Allegation investigated: Licensee didn't report relationship to responsible party
Licensing Program Analyst (LPA) Hiratsuka, conducted the investigation into the allegation above. The allegation had to do with an alleged inappropriate relationship between a staff and a resident which LPA went unfounded on the allegation. LPA interviewed staff and residents. The Administrator made a comment to someone that appeared they might have thought something but didn't look into it. LPA spoke to Administrator who stated they deny the relationship occurred. LPA cannot prove or disprove the administrator didn't know about the alleged allegation to report it. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violations occurred, and the findings are UNSUBSTANTIATED. Allegation investigated: Staff are not ensuring the facility is mold free Staff did not ensure the roof was fixed timely
On 5/1/2024 LPA Tryon arrived at the facility to complete the complaint. LPA met with Licensee/ Administrator B. Scott Terry. LPA has spoken with staff and inspected the facility. Regarding the allegation that the staff are not ensuring the facility is mold free, LPA inspected the house and found no mold anywhere in the building. There had been some mold around a bathroom window, but it has been cleaned up and resident encouraged to open the window while the shower is going to vent the steam. So, there was some mold, but it was cleaned up right away when licensee became aware. Allegation is UNFOUNDED. Regarding the allegation that staff did not fix the roof in timely: LPA interviewed licensee/administrator and inspected building inside. It was learned that there never WAS a roof leak; the leak that happened in one bathroom was caused by a broken shower pipe in the wall.
