Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Melissa Parks arrived on Monday April 13, 2026, to conduct the unannounced annual inspection. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed resident (5) and staff (2) files. All resident files contained the required paperwork. All staff files contained the required paperwork and training. LPA and Administrator Bianca toured the facility together to ensure the health and safety of residents in care. The following areas were inspected: resident bedrooms, bathroom, kitchen, living room, garage, and backyard. Facility had a fully stocked first aid kit. Water temperatures were within the required range. All required postings were observed. All chemicals and knives were kept locked and inaccessible to residents. Fire extinguisher had a current inspection tag. LPA obtained a copy of current liability insurance. Exit interview conducted. No deficiencies cited. Allegation investigated: Staff mismanaged residents' medications Staff did not treat residents with respect There is no sufficient staff at the facility to meet the needs of residents in care Administrator is not on the facility premises a sufficient number of hours
Licensing Program Analyst (LPA) Graham Gunby arrived unannounced on 12/23/2025 to complete and deliver findings to a complaint received on 05/30/2025. LPA met with Administrator Bianca Gheju and explained the purpose of the visit. Throughout the course of the investigation, the department conducted interviews and record reviews. Please continue to LIC9099C.. Unsubstantiated Allegation: Staff mismanaged residents' medications The department conducted record review, interviewing residents and staff to investigate this allegation. Two (2) residents’ interviews indicated that staff were giving them medications per their physician’s orders. Two (2) staff interviews reflected that staff were following resident’s physician’s orders and not mismanaging residents’ medications. Record review for R1s medications indicated that staff were administering R1s medications per their physician’s orders and there were no issues identified. Based on gathered information, this allegation was found to be Unsubstantiated. Allegation: Staff did not treat residents with respect Interview with R1 indicated that they feel that staff are "good" at the facility and care being provided was sufficient for R1's needs. Interview conducted with R1 indicated that they are treated well by facility staff and that their care needs are being met at the facility. Interviews with S1 and S2 indicated that the residents are treated well and receive sufficient care in relation to their needs. Allegation: There is no sufficient staff at the facility to meet the needs of residents in care Interviews indicate that there is sufficient staffing to meet the care needs of the residents. Staff are able to address resident needs in an efficient and timely manner. Observations indicate that staff are available to ensure that residents’ needs are addressed. S1 provided an updated LIC500 which showed substantial staff should be present. S1 stated that if staff call off, they are able to call in a substitute. Allegation: Administrator is not on the facility premises a sufficient number of hours During the investigation, LPA reviewed administrator qualifications, staff time sheets, and obtained copies of facility documentation. The current facility administrator Bianca Gheju does have a valid administrator certificate, and certificates were obtained prior to assuming the role as administrator. Facility documents show that the Administrator is at the facility every day and as needed. Documents also show there is sufficient staff at the facility to meet the residents’ needs. The administrator had a complete LIC308, designation of facility responsibility. Based on the investigation, observations and interviews conducted the allegation is UNSUBSTANTIATED. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is unsubstantiated. Allegation: Staff did not provide adequate meals to residents in care Based on two (2) staff interviews and two (2) resident interviews and department observation of the kitchen and meal service by the department found that there was an adequate amount of food for the residents. The food appeared to look appetizing and nutritious, sanitation in the kitchen appeared appropriate, residents said the food was good, and portions appeared plentiful. Food supplies in the facility were adequate by per requirement. Currently, there is no evidence to suggest that staff have failed to provide adequate food service. Allegation: Staff did not ensure a sufficient food supply was available at the facility for residents in care During the investigation, LPA conducted visits to the care home on 06/05/2025 and 10/08/2025 and observed the food supply at the facility. LPA observed a two (2) perishable and seven (7) day nonperishable food supply on cite for all visits. LPA observed food to be of good quality during all visits. Interview conducted with R1 indicated that they feel the food is "good" at the facility. Interviews with staff members, S1 and S2 indicated that the residents receive a sufficient amount of food of good quality at the home. Allegation investigated: Resident is being held against their will
LPA Parks arrived on Thursday September 11, 2025, to open a complaint investigation regarding the above allegation. LPA discussed the allegation with the Administrator, staff, and R1’s family. LPA obtained copies of R1's paperwork including medication list, LIC624, and admission agreement. LPA learned the following: R1 and their spouse previously lived at an Assisted Living Facility (ALF) in the bay area. In August, R1 and their spouse eloped to a hotel in the Sacramento area which later resulted in hospitalization. R1 was discharged to this facility while the spouse returned to the bay area ALF. R1 had a diagnosis of Dementia. According to staff, R1 did not have a history of exit seeking nor asked to leave the facility. R1 became upset on Friday September 5, 2025, saying they were held against their will and wanted to leave but had nowhere to go. R1 then became combative with staff. Staff were outside of the facility with R1 when a neighbor called 911. Facility, R1 and neighbors individually called 911. EMTs and police arrived, and R1 was transported to the hospital. As of today, R1 remains hospitalized. Interview with R1's Unfounded family revealed that R1 willingly moved into this facility on August 29, 2025. Based on the evidence provided, the preponderance of evidence standards was not met, therefore, the above allegation is found to be UNFOUNDED. An unfounded allegation means that the allegation was false, could not have happened and/or is without a reasonable basis. Exit interview conducted. Allegation investigated: Staff do not ensure the facility is comfortable for residents
Licensing Program Analyst (LPA) Graham Gunby arrived unannounced on 03/25/2025 to complete and deliver findings to a complaint received on 10/24/2024. LPA met with Administrator, Bianca Gheju and explained the purpose of the visit. Throughout the course of the investigation, the department conducted interviews and record reviews. Please continue to LIC9099C.. Substantiated Allegation 2: Staff do not ensure the facility is comfortable for residents. Based on interviews and observations the department found the facility is not providing the residents a comfortable environment to reside. The licensee resides at the facility as their permanent residence. The Licensee has (2) two young children (4 years old and 5 months old) that reside at the facility as well. Interviews indicated (2) residents have expressed discomfort from the children enter their bedrooms and taking their belongings. On 02/12/2025 LPA Gunby conducted a case management visit and observed a 5-month-old child at the facility. During this visit the administrator needed to change a resident’s briefs, while also caring for the child. LPA observed admin left the child to cry on the bed, while they assisted the resident. During a visit on 03/06/2025 LPA did not observe a child at the facility. As a result of this investigation, LPA finds allegations to be Substantiated - A finding that the complaint is Substantiated means that the allegation is valid because the preponderance of the evidence standard has been met. Allegation 1: Administrator is not present at facility a sufficient amount of time During the investigation, LPA reviewed administrator qualification, staff time sheets, and obtained copies of facility documentation. The current facility administrator Bianca Gheju does have a valid administrator certificate, and certificates were obtained prior to assuming the role as administrator. Facility documents show that the Administrator is at the facility every day and as needed. Documents also show there is sufficient staff at the facility to meet the resident’s needs. Before the complaint was made, the administrator was on medical leave. The administrator had a complete LIC308, designation of facility responsibility. Based on staff interviews and the information received, LPA finds the above allegations to be Unfounded meaning that the allegation is false, could not have happened, and/or are without a reasonable basis. Allegation 3: Staff are not qualified to administer medications. Based on staff and resident interviews along with record review, the department determined staff was qualified to administer medications. LPA obtained physician report, medication records, admission agreement, service plan, and training records. LPA obtained doctors notes stating R1 can request the PRN every 6 hours. Staff have completed all medication training and are qualified to administer medications. Staff contained the required training requirements and facility obtained the required doctor’s orders; therefore, the above allegations are Unfounded. A finding of unfounded means that the allegation is false, could not have happened and/or is without a reasonable basis. Allegation 4: Staff are not providing residents with adequate food service. Based on staff and resident interviews along with observations of the kitchen and meal service, the department found that there was an adequate amount of food for the residents. The food appeared to look appetizing and nutritious, sanitation in the kitchen appeared appropriate, residents said food was good, and portions appeared plentiful. Food supplies in facility were adequate per requirement. Currently, there is no evidence to suggest that staff have failed to provide adequate food service. Therefore, the allegation is Unfounded. Licensing Program Analyst (LPA) Graham Gunby arrived on Tuesday March 25, 2025 to conduct the unannounced annual inspection. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed resident (5) and staff (3) files. All resident files contained the required paperwork. All staff files contained the required paperwork and training. LPA and Admin toured the facility together to ensure the health and safety of resident in care. The areas toured included bedrooms, bathrooms, kitchen, common areas, and back yard. All chemicals, toxins, and sharps were kept locked and inaccessible to residents. Facility has one (1) fire extinguishers. In the areas toured, there were no health or safety violations observed. No deficiencies cited. Exit interview conducted. Licensing Program Analyst (LPA) Graham Gunby conducted an unannounced case management visit to the facility on 02/12/2025 for an incident that occurred on 02/09/2025. LPA met with Administrator Bianca Gheju and explained the purpose of the visit. During today's visit, LPA reviewed R1's file, received copies of the LIC 602, case notes and admission agreement. LPA spoke with the admin about staff providing supervision to R1. Administrator will submit the documents to LPA via email. R1 was referred to home health and will continue to receive care through the facility. Admin will continue to monitor R1 for any changes in condition. No deficiencies cited on today's date. Type of visit: Prelicensing
Licensing Program Analyst (LPA) Melissa Parks arrived on Monday April 22, 2024 to conduct an unannounced prelicensing visit. This is a change of ownership with residents in care. The Compliance and Regulatory Enforcement Tool was used during today's inspection. LPA reviewed 5 resident files and 2 staff files. All files contained the required paperwork. This facility has a fire clearance for 6 nonambulatory residents, with a total capacity of 6. There is a staff bedroom by the kitchen. Facility has all required postings in the entry way. LPA toured the facility with Administrator Bianca. The following areas were inspected for compliance: garage, kitchen, resident rooms, bathroom, backyard, and common areas. Facility has current fire extinguisher, carbon monoxide detector, and fully stocked first aid kit. Medications are kept locked in a cabinet in the hallway. Cleaning chemicals and knives/sharps are kept locked and inaccessible to residents. Component III has been completed at this time. The facility appears to be in substantial compliance and ready for licensure. The license will be granted upon completion of a final review and approval from the Licensing Program Manager and the Central Applications Bureau.
