On 10/06/2023 at 2:30 PM, Licensing Program Analyst (LPA) Pang Lee arrived unannounced to this facility to conduct a complaint visit. LPA met with direct care staff, Eliesa Qiolele and explained the purpose of the visit. The purpose of this visit is to deliver complaint findings for the allegations above. The current census is 6 with two facility staff. A telephone call was made to administer to read the complaint findings since administrator was not present during today visit. Allegation: Facility allowed excluded person to facilitate operations It was alleged that the facility allowed an excluded person to facilitate operations. On 03/09/2023 at 8:30 AM, Licensing Program Analyst (LPA) Avelina Martinez arrived at the facility unannounced to deliver complaint findings. LPA Martinez met with Ratu Vunimatana during today’s visit and explained the purpose of today’s visit. Throughout the course of this investigation, the Department conducted interviews, reviewed facility files, and reviewed medical documents. Starting around July 2022, It was learned resident 1’s (R1) food intake was decreasing due to them refusing to eat. R1 was also refusing to take their prescribed medication during this time. On 03/09/2023 at 9:15 AM, Licensing Program Analyst (LPA) Avelina Martinez arrived at the facility unannounced to deliver complaint findings. LPA Martinez met with Ratu Vunimatana during today’s visit and explained the purpose of today’s visit. Throughout the course of this investigation, the Department conducted interviews, reviewed facility files, and reviewed medical documents. The investigation revealed resident 1 (R1) sustained an unwitnessed fall on November 02, 2023, and it was unknown how long R1 was on the floor. R1 was found on the ground and unresponsive in their room by facility staff. R1 was sent to the hospital on November 02, 2023, and while hospitalized R1 was diagnosed with a hematoma to left forehead with 2cm hemostatic laceration. On 11/30/2022 at 12:58 PM, Licensing Program Analysts (LPAs) Avelina Martinez and Peng Lee arrived at the facility unannounced to deliver a complaint finding. LPA Martinez and Lee met with Eliesa Qiolele and explained the purpose of today's visit. Throughout the investigation, LPAs conducted interviews and reviewed facility documents. It was learned resident 1 (R1) got into an altercation with resident 2 (R2) on November 28, 2022 and November 29, 2022. It was learned R2 was struck in the face by R1 on one occasion. On another ocassion R1 pushed R2, and R2 fell to the ground. R2 reported they did not request to go to the hospital after the incidents, and no injuries were reported. LPAs did not observe any injuries on R2 during the November 29, 2022 visit. Due to the above noted information, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, and therefore the allegation is unsubstantiated. On 08/04/2021 at 11:03 am, Licensing Program Analyst (LPA) Avelina Martinez arrived at the facility unannounced to deliver complaint findings. LPA met with Jerry Acheampong during today’s visit. Throughout the course of this investigation, LPA Martinez conducted interviews, reviewed facility records, and resident records. LPA Martinez interviewed resident 1 (R1), and R1 was not sent out to the hospital. It was learned R1 was slapped on the the head by resident 2 (R2). Furthermore, R1 reported no injuries from the slap on head. Morevoer during this incident R2 slapped resident 3 (R3) acrossed the face, however, there were no injuries on face. It was reported by staff 2 (S2) that R3 was not injured during the incident. R3 was not sent out the hospital. Faclity staff called the Local Police Department, and R2 was removed from the facility.
