On 01/15/2026, Licensing Program Analyst (LPA) M Vega arrived unannounced at the above facility to conduct an Annual Inspection. LPA introduce self to Staff 1 (S1), stated the purpose of the visit, and was granted entry to the facility, Administrator was called over the phone and arrived few minutes later. LPA toured facility with Administrator (AD) Hasmik Jasmine Oganyan. The facility was observed to be at a comfortable temperature, of 74 degrees F. Facility is free of debris, in good repair, and no passageway obstructions or fire hazards were observed. Common areas were properly furnished and well-lit throughout. LPA observed some residents in common area prior to lunch watching television, others in their rooms resting. Department phone number and infection prevention information signs were posted thought the facility. Inspecting kitchen LPA observed the required 7 day supply of non-perishable food and 2 day supply of fresh perishables to be properly stored. An emergency disaster supply was observed. LPA observed rooms for residents were properly furnished, Clean and in good repair. All residents’ bedrooms were observed to be with comfortable temperature. Water temperature in Bathroom #5 105 degrees F. On the exterior of the property there were no obstructions. There was shaded seating for residents and in good repair. Fire exit properly maintained with self-latching gate. Medications observed to be locked in Kitchen area. LPA reviewed medication records to be accurate at the time of inspection. Cleaning supplies were observed to be in a locked cabinet in the laundry room. Fire extinguisher was observed with a service date of 06/05/2025. LPA reviewed Staff and Resident files. Resident files observed to have updated information. Facility is free of debris, in good repair, and no passageway obstructions or fire hazards were observed. Common areas were properly furnished and well-lit throughout. LPA observed some residents in common area prior to lunch watching television, others in their rooms resting. Type of visit: Prelicensing

On 01/26/24 at 1400 hours, Licensing Program Analyst (LPA) V Gorban conducted an announced Pre-licensing visit. LPA met with Licensee, Hasmik Oganyan, certification number 6026996740, expiration date 06/23/25 and discussed the purpose of the visit. LPA began the tour at the entrance of the facility that has one entrance point. LPA toured the inside and outside of the facility. LPA observed no obstruction to emergency exit from back yard of the facility on east side of the property. The facility was observed at a comfortable temperature of 78 degrees Fahrenheit, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. Common areas furnished and well-lit throughout. LPA observed the kitchen to be absent of any trash or debris, sharp objects are secured and inaccessible to residents. At this time, seven-day supply of non-perishable food were observed. Medications and chemicals were kept locked in separate cabinets. Resident’s all four individual bedrooms were observed to be furnished with bed, dresser, night stand, and overhead lightning. Mattresses, box springs, sheets, and linens, were absent of any tears and stains. Bathrooms and showers were equipped with non-skid mats and securely fastened grab bars. The bathroom’s water temperature was tested at 111 degrees Fahrenheit. Towels, linens, and personal hygiene supplies were observed in storage. There are no bodies of water outside. Report continues on LIC 809-C All Fire extinguishers are current with service date of 10/10/2023. Carbon monoxide and smoke detectors were observed to be operational. First Aid Kit was checked and observed to have the required supplies. Emergency exit plan, required phone numbers, and required postings were observed. A working facility telephone number (559-797-4202) was present and functional. Component III was reviewed with Licensee and Administrator. No deficiencies were observed during this visit. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22.
