Licensing Program Analyst (LPA) Kesha Lewis arrived at the facility unannounced to open a complaint for the above allegation. LPA was greeted by adminstrator and explained the reason for the visit. LPA Lewis interviewed S1-S2 and R1-R2 during this visit also the facility made copies of R1'S service plan. physicans report,SIR and care notes. Based on interviews with staff and residents it is found that the allegation that Staff did not treat resident with respect is UNSUBSTANTIATED. Allthough there was an inside that occored at the facility there is no substainating evidence that R1 was not treated with respect based on interviews with everone involved. Note that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, and therefore the allegations are unsubstantiated. On 8/13/2024 at 12:31pm, Licensing Program Analyst (LPA) Michael Bilger arrived unannounced to deliver and discuss findings for the allegations noted above. LPA met with Administrator Rebecca Cobb and explained the purpose of the visit. During this investigation, LPA conducted interviews with five staff members and three residents in care. Additionally, LPA conducted record reviews of medication log sheets, staffing schedule, and housekeeping schedule. LPA also conducted a facility observation on 07/10/2024 and 08/13/2024. . Allegation: Staff mismanaged residents’ medication. LPA conducted interviews, observations, and record reviews as noted above. Based on evidence reviewed, it was determined that facility is currently maintaining medication storage adequately including the use of locking mechanisms and inaccessibility to residents in care. Licensing Program Analyst (LPA) Avelina Martinez arrived at the facility unannounced on 05/23/2024 at 12:50 PM to deliver complaint findings, LPA Martinez met with Rebecca Cobb and explained the purpose of the visit. Throughout the course of the investigation, LPA Martinez conducted interviews and reviewed facility files. The investigation revealed resident 1 (R1) was not administered Insulin per medication orders. R1 was not administered insulin on April 11 thru 14, 2024. On April 15, 2024, R1 was sent to the Emergency Room (ER), which R1 was admitted into the hospital. R1 did not return to River Fountains of Lodi. As a result of this investigation, the Department finds this allegation to be Substantiated. A finding that the complaint is substantiated means that the allegation is valid because the preponderance of the evidence standard has been met. Licensing Program Analyst (LPA) Renee Campbell conducted an unannounced complaint visit at River Fountains of Lodi on 09/20/23 at 8:00 AM to present findings regarding the complaint allegations mentioned above. In regards to “Staff are verbally and physically abusing residents”, LPA Renee Campbell Interviewed clients R1 to R8. Of the eight residents interviewed, eight of them reported that they had not been showered in cold water by staff. LPA Campbell was also provided with a shower schedule that shows that residents select their preferred shower times. None of the residents interviewed, reported verbal abuse or heard that other residents were abused. In regards to “Staff does not ensure resident medications are taken once dispensed”, of the eight clients interviewed, seven reported that staff watched them taking medication or remained in the room. All residents reported they took their medication regardless.
