Allegation investigated: Facility mismanaged resident's medication Facility did not ensure that resident received medical care

Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to deliver the findings of the complaint investigation into the allegations listed above. LPA met with the Wellness Director Anabella Real and explained the reason for the visit. The investigation into the allegation, facility mismanaged resident's medication, revealed the following. It was reported that Resident 1(R1) did not receive Eliquis and Lasix for three days. A review of records show R1 moved into the facility on October 25, 2024 for respite care. R1's medication was handled by the facility. Staff reported that R1 provided medication from their last hospital visit at the time of move in. The Wellness Director reported that when R1 moved in, R1 did not have a doctor so they recommended a doctor for R1. R1 did not choose a doctor until October 31, 2024. R1 was self responsible and their physician report dated October 18, 2024 shows R1 can leave the facility unassisted. LPA met with Executive Director Bryan Hadley and Health and Wellness Director Nina Dadabhoy and explained the reason for the visit. The investigation into the allegation, facility is mismanaging resident's medication revealed the following. Resident 1 (R1) moved into the facility from a skilled nursing facility (SNF) on 4/21/23. It was reported that R1 had 7 medications which were transferred with R1 to the new facility. A review of records shows R1’s medication order summary dated 4/22/23 lists 12 medications, 4 of which are not active. The medication administration record (MAR) for April 2023 lists 7 medications with all prescriptions starting on 4/27/23. Staff reported that when R1 arrived at the facility they contacted the doctor and the pharmacy to refill R1’s prescriptions. LPA interviewed residents and staff in assisted living area and the memory care unit. LPA obtained and reviewed facility and resident records. The investigation into the allegation, unqualified staff administer injections to residents in care, revealed the following; The facility has residents that are diabetic. Licensing Program Analyst (LPA) Kimberly Lyman made an unannounced complaint visit to initiate an investigation into the above allegation. LPA was greeted and granted entry into the facility and explained the reason for the visit. Administrator Brian Hadley joined the visit in progress. During the course of the visit, LPA toured the facility, interviewed staff and witness as well as reviewed and obtained pertinent documentation such as shower schedule and shift report. Regarding the allegation that staff did not assist resident with their shower, the investigation revealed the following: Resident 1 (R1) had a preferred caregiver for showering. Caregiver switched shifts beginning May 2022 and is unable to provide shower due to R1's preferred shower time. R1 requests a 7 PM shower time and caregiver switched from evening shift to morning shift. Per interviews with staff, facility has attempted to work with resident in regards to shower time and caregiver providing the shower.
