Allegation investigated: Uncleared adults living in facility
Licensing Program Analyst (LPA) Magda Malcore met with Licensees, Antonia Jones and Douglas Jones at Community Care Licensing Division (CCLD) Regional Office to deliver the findings on the above allegation. LPA Malcore explained the purpose of the office visit. Regarding the allegation, uncleared adults living in the facility, interviews with the Licensee/Administrator Antonia Jones, staff, resident, and outside parties reveals not enough witnesses to corroborate the allegation. Additionally, LPA observed no physical evidence at the facility to corroborate uncleared adults living at the facility. Based on observations and interviews with relevant parties, the above allegation is Unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced complaint visit to the facility. LPA met with Licensee/Administrator, Antonia Jones and discussed the purpose for the visit. Regarding the allegation, facility has no administrator, Licensee Jones stated she is current Licensee and Administrator for the facility. Based on lack of evidence, LPA observations and interviews, the allegation is Unfounded. A finding that the complaint allegation is Unfounded means that the allegation was without a reasonable basis. An exit interview was conducted where this report LIC9099 was discussed and a copy provided to Licensee/Administrator Jones at the conclusion of the visit. Allegation investigated: The facility did not provide resident's refund per the admissions agreement.
Licensing Program Analyst (LPA) Mary Rico conducted an unannounced visit to investigate and deliver findings on the allegations listed above. LPA met with Administrator Catherine M Aleman explained the purpose of the visit. The investigation consisted of staff interview and document reviews. For the allegation, the facility did not provide resident's refund per the admissions agreement. During staff interviews, S1 informed LPA that the R1 passed away on 3/10/2024 and the facility was in the process of issuing R1 responsible party refund. During record review, R1 Admission Agreement indicated the responsible party is intitle for a refund.
