On 8/8/2025 at 8:00 am Licensing Program Analyst (LPA) Noel Wolf Petersen arrived unannounced to conduct a complaint investigation into the above allegations, LPA met with Bonita Washington and administrator Makayla White by phone and explained the purpose of the visit. Physical plant inspection was conducted in connection with the allegations of the complaint, facility is clean; no bugs or foul odors were observed, no irregular abundance of trash or grime. The LPA conducted 6 interviews of the 6 residents in care, to the extent that they were able to respond, no neglect or fears were expressed. LPA interviewed 3 of 10 staff, none expressed a concern of the possibility other staff were inapporpriately loud or aggressive with the residents, or self reported thier own possible misconduct. This department has investigated the complaint alleging the above. Allegation investigated: Employees have not passed their criminal background checks. Residents incontinent care needs are not being met. Staff restrain residents.
On 10/11/23 Licensing Program Analyst (LPA) Kimberly Viarella made an unannounced visit to this facility to close the complaint investigation regarding the above allegations. The LPA identified herself, the purpose of the visit, and asked to speak to the Designated Facility Administrator. The Administrator was home sick and a staff member was identified as her Designee. The LPA met with the Designee and a brief interview followed to review the findings of this investigation. Based on observations, a records review, as well as interviews, the department has determined the above allegations were UNFOUNDED. Regarding the first allegation: Employees have not passed their criminal background checks. The LPA reviewed the staff roster to ensure that all members had received their background clearances. Two individuals were not listed on the LPA's Guardian roster.
