Licensing Program Analysts (LPA) Seo Jeon and Aziz Faizi conducted an unannounced annual required visit. Upon entry, LPA was greeted by Marissa Patimo, Caregiver, and informed them of the purpose of the visit. At the time of the visit, there were one (1) staff member and two (2) residents present. Facility Overview: The facility is a one-story home with four (4) bedrooms and three (3) bathrooms, including an attached garage. There are no pools or known firearms on the premises. Infection Control: LPA observed that hygiene and cleaning supplies were available for regular facility maintenance. The facility has infection control plan in file. Physical Plant: The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. The outdoor area was free of hazards. Laundry equipment was in good working condition. Sharp and dangerous objects were securely locked and inaccessible to residents. Both the smoke detector and carbon monoxide detector were operational, and the hot water temperature was 109°F. LPA observed fully charged fire extinguisher in the laundry closet. Food Service: The facility’s kitchen was clean and equipped to prepare food. The facility maintained the required two-day supply of perishable foods and a seven-day supply of non-perishable foods. Continued on LIC809-C.... Care & Supervision/Administration: Adequate staff were present to supervise clients during the visit. The administrator holds a current administrator’s certificate. Record Review and Resident/Staff Files: LPA reviewed files for two (2) staff members, confirming criminal clearances, updated training, and CPR/First Aid certification. Two (2) resident files were reviewed and contained all required documentation. Health-Related Services/Incidental Medical Services: All resident medications were securely locked. LPA reviewed centrally stored medication records for two (2) residents and confirmed that all medications were up to date. Disaster Preparedness: LPA reviewed the facility’s emergency and disaster plan, including disaster drill log. LPA observed that the disaster drill log was not updated. A citation was issued. All facility exits were clear of obstructions. One citation was issued during the visit. Type of visit: Prelicensing

On January 21, 2025, Licensing Program Analyst (LPA) Seo Jeon made an announced visit to the facility for the purpose of conducting a pre-licensing inspection. LPA met with Applicant, Kurt Rosenberg, who accompanied LPA for the inspection. The Applicant has submitted an application for six (6) residents (six non ambulatory and one bedridden). On September 26, 2024, the Riverside County Fire Department approved a fire clearance for which the applicant has applied for. The facility has an approved hospice waiver for four (4) residents. The home is a single story structure consisting of four (4) bedrooms, three (3) bathrooms, kitchen, dining room, family room, garage, backyard with a covered patio, and two (2) locked sheds that are being used for storage. The bedrooms were observed to have bed, lighting, night stand, chest of drawers and area for sitting. The bathrooms had nonskid mats and grab bars. There is plenty of extra linen (sheets, blankets, towels) that were observed to be in good repair. The smoke and carbon monoxide detectors are a dual system that were tested and found to be operable. The hot water temperature was tested and was found to be within regulatory limits measuring at 111 degrees Fahrenheit. The facility is equipped with flashlights, night lights and solar panels. The facility has an emergency disaster plan, dementia plan and infection control training plan on file. The facility has a sufficient supply of dishes, cooking and eating utensils, that were observed to be in good repair. The facility food supply was observed to be sufficient as there was 2-day supply of perishable and a 7-day supply of nonperishable food items. The facility has an emergency food and water supply. There is a fully stocked first aid kit with instruction manual. Continued on LIC809-C.... The passageways, and ramps/inclines are clear and free from obstruction. The home has 1 fully charged fire extinguisher. The facility does not have any known guns or ammunition stored on grounds. The sharps/knives are stored in a locked drawer in the facility kitchen. The medications will be kept in individual boxes that will be stored in a locked cabinet located in hallway. Upon entry to the home in the wall the required postings (facility sketch, resident council, theft and loss policy, personal rights, PUB475 CCL/dept complaint poster and he Long term Care Ombudsman poster) were observed. The facility was observed to have activities to encourage socialization such as, board games, puzzles, and karaoke as well as a covered patio with plenty of outdoor space for walking and physical activities. LPA observed that the physical plant is clean, in good repair, and to be hazard-free during today’s visit. LPA determined the facility meets the operational requirements for licensure. The Pre-licensing inspection is complete and has satisfied all requirements in accordance with Title 22, California Code of Regulations.
