During a Regional Office Meeting, Licensing Program Manager (LPM) Robyn ClarkLicensing Program Analyst (LPA) Sabel Martinez delivered a revised Substantiated Complaint report. The LPA discussed this with Administrator Gaurav Rathi. Community Care Licensing (CCL) has investigated the above allegation. The investigation consisted of records review, interviews with facility staff, residents and outside sources which included medical professionals and family members. It was reported to CCL that facility staff restrain residents to their wheelchairs. It was specifically reported that a witness observed a staff member place a belt around a Resident (R1) and their wheelchair which restricted their ability to move.
