Allegation investigated: Physical Abuse

Licensing Program Analyst (LPA) Contreras arrived unannounced to deliver findings to the above allegation. LPA was greeted by Supervisor Maria Villa. Administrator(admin) Anirudh Wadhwa was unable to attend visit. Reporting Party (RP) alleges physical abuse. Complainant states that resident (R1) was being handled in an aggressive manner and staff hit resident. During investigation, LPA conducted interviews, reviewed records and made observations. LPA interviewed staff and residents, all residents interviewed did not observe or have witnessed any residents being handled aggressively or in an abusive manner. Five out of five witness interviews did not report any physical abuse witnessed or had concerns regarding physical abuse occurring in facility. All five out of five witnesses did not observe any abuse toward R1 or other residents in facility. Four out of four staff interviewed did not report witnessing any physical abuse or handling residents in an aggressive manner. Continued onto 9099C... Unsubstantiated continued from 9099... Allegation investigated: Facility is unkept and malodorous Resident's needs not being met

Licensing Program Analyst (LPA) Contreras arrived unannounced to deliver findings to the above allegations. LPA was greeted by Supervisor/Med Tech Maria Villa. Administrator Anirudh Wadhwa was unable to attend the visit. During the course of the investigation, the Department requested and reviewed documents, conducted interviews, and made observations. Reporting Party (RP) alleges facility is unkept and malodorous. LPA visitied facility on 6/08/2026 to initiate complaint and conducted a walk-through of the facility. LPA did not observe any malodors and facility appeared to be clean and in good conditions. On 6/09/2026 LPA conducted a Post-Licensing Inspection visit and conducted another walk through of all rooms in the facility. LPA did not observe any malodors or uncleaniness in premises. No deficiences were cited during the visit. LPA conducted a health and safety walk through today 7/24/2026 of entire facility premises and rooms and observed all bedsheets to be cleaned and free of odors. In addition, bathrooms accesible to residents were cleaned and were stocked with toilet paper, paper towels, and soap. continued onto 9099-C.... Unsubstantiated continued from 9099...... No additional reported concerns were discussed during resident interviews. Type of visit: Case Management - Legal/Non-compliance

Licensing Program Analysts (LPA) Ethel Contreras arrived unannounced and met with administrator (Admin) Anirudh Wadhwa and Compliance Manager Lia Miller for the purpose of conducting a Non-Compliance inspection placed from previous ownership of facility, which will continue further notice. · Administrator duties/qualifications · Insufficient care and supervision · Building and grounds · Staff training requirements · Facility's future compliance There were 23 residents at the time of inspection. LPA reviewed the above-mentioned compliance issues and found progress is being made. TSP arrived to prepare for pre-licensing visit in Feb 2026. New Administrator worked with the Technical Support Program (TSP) to bring the facility into compliance and prepare for new license. Admin mentioned they will not need TSP at the moment. · Administrator duties/qualifications CCL reviewed supporting documents submitted and found that Mr. Wadhwa has substantial experience working as a Licensee and Care Manager of a Residential Care Facility for the Elderly (RCFE) An exception was granted to Anirurdh Wadhwa to be acting administrator on 12/04/2025. Admin Cert exp. 7/24/2026. Admin currently working to renew admin cert. Continued to 809C...... continued from 809... Insufficient care and supervision A review of the LIC500 and staff schedule shows adequate coverage 24 hours a day, with 2-3 care staff per shift as well as a cook who is here 7 days a week and the Administrator who is scheduled from 10am to 2 pm five days a week, and supervisor who is on site Monday thru Friday the AM shift and on-call 7 days a week. There have been no complaints or incidents reported over the past three months due to insufficient care and supervision. In addition, facility has a compliance officer that comes once a week 11:00am-5:00pm . Staff training requirements A review of 3 personnel files found that three out of three employees are up to date with first aid/cpr training. Training documentation was found to be up to date for all three files reviewed using Advanced Healthcare training program and courses/hours documented. Facility's future compliance The new Administrator and staff have worked diligently to improve the facility. The flooring has been replaced throughout the building, the walls painted, lighting installed, closet doors replaced, bathrooms updated, new blinds in each resident's room. Linens were found to be within regulation and there was an adequate supply of hygiene products as well as soap. Paper towels appeared to be missing from one restroom. LPA had conversation to ensure that all toiletries are always kept stocked. Sun room is done being built and will be used as an activity room. Admin working to have air fresheners and floor cleaners with SYTAX including installing dispensers. Awaiting recliners and furniture for residents to be ready to use. Post licensing was also conducted today and no deficiencies were given. One Technical violation was given due to refrigerator/freezer observed to have crumbs/ food specks throughout. Licensing Program Analyst (LPA) Contreras arrived unannounced to conduct a post-licensing visit. LPA was greeted by Administrator(admin) Anirudh Wadhwa. Facility is an Adult Residential Facility for the Elderly with an approved capacity of of 30 residents, there are currently 23 residents in care. LPA and admin toured all buildings and grounds and were found to be clear from obstruction. Facility was found to be clean and at a comfortable temperature. All emergency outdoors exit that could pose an obstruction were clear. All required postings were visible. New license was not posted due to not having a copy, LPA provided copy of new license and admin posted it in a prominent place. There was a sufficient supply of perishable and nonperishable foods as required by Title 22 Regulations. LPA observed expiration dates noted on all food items. Emergency food and water observed. LPA had conversation with admin that freezers and refrigerators must be kept clean from food particles/specks throughout (Technical Violation given). All resident rooms were observed to be clean and have the proper linens. There was an appropriate supply of cleaning products, linens, hygiene products and paper products available for residents. Oxygen tanks are stored properly. Bathrooms were equipped with necessary grab bars. Paper towels were missing in one restroom. Hot water temperature for sinks found to be within Title 22 regulations of 105 to 120 degrees Fahrenheit. Facility's fire extinguishers were last inspected Sept.25,2025. Smoke and carbon monoxide detectors are hardwired through the fire department with a last inspection and testing on 9/04/2025. Continued onto 809C.... continued from 809.... LPA conducted a file review check for 4 residents. All documentation found to be in file including Admission Agreements and Medical Assessments up to date. LPA conducted staff file review for 3 staff. All documentation and training up to date. LPA and admin conducted medication spot check and found Centrally Stored Medication log up to date and medication locked and secure. No deficiencies cited during todays visit. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Contreras arrived unannounced to conduct a pre-licensing inspection and was greeted by applicant/ current administrator(admin) Anirudh Wadhwa. Facility is an Adult Residential Facility for the elderly in process for a Change of Ownership. Facility currently has 21 residents in care. LPA and admin toured the indoor and outdoor portions of the facility. Facility was found to be clean and at a comfortable temperature. All required postings found to be visible. Activities calendar visible. All resident bedrooms were found to be in clean and good repair. All had required chair, chest drawer and beds were made with appropriate linens. One resident bedroom lacked a window screen. Restrooms had grab bars and bath mats. Sun room is currently under construction for resident activities. LPA advised admin to have an outdoor area available for residents. Admin cleared a second outdoor area to be free from hazards for residents during inspection visit. LPA observed there to not be emergency food and emergency water supply. Admin ordered emergency food supply during inspection and brought water supply to meet the 72 hour regulation. LPA observed there to be perishables to meet Title 22 Regulations. There were canned goods and non-perishable foods in pantry. Refrigerator and freezer were functioning and operational. LPA observed foods in freezer to not be labeled or have expiration dates. LPA had conversation with admin that all food in freezer and refrigerator must be labeled with expiration dated noted. LPA observed there to be plates and utensils including additional kitchenware. All cleaning products, sharp items, other toxins and chemicals are locked and inaccessible for residents. There are an ample amount of extra linens, towels, hygiene products. LPA observed fire extinguisher charged and last inspected on 9/25/2025. Fire alarm and carbon monoxide alarms are inspected through Fire Marshall with a last inspection conducted on 9/4/2025. Signal system on all doors leading out of facility were on, tested and found operational. First aid kit available. Continued onto 809C.... continued from 809.... Water was measured in faucets used by residents and was not found to be within regulation between 105 & 120 degrees F. Admin lowered water heater, LPA conducted a second water measurement which was found to be in regulation in all 3 bathrooms accessible to residents. LPA discussed with admin keeping a temperature log check to ensure water is not over 120 degrees in all faucets. LPA conducted a medication spot check and observed that all medication is centrally stored and locked in newly constructed medication room. Centrally Stored Medication Log (CSML) is up to date. LPA conducted a file review for residents. All documentation including Medical Assessment found to be up to date and in file. LPA conducted a staff file review. All documentation found to be up to date. CPR and training up to date. Administrator Certificate: Anirudh Wadhwa #7016455740 Expires:7/24/2026 COMP III has already been discussed with applicant/current administrator in previous visit conducted on 2/05/2026. Items Addressed and Discussed during Inspection: - Expiration dates must be kept on all food items - One window screen missing in resident room (Will send picture of new window screen) - Water temperature must be regularly checked to be at 105-120 degrees F (Log will be made for temperature checks) - Outdoor area must be available for residents (Outdoor cleared during visit) - Emergency food and water needs to be in facility (Ordered during visit w/ proof of receipt) Pre-Licensing completed. Facility is ready to be Licensed. LPA will submit Pre-Licensing Application Report to the Application Unit Analyst in Sacramento. Application Unit Analyst will notify Applicants of Status.
