On September 16, 2025, Licensing Program Analyst (LPA) Jenifer Tirre conducted an unannounced required visit using the CARE Inspection Tool. LPA was greeted by staff and granted entry after stating the purpose of the visit. Administrator Edwin Panganiban arrived shortly after and was available to assist with the facility inspection. The facility is licensed for six (6) non-ambulatory residents with approved hospice waiver for two (2) residents. Currently, there are two (2) Hospice residents present during today’s visit. This is a single story home with attached garage facility. The facility has six bedrooms (four private resident rooms, one shared resident room, and one staff room), two bathrooms, kitchen, dining area and common area living room. LPA conducted a tour of the physical plant accompanied by Administrator Edwin Panganiban , and the following was observed: There were no bodies of water on the premises. All rooms were inspected. Beds and bedding supplies were in operational condition, lighting was provided, and storage for the client's personal belongings was observed. Bed linens, blankets, and bath towels were available during the visit. Bathrooms were operational with water temperature measured at 114.0 degrees F. A comfortable temperature of 74 degrees F. was maintained in the facility. LPA observed the facility to be furnished at the time of the visit. Storage areas for cleaning supplies and sharps objects were stored and not accessible to residents. The kitchen was inspected, and sufficient two day perishable and seven day non-perishable food was maintained adequately. CONT ON 809C Facility has supply of emergency food and water. Facility has a fire extinguisher mounted and fully charged. A review of the Medication Records Administration (MAR) was conducted, and LPA observed the records are in compliance. During the visit, LPA observed the facility's infection control practices. LPA observed screening protocols for visitors, staff, and residents. LPA observed the facility has supply of Personal Protective Equipment (PPE). All mandated inspection control posters were posted. LPA observed First Aid Kit was maintained. A working landline phone was operational. The last fire drill was conducted on September 20, 2025. The facility had operational smoke and carbon monoxide detectors in bedrooms and common areas. The facility has current liability insurance on file effective 1/28/2025- 1/28/2026. A review of four residents (R1-R4) service files and four staff (S1-S4) personnel files revealed to be complete. The facility has the current administrator's certification on file for Edwin Panganiban #6070043740 - Expiration 07/27/2027. No deficiencies during this inspection visit. Type of visit: Case Management - Other

On this day Licensing Program Analyst (LPA) Jenifer Tirre made an unannounced visit to conduct annual inspection, during visit, LPA Tirre observed facility had concrete construction in back yard area being performed. Administrator/ Shareholder Edwin Panganiban and Shareholder Christopher Curtis informed LPA that back yard area is having cement put in today as base for a large storage unit that is being built in outside back yard area. LPA was provided a copy of Permit issued by City of Yorba Linda. Administrator Panganiban stated that protocols have been put in place for residents to be supervised during construction period. Shareholder Curtis informed that construction in back yard is projected to take another two weeks. LPA Tirre advised Administrator/ Shareholder Panganiban to provide weekly updates to Department on construction status. Department will make a follow up visit to confirm facility has completed project and that there is no safety or health concerns regarding care of residents. Type of visit: Case Management - Annual Continuation

On October 31, 2024, around 8:30am Licensing Program Analyst (LPA) Edward Kim arrived to complete the required 1-year annual visit that was started September 16, 2024. LPA Kim was greeted and granted entry by Caregiver (CG) Eliza Oriel. Administrator (AD) Christopher Curtis arrived to the facility at 8:50 am. AD Christopher Curtis could not stay for the entire visit and said CG Noel Oriel will assist with the visit and can sign the report on their behalf. During the visit, LPA conducted a full audit of all resident files, and all resident medications. The smoke detectors and carbon monoxide detectors were operable. LPA Kim conducted two resident interviews and one staff interview. Deficiencies were cited during this visit as per Title 22 Division 6 Chapter 8 of the California Code of Regulations. A technical violation was assessed during the visit. On October 31, 2024, at 11:01am Licensing Program Analyst (LPA) Edward Kim conducted an unannounced case management POC visit in conjunction with the annual continuation visit on October 31, 2024 visit. LPA was met and greeted by Caregiver (CG) Noel Oriel and explained the purpose for the visit. LPA verified that S1 and S2 have all their necessary and complete paperwork in their files. The deficiency (See LIC 809D dated 9/16/24) has been cleared. Facility representative provided with the POC letter documenting the correction. Type of visit: Required - 1 Year

On September 16, 2024, at 1:25pm, Licensing Program Analyst (LPA) Edward Kim conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Kim was greeted and granted entry by Caregiver Noel Oriel. Administrator Christopher Curtis arrived to the facility around 2:00pm. The facility is licensed to operate for six (6) nonambulatory residents and has a hospice waiver for two (2) residents. The facility is a single-story structure located in a residential neighborhood. It consists of the following: five (5) resident bedrooms, one (1) Staff Bedroom, two (2) bathrooms, living area, dining area, kitchen, and an attached two car garage. LPA Kim toured inside and outside of the physical plant with AD Curtis. There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident’s personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. The Resident’s rooms were inspected: Resident Room 1, Resident Room 2, Resident Room 3, Resident Room 4, Resident Room 5, and Staff Room. Bathrooms were found to be clean and operational. The water temperature measured at 136.9 degrees F bathroom 1 and 120.2 degrees F in bedroom 5 bathroom. LPA Kim observed the facility to be sanitary and appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to residents. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food available and maintained properly. Evaluation Report Continues on LIC 809-C During the visit, LPA Kim observed the facility's infection control practices, plan of operation, and screening protocols for visitors, staff, and residents. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). A working telephone (714-993-0449) remains available. Due to time constraint a continuation of this inspection will be conducted at a later date. LPA will conduct record review of resident, medication review, emergency drill log review, smoke detector and carbon monoxide detector check, staff interviews, and resident interviews . Deficiencies were cited during this visit as per Title 22 Division 6 Chapter 8 of the California Code of Regulations. A civil penalty was assessed during the visit.
