LPA met with facility staff Aisake and explained the purpose of the visit. Aisake called the facility designated administrator to inform that CCLD was present in the facility. The purpose of this visit is to deliver complaint findings for the allegations above. The current census is 5. A brief interview with conducted with Aisake. Licensing Program Analysts (LPA) Kevin Gould made an unannounced inspection to Love and Comfort Elderly Care RCFE on 6/25/25 at 9:15pm to conclude the investigation of the above allegations and to deliver the findings. LPA Gould met with staff, Isaac Ratu and together discussed the investigation details. Based on the interviews conducted during the investigation process and statements obtained during the investigation process, LPA Gould was unable to corroborate the allegations. LPA conducted interviews with R1 and staff present and R1 confirmed being taken to medical appointments and seeing a physician regularly. LPA observed documentation of medical visits with a physician in May and June 2025. Additionally, LPA reviewed all R1's medications on two occasions and did not observe any medications classified as an NSAID. LPA observed the previous medication administered to R1 did not observe any medications classified as a NSAID. LPA attempted to review R1's medication administration records but the facility was unable to locate or did not retain those records for the former resident. LPA is unable to determine if the facility did in fact administer medications to former resident. Additionally, the former resident has moved out of the facility three months ago and LPA is unable to obtain any evidence to support the allegation. Licensing Program Analysts (LPAs) Kevin Gould and Cynthia Tamayo conducted an unannounced complaint inspection at Love and Comfort Elderly Care RCFE on 3/7/25 at 9:00am to inform the licensee of complaint allegations mentioned above. LPAs met with the licensee and together discussed the purpose of todays inspection. During this investigation LPA Gould interviewed S1 and R1 (See confidential name list LIC-811 dated 3/7/25) LPA Gould conducted file review for R1 and reviewed R1's medication administration records and centrally stored medications. Based on the interviews and documents reviewed during the investigation process, the allegations are substantiated. LPA observed medication bottle Sodium Bicarbonate with no current medications and no refills currently at the home. LPA observed this medications to have been given above the normal dose as LPA observed am and noon medications already signed off and administered to resident ahead of the time prescribed by R1's physician. LPA Gould met with staff, Delova Boswell and together discussed the investigation details. Based on the interviews conducted during the investigation process and statements obtained during the investigation process, LPA Gould was unable to corroborate the allegations. LPA attempted to contact the reporting party via email and phone with no response. LPA conducted interviews with two staff members who all denied the allegations above. LPA observed fresh food prepared for residents and a plentiful supply of fresh fruits and vegetables LPA also observed staff members cleaning with appropriate gloves and PPE. Licensing Program Analysts (LPA) Kevin Gould and department representative Shariff Osman made an unannounced inspection to Love and Comfort Elderly Care RCFE on 9/5/24 at 9:00am to conclude the investigation of the above allegation and to deliver the findings. LPA Gould met with staff, Isikeli Tuikentatuba and together discussed the investigation details. Based on the interviews conducted during the investigation process and statements obtained during the investigation process, LPA Gould was unable to corroborate the allegations. The alleged victim made contradictory statements to Long term care ombudsman, department investigators and law enforcement. LPA Gould attempted to conduct additional interview with alleged victim, however they had left the facility prior to LPAs arrival and the service coordinator states they have refused further service and their whereabouts are unknown at the time of writing. Licensing Program Analyst (LPA) Kevin Gould and department representative, Shariff Osman conducted an unannounced complaint inspection at Love and comfort Elderly Care RCFE on 9/5/24 at 9:00am to inform the licensee of complaint allegation mentioned above and to deliver findings. During this investigation LPA Gould interviewed R1, R2, R3 and S1 (See confidential name list LIC-811 dated 9/5/24). Based on the interviews and statements obtained during the investigation process, the allegations are substantiated. S1 and two residents interviewed corroborated the allegation that the facility kitchen, living room and back yard access are locked to residents overnight by placing a door from the garage in the doorframe and securing it baring residents from a significant portion of the home. R1 confirmed that they utilize a bedside commode in their room and have at times shared a room with another resident. Licensing Program Analyst (LPA) Kevin Gould conducted an unannounced complaint inspection at Love and Comfort Elderly Care RCFE on 8/1/24 at 1:30pm to inform the licensee of complaint allegation mentioned above and to deliver findings. Based on the interviews conducted during the investigation process and statements obtained during the investigation process, LPA Gould was unable to corroborate the allegations because all current residents and staff members interviewed denied the allegations. LPA conducted five resident interviews, all residents interviewed denied the allegation and denied witnessing any staff member threaten a resident. LPA conducted two interviews with staff members who both denied the allegation. LPA contacted the placement agency who denied the allegation and provided statements to LPA that former resident was an aggressor with staff members. Licensing Program Analysts (LPA) Kevin Gould made an unannounced inspection to the Love and Comfort Elderly Care RCFE on 4/11/24 at 9:00am to conclude the investigation of the above allegation and to deliver the findings. LPA Gould met with staff, Robert Barnet and together discussed the investigation details. Based on the interviews and statements obtained during the investigation process, the allegations are substantiated. LPA conducted interviews with resident and one staff member. see confidential name list LIC-811 dated 4/11/24) staff interviewed confirmed that resident was taken to the hospital by a driver from a transportation agency Kirn transportation. LPA discussed R1's 602 which indicated the resident is unable to access the community independently. all resident who cannot access the community independently are required to be supervised while outside the community to ensure safety of residents.
