Type of visit: Required - 1 Year

On June 23, 2026 at 10:00 AM Licensing Program Analyst (LPA) Toni Nwala arrived unannounced to conduct an annual inspection. Upon arrival LPA was greeted by facility staff and granted entry. LPA began inspection with introduction, visit purpose and provided the facility Administrator with LPA identification and business card. Facility Overview: The facility is a one-story home with (6) bedrooms and four(4) bathrooms, including an attached garage. There is a fenced swimming pool in compliance with building code. There are no known firearms on the premises. Infection Control: LPA observed that hygiene and cleaning supplies were available for regular facility maintenance. The facility’s infection control plan was reviewed and found to meet department requirements. Resident record review began- Two (2) records were reviewed. LPA reviewed for admission agreement, medical assessment and TB test results, consent forms, identification and emergency information, appraisal needs and service plans, centrally stored medication/destruction records, safeguard for personal property/valuables, and personal rights notification. This facility is meeting documentation requirements. Continue on 809-C Physical Plant and Safety of Environment/Operational Requirements- LPA toured the facility inside and outside. The home is maintained at a comfortable temperature for the clients. Lighting is sufficient for safety and comfort. Water temperature measured 115.2 degrees F. Laundry facilities and a locked cabinet is present for storing laundry soap and other chemicals in the facility. All outdoor and indoor passageways are free of obstruction. A locked area is provided for medications and sharp objects. LPA verified there is a telephone working at this location. Food Service- Food supply meets the requirement of one week supply of nonperishable and 2 day supply of perishables food on hand. Food prep areas are clean and organized. LPA began review of employee records- Three (3) records were reviewed. LPA reviewed employee record for first aid certification, fingerprint clearance, personnel/job application, health screening and TB test results, criminal record statement, employee rights, training verification, and current administrator certification. CPR and requirements have been met. The facility employs enough staff to maintain cleanliness and meet the needs of the clients in care. Administrator certification is present and current. LPA made observation throughout the inspection process to assess if the facility remains in conformity with the State Fire Marshall regulations. The facility has not exceeded its capacity limitation and the structure remains unchanged according to the approved floor plan; Smoke detectors and carbon monoxide detectors were tested and found to be operational. Fire extinguishers are tested or replaced annually and were last done so on Feb 3, ,2026. The facility is conducting emergency disaster drills. The last disaster drill was conducted on Feburary 26, 2026. Based on the information received during this visit today, there are zero (0) deficiencies is being cited per Title 22, Division 6 of The California Code of Regulations. Allegation investigated: Staff do not ensure resident receives sufficient fluids, resulting in dehydration Staff do not ensure resident's medical needs are being met

Licensing Program Analyst (LPA) Abdoulaye Zerbo conducted an unannounced subsequent visit to the facility to deliver findings for the above allegations. LPA was greeted and granted entrance by Administrator Letiwe Patton. LPA identified himself and discussed the purpose of the visit. It was alleged that Staff do not ensure resident receives sufficient fluids, resulting in dehydration. During the investigation, LPA interviewed the licensee who stated R1 is offered fluids on a regular basis. LPA interviewed a confidential witness who stated the facility is providing excellent care and expressed no concerns regarding staff practices. The confidential witness confirmed that R1’s elevated glucose and dehydration were the result of R1’s refusal to take medication and follow dietary recommendations. Unfounded Interview with R1 revealed that they have plenty of water but only prefers a particular brand of water. It was alleged that staff do not ensure resident’s medical needs are being met. Interviews revealed no evidence that staff failed to meet R1’s medical needs. The confidential witness stated their only concern is R1’s personal refusal to take prescribed medications. The confidential witness emphasized that the facility is “excellent” and that staff have not contributed to R1’s medical challenges. R1’s refusal of care is voluntary and not due to staff neglect. LPA interviewed R1 who stated that their refusal to take medication is voluntary. Based on interviews, observation and records review, the allegations of Staff do not ensure resident receives sufficient fluids, resulting in dehydration, and facility Staff do not ensure resident's medical needs are being met are deemed unfounded. A finding of unfounded indicates that the allegation is false, could not have happened, or is without a reasonable basis. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Abdoulaye Zerbo conducted an announced pre-licensing inspection at the facility and met with Licensee Letiwe Patton Application: The inspection is an initial application for a Residential Care Facility for Elderly (RCFE). The Indio Fire Department approved a fire clearance on 04-17-2025 for five (5) non-ambulatory clients and one(1) bedridden in bedroom one. Buildings and Grounds: The facility is composed of six (6) residents' bedrooms, four (4) bathrooms, a living room, a kitchen area, a detached office and a garage. Water temperature was measured within regulation at 110.5 F. The interior and exterior walkways of the facility were observed to be clutter free with no obstructions present. Smoke and Carbon Monoxide detectors were in working order. There is a pool meeting the department's requirements and there are no weapons stored in the facility. Residents' bedrooms are fully furnished, and privacy is available. Outdoor areas have sufficient room for activities and leisure. A washing machine and dryer were available and in working order. The facility has a working phone witnessed by calling the phone number. Storage and Supplies: Medications, will be stored in a locked cabinet in the hallway, inaccessible to any unauthorized individuals. Staff and residents files will be stored in the office. A complete first aid kit was observed to be available. Cleaning supplies will be stored in the garage. Linens, and equipment appeared to be in good repair and sufficient for the approved census. Fire extinguishers were available and fully charged with an expiration date of December 21, 2026. Continued 809-C... Food Service: Utensils and dishware are sufficient for the requested capacity. The refrigerator and freezer are in working order. Sharps knives will be stored in a locked cabinets in the kitchen area, available only to authorized individuals. Forms: The following signs were observed to be posted at the facility: Personal Rights, Complaint information, Emergency disaster plan, the facility sketch. LPA verified the Administrator's Certification, with an expiration date of January 08, 2027 and CPR certification with the expiration date of December 2026. The applicant is scheduled to complete COMP III on June 10, 2025. LPA determined the facility meets the operational requirements for licensure. The Pre-licensing inspection is complete and has satisfied all requirements in accordance with Title 22, California Code of Regulations.
