedroom #2 has a non-functional fireplace. All bedrooms were observed to be clean, sanitary, with ample supply of linens. Continued on 809-C Surrounding Grounds: The backyard was large enough for activities and included appropriate furniture with ample seating for six (6) residents. The outdoor area was free of hazards and obstructions; no body of water was observed. The side exit door was observed to be unlocked from inside. The detached garage is currently used as office space. LPA observed all resident and staff files locked in a cabinet in the office area. Laundry room is adjacent to the garage/office and was kept locked. Resident Files: LPA conducted a file review of resident records to ensure compliance of licensing forms. Staff Files: LPA conducted a file review of staff records to ensure forms and training certificates are up to date and compliant with licensing forms. Medications: Medication and medication records were reviewed for proper documentation. LPA counted the medications and ensured proper dosage were given to the resident. Licensing Program Analyst (LPA) Abeye Duguma met with Christine Lekhlyan for a Required One (01) Year visit. LPA explained the reason for the visit. A tour of the physical plant was conducted at 9:00 AM and the following was noted: There is one entrance being utilized at the facility. The facility has a total of six (06) bedrooms and two (02) bathrooms. The facility is fire cleared for six (06) non-ambulatory. The facility is currently occupying zero (00) residents. The facility has a covered shaded area for residents and visitors. The facility does not have a swimming pool/body of water. Laundry detergents, cleaning agents and other toxins are locked away. Food storage and preparation areas are clean and inaccessible to pests. Knives and sharps are observed to be locked and inaccessible to residents. The living and dining room are neat and clean. The facility maintains a comfortable temperature at 71°F. The smoke and carbon monoxide detectors are hardwired, interconnected and observed to be operational. Fire extinguisher is located near the kitchen, observed to be fully charged. The residents' rooms are adequately furnished with appropriate lighting system. Hallways are well lit. There is enough personal hygiene product provided by the licensee. (continued on LIC 809-C) The bathroom was checked for cleanliness and proper operations. The hot water temperature was measured at 110.8°F. There was enough clean linen available in the cabinets. No health and safety hazards noted during the visit. Exit interview conducted.
