Licensing Program Analyst (LPA) Kristin Kontilis conducted an unannounced Annual Required visit and inspection of the facility. At the time of arrival, there were five (5) residents in care, and one (1) staff as well as Administrator Viktoriia Andreichenko were on duty. Entrance interview conducted: LPA explained the purpose of the visit. The facility is a one-story Residential Care Facility for the Elderly (RCFE). The facility accepts residents with a dementia diagnosis; has hospice care waiver for four residents; and a fire clearance for six non-ambulatory residents. Currently there are no residents on hospice residing in the facility. A tour of the physical environment and accommodations were assessed, and the following was noted: LPA observed the required posting of the complaint poster and Resident’s Rights. LPA inspected the one-story facility for fire safety, personal accommodations, and food service. The facility maintains a comfortable room temperature. First aid kit was observed to be complete. The physical environment was checked for cleanliness and condition. Walls, windows, ceilings, doors, floors and floor coverings were checked. There are two fire extinguishers on the premises last serviced on 5/26/2026. There is a total of one (1) carbon monoxide detectors and five (5) smoke alarms throughout the facility all in good working order. Residents participate at-will in activities including exercising, walking around the facility grounds and the neighborhood, music activities, and dance performances. Please continue to 809-C, Pg 2. Snacks and beverages are available for residents in care upon request. LPA observed the kitchen cabinets, refrigerator, stove, and counters are clean. Cleaning agents and the toxic chemicals are kept in a locked garage. Medications are kept in a cabinet located in the Administrator’s office. Sharps are kept in a locked box under the kitchen sink.. The backyard has a deck with table and chairs, a gazebo, raised garden area and a ramp for access throughout the yard. The backyard is conducive for outdoor visitations. The front yard has garden areas and access as the main entrance into the facility. The recycling bin, green waste bin, and trash bins are standard bins with flip lids. The facility has three (3) shared bedrooms for a capacity of six residents. All of the bedrooms are furnished with lights and nightstand lamps to provide sufficient lighting. Bedroom #1 and 2 utilize a bathroom off the hallway. Bedroom #3 has a private bathroom utilized only by the residents who reside in Bedroom #3. Residents records and personnel records were reviewed and found to be in good order. All persons associated with the facility have a criminal background clearance. Administrator's certificate is current. LPA observed a sufficient amount of perishables for two days and non-perishables for seven days. Exit interview conducted. No deficiencies noted. Entrance interview conducted: LPA explained the purpose of the visit. The facility is a one-story Residential Care Facility for the Elderly (RCFE). The facility accepts residents with a dementia diagnosis; has hospice care waiver for four residents; and a fire clearance for six non-ambulatory residents. Currently there is one resident on hospice residing in the facility. Allegation investigated: Staff handled resident roughly.

Licensing Program Analyst (LPA) Kristin Kontilis conducted a subsequent complaint visit to deliver final findings for the above allegation. The initial visit was conducted on 5/29/2022 by LPA Kristin Kontilis, and LPA interviewed residents and staff from 10:30 am to 2:30 pm. During today’s visit, LPA met with Viktoriia Andreinchenko and explained the reason for the visit. On the allegation: Staff handled resident roughly. It was alleged that Resident 1 (R1) sustained bruising of unknown origin on their right side of chest and arm. It was described as a round bruise. R1 mainly speaks Spanish, but is alert enough to talk and have a conversation. A witness stated sometimes R1 is resistant to care. Staff stated R1 can be “feisty” and hit against staff while providing care. Staff observed the bruise on R1 and were initially unsure of it’s origin because R1 stated it was “from surgery.” Staff later thought the bruising might be from transferring R1, due to the location of the bruises on R1’s sides and arms. Staff Please continue to 9099-C, Pg 2 Unsubstantiated stated R1 does not assist them with transfers but rather pushes their weight against the staff, which may be the cause of the bruising. Staff and administrator demonstrated their transfer process for LPA. Staff and administrator have decided to add towels on the staff’s arms, to provide padding for staff that put their arms under R1’s arms for transfers. They are also implementing a new transfer, where they ask R1 to put their arms around the staff’s neck during the transfer. LPA noted R1 is on blood thinners, and interviews revealed R1 could bruise easily as a result. LPA attempted to interview R1, but R1 stated in Spanish “no preguntas” (no questions). R1 had a visitor on 5/18/2024 that did not observe any bruising. R1’s family member stated on 5/29/2024 that they had no concerns about R1’s care. Based on the information obtained, the allegation is deemed Unsubstantiated at this time. Exit interview conducted.
