On 12/05/25, Licensing Program Analyst (LPA) Kesha Lewis made an unannounced visit to this facility to deliver findings about the above allegations. LPA identified herself upon arrival, stated the purpose of the visit. Based on interviews with staff and residents and LPA'S observation of the facility. LPA could not find evidence that the above allegation happened. Therefore the allegations are UNSUBSTANTIATED. A finding that the complaint allegation is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. Allegation investigated: Staff did not clean resident’s room.
LPA Kesha Lewis made an unannounced visit to this facility today to open a complaint investigation for the allegation listed above. LPA met with facility staff and explained the purpose of today’s visit. Allegations: Staff did not clean resident’s room. After reviewing R1'S room, speaking with RP and interviews with residents and staff regarding the allegation LPA Lewis finds the complaint to be unsubstantiated. LPA toured R1'S room and there was not noticable smeels, room was vacumed and apeared clean. Therefore, this complaint is UNSUBSTANTIATED. A finding that a complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
