Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Staff #1 (S1) at 1pm. During today’s visit, LPA spoke with the Administrator, Shirley Hills, via phone. Administrator gave permission for staff to sign the report. The facility is a five bedroom, two bathroom single story residence with an approved fire clearance of six non-ambulatory residents; of which all may be bedridden. The facility is approved with a hospice waiver for six residents. The facility currently has a census of five residents in care. During today's visit LPA toured the facility and tested the smoke and carbon monoxide detectors. All detectors were operational. LPA observed the two fire extinguishers were charged and inspected on December 8, 2025. The facility conducted a fire drill on September 10, 2025. LPA observed the First Aid Kit had all the required elements and the facility had a First Aid book on hand. LPA observed medication storage and reviewed the centrally stored medications. Per review medications are being given as prescribed. While touring the kitchen, LPA observed two-days of perishable items in the refrigerator and seven-days of non-perishable items in the freezers and pantry. Emergency supplies and water were observed in the garage. All appliances, including washer and dryer, were in working order. Chemicals were secured and locked in the laundry room. Sharps and knives were secured in the medication storage. (Continued on LIC 809-C) (Continued from LIC 809) LPA tested the hot water temperature in one of one client bathroom since the second bathroom is only used by staff and guests. The hot water temperature measured 111.0 degrees Fahrenheit. LPA toured the exterior of the property and there were no hazards or obstructions in pathways. There is ample yard space for activities and the facility recently held a Christmas party event in the backyard. A covered seating area was observed for residents to enjoy. LPA observed the Theft and Loss policies, emergency information and the PUB 475 See Something, Say Something Poster. LPA reviewed two of two staff training and fingerprint records and conducted a complete review of resident records. Bed rail orders were on file. LPA interviewed alert residents regarding their quality of care and spoke to staff present regarding care provided. LPA confirmed that administrator has a current administrator certificate which expires on February 14, 2027. LPA spoke with Administrator regarding licensee fees that were due and confirmed the licensee mailing address on file. Based on the observations made during today’s visit, the facility appears to be in compliance with Title 22 Division 6 of the California Code of Regulations, no deficiencies cited on this date. Licensing Program Analyst (LPA) Rose Ruppert made an announced visit to the facility for the purpose of conducting a pre-licensing inspection. LPA met with Administrator (AD) Shirley Hills. An application for a Change of Ownership (CHOW) for a Residential Care Facility for the Elderly (RCFE) was received by our agency on August 21, 2024 for a total capacity of six; six non-ambulatory, six bedridden and approved for six residents on hospice. The facility is a one story home with four resident bedrooms, one caregiver bedroom, two bathrooms, a living room, a kitchen, a dining area, a large covered outdoor patio, and a courtyard in the center of the home. The hot water temperatures measured 117.6 degrees Fahrenheit for both bathrooms. All exiting doors had alarm notifications. There is a backyard exit gate on the side of the house that is self-latching and unlocked and there were no obstacles observed in the large backyard. LPA observed the See Something, Say Something poster (PUB 475), Personal Rights, Visiting Hours and Emergency Disaster Plan in the facility mounted on the wall in the entry area.. Resident bedrooms had the required furnishings and all beds had linens and blankets. All toxic chemicals, cleaning solutions, and disinfectants are inaccessible to residents and sharps are in a locked drawer. Medications are also stored in a locked cabinet in the kitchen. The First Aid kit has all the required elements and includes a First Aid manual book. There are two fire extinguishers that are charged and were serviced on December 10, 2024. LPA confirmed the administrator has a current certificate that expires February 14, 2025. (Continued on LIC 809-C) (Continued from LIC 809) Residents were observed in the living room watching television, one was working on a crossword puzzle, one was watching a walk through Japan series and another was resting in her bed. LPA observed the facility had more than the required two days perishable and seven day non-perishable food items. Smoke detectors and carbon monoxide detectors tested operational. This facility has a pendant call system for residents. LPA conducted the Component Three Orientation with AD. AD was notified that the final application approval will be issued by the Centralized Applications Bureau in Sacramento. Facility Type: RCFE Application Type: CHOW Capacity: 6 Method: Telephone call with CAB COMP II Participants: Hills, Shirley & Bermudez, Arlene On 12/11/24, the applicant/administrator participated in COMP II at CAB via telephone call with analyst at CAB. Identification of the applicant and administrator was verified by confirming driver’s license number. During COMP II, applicant and administrator confirmed the understanding of Title 22. Component II was successfully completed. Applicant and administrator were advised to email/fax signed LIC 809 with copy of photo ID to CAB.
