Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced visit to deliver the complaint investigation findings, met with Administrator (ADM) Debbie Aguilar and stated the purpose of the visit. On 4/15/2024 The department received a complaint alleging that the resident sustained severe burns while in care due to lack of care from staff. On 4/17/2024 - LPA Simi Rai conducted the initial investigation.
