On 05/17/24 Licensing Program Analyst (LPA) Mario Leon conducted a required annual inspection using the CARE Inspection Tool. California Department of Social Services was greeted by staff one Licensee, Jovan Iglesias (S1) and the purpose of the visit was explained. As a part of the inspection, LPA reviewed all 5 client service records. One (1) resident did not have an updated physician's report and one (1) resident did not have any physician's report. One deficiency has been cited. LPA inspected the inside/outside facility grounds along with S1. This home consists of 4 client bedrooms and 3 client bathrooms, living room, kitchen, dining area, activity room, laundry area and patio. There were 5 residents present during the inspection. There were adequate perishable and non-perishable food supply were kept at the home and fully stocked in kitchen cabinets. LPA reviewed facilities' 5-year fire sprinkler system report, noting all fire equipment are working properly. During the inspection, LPA observed the following client rooms: mattresses and box springs in working condition, adequate lighting present, plenty of dresser/closet space is present, and all bed linens present. LPA observed additional stocked bedding and towels in closets. All bathroom fixtures are in working condition. Water temperature properly measured at 116.7°F (degrees F) in bathroom #1. Showers were free of mold/mildew, adequate lighting, and sufficient toiletries were accessible to clients. LPA observed sufficient bedding, linens, and walls and floors were clean and in good repair. During today's visit, there has been one (1) deficiency cited. Please see LIC809-D. Type of visit: Case Management - Annual Continuation

On 05/19/2025 , Licensing Program Analyst (LPA) Deborah Lee conducted an unannounced Case Management Annual Continuation to complete Inspection started on 5/14/25. LPA met with Jovan Iglesias and explained the purpose of the visit facility is licensed to serve a total of 6 adults ages 60 and above, of which 5 may be non-ambulatory residents. There is a hospice waiver for 2. A total of 6 residents are currently residing in this facility. The Annual Licensing Fees are current. Structure/physical plant: This home consists of 4 client bedrooms and 3 client bathrooms, living room, kitchen, dining area, activity room, laundry area and patio. There were 5 residents present during the inspection. LPA observed all required documents posted throughout the facility. The facility has a working landline telephone. Smoke and carbon monoxide detectors were operational. There is a fully charged fire extinguisher located near front of resident. The last emergency drill was conducted on 5/2/25. page 1 of 2 Files: LPA reviewed 5 resident files and found that (5 ) out of (5 ) contained all the necessary documentation. LPA reviewed 4 staff files and found that ( 4) out of (4) contained the required documentation, certification, and training. Liability Insurance expires on 8/16/25 Medications: LPA observed that medications were safe, locked, and inaccessible to the residents. All medications observed were labeled and maintained in compliance with label instructions and State and Federal law. No deficiencies are being cited during today's visit. On 05/14/2025 , Licensing Program Analyst (LPA) Deborah Lee conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with Wilma Iglesias LPA explained the purpose of the visit facility is licensed to serve a total of 6 adults ages 60 and above, of which 5 may be non-ambulatory residents. There is a hospice waiver for 2. A total of 6 residents are currently residing in this facility. The Annual Licensing Fees are current. Structure/physical plant: This home consists of 4 client bedrooms and 3 client bathrooms, living room, kitchen, dining area, activity room, laundry area and patio. There were 5 residents present during the inspection. Kitchen: LPA toured the kitchen area and observed supplies of nonperishable foods for a minimum of one week and fresh perishable foods for a minimum of two days. Knives and toxins were kept in locked storage cabinet. Page 1 of 2 Bedrooms/Bathrooms: All 4 bedrooms were inspected. The mattresses were in good condition, adequate lighting, plenty of dresser and closet space observed. Walls and floors were clean and in good condition. Comforters, bed linen, bath towels and mattress protectors were adequately stocked. Bathroom toilets and water faucets worked properly, grab bars were secure, and a non-skid mat was in place. Adequate lighting and toiletries accessible to residents. LPA tested hot water temperature, and it measured between 105 and 120 degrees Fahrenheit. This facility provides residents with hygiene products such as feminine napkins, soap, toilet paper, toothbrush, toothpaste, and comb. Safety: LPA inspected the First Aid kit and found it contained an ample supply of required items: scissors, tweezers, gauze, disinfectant wipes, band aids. LPA observed all exits to be clear and easily accessible. All toxins locked and inaccessible to residents in care. LPA observed all required documents posted throughout the facility. The facility has a working landline telephone. Due to time restraints, LPA to complete inspection on subsequent visit. There were no deficiencies cited during today’s visit. There were adequate perishable and non-perishable food supply were kept at the home and fully stocked in kitchen cabinets. LPA reviewed facilities' 5-year fire sprinkler system report, noting all fire equipment are working properly. During the inspection, LPA observed the following client rooms: mattresses and box springs in working condition, adequate lighting present, plenty of dresser/closet space is present, and all bed linens present. LPA observed additional stocked bedding and towels in closets. All bathroom fixtures are in working condition, yet located cleaning agents under bathroom 1. Cleaning agents were immediately relocated while LPA was on-site, see LIC9102A. Water temperature properly measured at 108.0 degrees F in the kitchen. Showers were free of mold/mildew, adequate lighting, and sufficient toiletries accessible to clients. LPA observed sufficient bedding, linens, and walls and floors were clean and in good repair. All 3 staff present have obtained criminal clearance, though not associated to the facility. See LIC809-D. During today's visit, there has been one (1) technical violation, 1 technical advisory, and 1 deficiency cited.
