Allegation investigated: Resident eloped from facility due to insufficient door alarms

Licensing Program Analyst (LPA) Melissa Parks arrived on Tuesday August 18, 2026, unannounced to conduct a complaint visit regarding the above allegation. LPA met with Administrator Carmen and explained the purpose of the visit. LPA interviewed the Administrator regarding the allegation. LPA learned the following: On Monday, August 10, 2026, staff were doing morning rounds at approximately 6:30am and could not locate R1. Staff previously observed R1 sleeping at approximately 2:30am. Staff then called the Administrator who arrived at the facility. The administrator called R1's POA to assist with locating them. Police were notified and assisted with the search. R1 was located sitting on the porch at a neighboring house at approximately 9:20am. R1 was then escorted back to the facility. Per R1's physicians report, Resident (R1) is not able to leave the facility unassisted due to a diagnosis of dementia. continued on 9099-C Substantiated Based on the information detailed above, LPA finds the allegation to be substantiated. Plan of correction: Licensee now uses a whole house alarm to alert staff when doors are opened. Additionally, a floor alarm was placed in R1's hallway to alert staff if they are exiting their hallway. This citation is cleared at the time of the complaint visit.

Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Melissa Parks arrived on Thursday November 13, 2025 to conduct the unannounced annual inspection. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA Parks reviewed resident (5) and staff files (4). All resident files contained the required paperwork. Staff files contained the required paperwork and training. LPA Parks and Administrator Carmen toured the facility together to ensure the health and safety of residents in care. The areas toured included resident rooms, bathrooms, living room, kitchen, garage, storage sheds, and backyard. In the areas toured, there were no health or safety violations observed. Facility was clean and well organized. All required posting were observed. All knives/sharps were kept locked and inaccessible to residents. First Aid kit was fully stocked. Fire extinguisher was observed with a current inspection tag and facility was current on fire drills. Water temperatures were within the required range. LPA scanned copies of the facility's liability insurance, LIC500 and LIC308. No deficiencies cited. Exit interview conducted. Type of visit: Prelicensing

Licensing Program Analysts (LPA) Graham Gunby and Cheyenne Ratajczak arrived on Thursday November 14, 2024 to conduct an announced prelicensing visit. The Compliance and Regulatory Enforcement Tool was used during today's inspection. All files contained the required paperwork. This facility has a fire clearance for 6 (six) nonambulatory, of which 6 (six) can be on hospice. Facility has all required postings in the entry. LPAs toured the facility with the Administrator, Carmen Ion. The following areas were inspected for compliance: kitchen, backyard, resident bedrooms, bathrooms, and common areas. Pool is fenced in and inaccessible to the residents. Facility has current fire extinguishers and a full first aid kit. Medications will be kept locked in a cabinet in the kitchen. Cleaning chemicals and knives/sharps are kept locked and inaccessible to residents. Component III has been completed at this time. The facility appears to be in substantial compliance and ready for licensure. The license will be granted upon completion of a final review and approval from the Licensing Program Manager and the Central Applications Bureau. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22.
