Allegation investigated: Resident fell sustaining injuries due to staff neglect. Staff did not seek medical attention for resident. Staff are overmedicating resident. Staff are not meeting resident's needs.

Licensing Program Analyst (LPA) Sandra Urena conducted an unannounced investigation visit for the allegations listed above. The LPA arrived at the facility and was greeted by staff, and the LPA informed them of the visit. The staff contacted the Licensee Dylan Dylan Hull on the phone to inform them of the visit. Kim Anderson Licensed Vocational Nurse (LVN) arrived and the LPA explained the reason for the visit. On 07/24/2023, Licensing Program Analyst (LPA) Sandra Urena conducted an unannounced initial investigation visit for the allegations listed above. The LPA arrived at the facility and met with Administrator Dylan Hull and Kim Anderson and explained the reason for the visit. At 11:30 a.m., the LPA interviewed the administrator, and staff and at 12:00 p.m., conducted record review pertinent to the investigation. This is an amended report. On 10/22/2024, Licensing Program Analyst (LPA) Kelly Dulek met with Facility Designee Kim Anderson with the purpose of issuing final findings on the allegations above. Administrator/Licensee Dylan Hull arrived at 10:05AM. Entrance interview conducted. Licensing Program Analyst (LPA) Esther Cortez (Cortez) conducted an unannounced initial complaint visit to the facility above on 08/16/2023 from 09:40am to 03:45pm. LPA Cortez met with Kim Anderson, Facility Designee. LPA Cortez interviewed staff and residents. LPA Cortez toured the inside and outside of the facility. LPA Erika Miller conducted additional interviews on 10/15/2024. Licensing Program Analyst (LPA) Ashley Smith arrived unannounced for an initial 10-day visit. The LPA met with House Manager Tracy Varnell and Administrator Dylan Hull and explained the reason for the visit. During today’s visit, the LPA interviewed staff at 9:50 a.m., 10:10 a.m., 10:35 a.m., and 10:40 a.m.; toured the kitchen area at 9:55 a.m., and obtained documents. Regarding the allegation: Facility did not follow resident's admission agreement. It was alleged that the licensee failed to follow the admission’s agreement regarding Resident #1 (R1). A review of documentation indicated that documents were signed on 7/11/2022. Per the admission’s agreement, there is a non-refundable pre-admission’s fee of $7500. However, it was decided that R1’s responsible party would pay a pre-admission fee of $3750, at a 50% discounted rate. Licensing Program Analysts (LPAs) Ashley Smith and Elsie Campos arrived unannounced for an initial 10-day complaint visit. The LPAs met with Administrator Dylan Hull and explained the reason for the visit. During today’s visit, the LPAs conducted a physical plant tour at 9:00 a.m. and spoke with the Administrator. Regarding the complaint, it was alleged that the facility did not have the Ombudsman poster in the facility. During the physical plant tour, whereas it was observed that the number to the Long Term Care Ombudsman (LTCO) was posted in the entryway, the facility did not have the required Ombudsman poster in the entryway. During today’s visit, the Administrator instructed staff to obtain the poster from the LTCO Ventura Office. Based on the investigation, this allegation is deemed Substantiated at this time.
