Allegation investigated: Staff are not providing timely care during the night time hours.
LPA Calzada arrived unannounced to deliver complaint findings for a complaint received on April 29, 2025 and met with Administrator, Jina Kim, arrived shortly. Also present was Julie Smith, staff. During the investigation, LPA reviewed multiple documents related to resident (R1), including but not limited to, Pre-Appraisal assessment, physician’s report, care plans (2), various email communications, eviction notice, and other documentation. Additionally, multiple staff and (2) family members of (R1) were interviewed. LPA also interviewed (4) family members for other residents. The results of the investigation are as follows: Resident (R1) moved to the facility on April 6, 2025 after being assessed three weeks prior, in March 2025. The facility provided a copy of a Resident Appraisal (dated April 7, 2025) noting (R1) has a diagnosis of Amyotrophic Lateral Sclerosis (ALS), is not able to move legs much or bear weight, is out of bed all day but mostly in the wheelchair, has weakness on arthe state’s words, verbatim · CDSS document, Jul 31, 2025 · control 59-AS-20250429120646
